1 Birmingham Adult Mental Health Services Locality Network Brief April 2014 update Commissioning 2014 /15 Negotiations with providers of mental health services are continue, with the status as of April as follows: Birmingham and Solihull Mental Health NHS Foundation Trust. Key issues are: The onward development of the IAPT service; and The overall financial envelope We are working with the Trust to resolve these issues and ensure sign off of the contract by end of April Youth Transitions Service description: Mental health services for children, youths and adults up to the age of 25 (0 25 years) The 12 week Public Consultation completes on April 18 th. Together with CAMHS commissioning colleagues we have visited a wide range of forums and held specific events for stakeholders including a session held for children of school age on a Saturday morning. We will be reviewing all the feedback and amending the service specification and Business plan as required in response to the feedback received. We will then begin the Pre Qualification Process after a follow up session with for providers. The Procurement will occur throughout the summer with a view to a contract award in March 2015 and new service to commence in October This will address the concerns around confusion with transition pathways and encourage early interventions for the full range of mental health issues that young people may present with. The aim will be for this early intervention to produce better outcomes and opportunities for young people with mental health issues in a non stigmatising manner, develop and maintain links with key social determinants of education and employment opportunities to prevent life long chronicity. 3 Primary Care & Community Mental Health Services Continued Birmingham South Central CCG Pilot for Wellbeing Hub Approved South Central CCG s pilot of the Edgbaston Wellbeing hub and spoke approach went live on 1 st April. This is the culmination of ground breaking work with a number of third sector partner agencies to develop a central single point of access that GP s, service users and others can refer to receive access to social support and therapies in the community to aid their mental health & wellbeing. It does not replace the secondary care treatment that people may require but will augment this and give alternatives to medicalised approaches for anyone living in the Network. This is a unique venture as it has a life span approach to mental health including provision for children, families and adults in their own right. We are working with BCU around research and evaluation and will link with the wider Birmingham development of Primary Mental Health wellbeing services to share learning and good practice. For further information, please contact Robert Devlin, Joint Commissioning Manager. robert.devlin@nhs.net Tel: Outreach pilots in Birmingham Cross City & West Birmingham (Sandwell & West Birmingham CCG) We are planning to pilot outreach community support to a small number of practices in Birmingham Cross City and some practices in the West Birmingham area of Sandwell & West Birmingham CCG. The pilots will support patients with low level mental health issues undertake a person-centred assessment and support patients by guiding them to other service provision within the community, ensuring that their holistic needs are met. We are currently in discussion with providers and GP practices to set these pilots up and the evaluation will also ink with the wider development of the Primary Care & Community Mental Health redesign to share learning and utilise good practice. 4 Third Sector The CCG funded third sector contracts have been renewed as planned with no change to existing commissioned services. The deflator level has been reset to 1.5% for contracts above 50,000 in value, which is in line with the deflator value agreed with Birmingham and Solihull Mental Health Trust Manager lead(s): Baljit Bahi, Joint Commissioning Manager Tel: or Andy Simpson, Commissioning Support/Contracts Manager Tel: Dementia/ Dementia Strategy Give me something to Believe in the Dementia Strategy for Birmingham and Solihull will be formally launched on 19 th May 2014 at Birmingham City Football Club pm at a event led by Angela Rippon, a Dementia Care Ambassador for the Alzheimers Society who has personal experience of caring for someone with dementia, and Dr Patrick Brooke, Chief Officer for Solihull CCG and Chair of the Dementia Sub- Group. Stakeholder engagement around the strategy is now underway as part of the consultation plan with discussions at local commissioning networks and patient engagement events. This will lead on the annual review of the dementia strategy as well as being a forum to share information on good practice and comment on local services. 5 Street Triage pilot From January 2014 funded for 12 months by the Home Office The Street Triage scheme is funded by the Department of Heath for 12 months, and brings together police, ambulance and BSMHFT to provide a joint response to people experiencing a mental health crisis on the street. The aims of the scheme are to improve the individual experience of people undergoing a mental health crisis; to improve the pathways of care; to divert people where appropriate from the Place of Safety (PoS), A&E or police custody; and to reduce the amount of time spent by police and ambulance services in coordinating their responses. The scheme began operations on 10 January It consists of a police officer, West Midlands Ambulance paramedic and a CPN in an unmarked ambulance car. It covers Birmingham and Solihull local police unit areas. The operating hours are 10 a.m. through to 2 am (Monday-Thursday) and 10 am through to 3 am (Friday- Saturday). It takes calls received by either police or ambulance where they would normally have looked to send police officers and/or an ambulance to the scene. The street triage car provides the initial response. The staff can combine checks from health and police records to give stronger background information. The CPNs are managed via the Trust s Place of Safety, and can therefore ensure that only those who are likely to need admission to hospital are brought to the PoS. The scheme s governance arrangements consist of operational governance by the managers of the three parts of the service, and a strategic governance board which additionally includes commissioners and patient organisation representatives. Initial Data The data from 10 th January to 31 st March shows that street triage dealt with a total of 595 incidents. Some 80% were for people already known to mental health services, and these could be diverted to Community Mental Health Teams, or other mental health services. 6 Adults with Autism guide for GPs The Autism Strategy in Birmingham has put in place a mechanism by which patients can get assessed and diagnosed with Autism. A Guide for GPs in Birmingham has been produced which was launched on 10 April and is due to be distributed to GP practices in the next few weeks. We have 2 types of assessment available: Standard Autism Assessment undertaken by an independent consultant psychiatrist with experience in diagnosing autism. Suitable for most people who are able to accurately report their needs and or who are not severely affected by their possible autism. Complex Autism Assessment undertaken by a multidisciplinary team comprising of consultant psychiatrist, clinical psychologist and speech and language therapist. Suitable for people who have marked speech and language delay/difficulties, complex psychological presentations or where possible autism presents a major difficulty for them. Referral Process Should a GP wish to proceed with a formal assessment for Autism the process would be to ask the patient to complete an AQ assessment (attached to this brief or available to download from The test then needs to be scored, (if you are getting the patient to score themselves please do not let them know what the required cut off score is). If the patient scores over a score 32 then this would be indicative that the person may have autism and a formal assessment can be considered. A score of under 32 would indicate autism is not present. If your patient scores over 32, they should be flagged using read code 1J9 and can be referred for a diagnosis. Please send a full letter of referral to the Birmingham Joint Mental Health Commissioning Team at autismassessment.referrals@nhs.netasking to arrange an assessment. In your referral can you please: 1. Explain what has prompted the referral and why the person wishes to go for an assessment 2. Clearly state your assessment of the medical, psychological and social impact on this person of receiving a diagnosis 3. Give details of any comorbidities and medical conditions 4. Give details of all involvement that the person has had with any services 5. Attach any relevant reports from paediatric services, educational psychology and a copy of their AQ assessment 6

7 Adults with Autism guide for GPs Continued In addition If you could indicate, based on your assessment of the patient if you feel that person would need a standard or complex assessment I would use this to help triage the referral to the appropriate service.
