We are dedicated to the ogoig ehacemet of safe, ethical ad competet utritio services provided by Registered Dietitias i their chagig practice eviromets. Supportig ad moitorig competece throughout a Registered Dietitia s career is a core compoet of the College s missio. It is essetial i meetig the orgaizatioal visio of esurig that the people of Otario ca be cofidet that the College demostrates regulatory excellece i the public iterest. For the past three years, I have bee a member of the Quality Assurace Committee. My participatio i this committee has give me greater isight ad appreciatio for the importace of esurig cotiued competece as a practisig RD. The Quality Assurace Program tools implemeted by the College have evolved cosiderably over the spa of my career. My ivolvemet i the Quality Assurace Committee has sigificatly trasformed my perspective towards these tools ad processes. I particular, I have gaied a appreciatio for the value of the Self-Directed Learig Tool (SDL Tool). I recogize that this statemet may lead to a collective groa from RDs but I must admit that the activity of critiquig the SDL Tool submissios of my peers has bee particularly helpful i ehacig my ow skills i developig learig, utritio care, ad career-related goals. Reflectig upo my practice ad relatig my aual self-directed learig goals to the performace idicators ad associated competecies idetified withi this tool has helped focus my learig plas. The process of reviewig SDL Tools has also helped me to employ clear ad cocise laguage to covey desired learig outcomes that are relevat, meaigful ad tagible. The Quality Assurace Committee is also ivolved i the developmet, evaluatio ad revisio of may of the competecy assessmet tools. The work of the Committee frequetly icludes spirited discussio about the stregths ad limitatios of the tools ad processes themselves. This exposure has helped me recogize the thoughtfuless ad skill which is applied whe evaluatig ad revisig competecy activities. I also appreciate that assessig competecy is a challegig task, ad I do respect that there is some flexibility icorporated ito the College s competecy assessmet processes to allow for applicability to a wide variety of practice areas. Aother iterestig compoet of cotiued competece that has frequetly arise is that of advaced level versus etry level practice skills. Although we curretly do ot have criteria to desigate advaced level competece, it is evidet that the College ad our members alike share the commo goals of advacig dietetic kowledge ad skills to support professioal growth. 3 The Complexities of Coset Last fall, our practice advisors travelled aroud Otario for our aual workshop series speakig to over 700 members o the topic of coset. While we were workig o the presetatio, discussig the issues ad developig the scearios, it became clear that this would be a iterestig ad challegig topic. Melisse L. Willems, MA, LLB Registrar & ED The issue of coset, particularly coset to treatmet, seems relatively simple at first: explai the treatmet, get coset, ad go ahead. It s ot so simple, though, whe you start to talk about ucoscious patiets, distressed parets, emergecy rooms, iter-facility trasfers, multiple caregivers, the desire to do good, the desire to do o harm ad the right to decide. How these all itersect (ad, i some cases, they do all itersect) is ot so simple. We deliberately chose difficult scearios to tease through these issues. We wated the workshops to be as egagig ad istructive as possible. We kew that a cursory review of the legislatio ad re-hashig of coset 101 would t be eough. I preparig for the workshops, we worked through the scearios, the subtleties ad the what-ifs; a umber of you were key i sharig your thoughts ad experieces to help us do this. This oly part of the work. With each workshop, we icreased our uderstadig of the realities RDs face whe obtaiig coset. We leared that the coset process is ofte iheret ad ivisibly itegrated ito the patiet-practitioer relatioship. We explored together the importace of obtaiig iformed coset, how this is critical to the digity of cliets ad at the heart of cliet-cetered care. Participats worked with the ew Stadards of Coset by applyig them to practice scearios ad situatios from diverse dietetic practice areas ad settigs. Below are additioal aswers to questios which were asked by workshop participats. HEALTH CARE CONSENT ACT, 1996 Is a siged coset form the same as obtaiig iformed coset for treatmet icludig assessmet? RDs sometimes cofuse a siged coset form with obtaiig iformed coset. Obtaiig a iformed coset is a process that ivolves a coversatio whereby the cliet the clearly uderstads the reasos for the treatmet, the process, the risks ad the outcomes expected. The siged coset form is the cofirmatio that the coversatio took place ad that the cliet cosets to the dietetic treatmet proposed by the dietitia. Coset to treatmet does ot always eed to be obtaied i writig or eve cofirmed orally. I most routie assessmets, cliet coset may be implied. If a particularly risky itervetio is recommeded, the a writte coset form may help a dietitia esure that a proper coset was obtaied. The coset form should be simple ad easy to uderstad to avoid cofusio as much as possible. Writte coset forms are ot ecessarily a defece to a allegatio of failig to obtai coset. A cliet ca still claim that the form was ot clearly explaied before their sigature was obtaied, or that they did ot uderstad or appreciate what was siged. For orgaizatios, coset forms should refer to the orgaizatio's privacy policy, with clear idicatios of why persoal health iformatio is collected, how it will be used ad uder which circumstaces it might be disclosed to aother party, if at all. Whe do I eed a cliet s coset to disclose persoal health iformatio? Coset is almost always required for every collectio, use or disclosure of persoal health iformatio. Uder PHIPA, there are circumstaces that permit or require RDs to disclose cliet persoal health iformatio without coset. The complete list of whe iformatio ca be disclosed without coset is o the website of the Iformatio ad Privacy Commissioer of Otario. Results revealed that Dysphagia maagemet was oe of the highest risk activities idetified by RDs i their practice. Addressig areas of high risk i dietetic practice is importat to help the College fulfill its public protectio madate. The Alliace of Caadia Dietetic Regulatory Bodies established the 2017 Competecies for Dysphagia Assessmet ad Maagemet required for safe, ethical, ad effective practice i dysphagia-related care. The process to establish the ew Competecies icluded coductig a evirometal sca of existig dysphagia competecies ad cliical practice stadards, cosultig RDs, other health professioals, ad Dietitias of Caada represetatives. The competecies were validated via a atioal survey. NEW 2017 DYSPHAGIA COMPETENCIES ENDORSED I March 2017, the College Coucil edorsed the ew Dysphagia Competecies, which defie the expectatios for safe, ethical, ad effective dysphagia maagemet. The dysphagia competecy statemets do ot stad aloe ad are ot a protocol. Buildig o the Itegrated Competecies for Dietetic Educatio ad Practice, 1 they set out additioal performace idicators for dysphagia assessmet ad maagemet. They idetify the specific kowledge ad skills required for screeig, coductig a cliical (bedside) swallowig assessmet, ad for participatig i a istrumetal swallowig assessmet. Performace idicators are ot repeated i each sectio, as each sectio builds o the previous. Dysphagia maagemet was oe of the highest risks activities idetified by RDs i their practice. RDS ARE ACCOUNTABLE TO PRACTICE DYSPHAGIA MANAGEMENT SAFELY RDs wishig to perform ay task or fuctio related to dysphagia have a duty to assess ad evaluate whether they have the persoal expertise or competece to do so safely ad effectively, both from the professioal ad public protectio poits of view. They must have the appropriate educatio, practical traiig ad metorship to provide safe, competet dysphagia maagemet. The Dysphagia Competecies apply to RDs who are gaiig experiece i this practice area as well as those with expertise. RDs are accoutable to practice dysphagia assessmet ad maagemet accordig to their provicial regulatory stadards ad college policies. Some RDs offer these value-added services to their cliets who request them. Others work for orgaizatios who offer such services to cliets throughout Caada ad i other coutries. As a result, the College is receivig icreasigly more calls from RDs askig for guidace about telephoe ad web-based cousellig. Curretly, the College does ot have specific stadards or policies surroudig telephoe or web-based cousellig. However, priciples of safe, ethical ad competet dietetic practice still apply to cousellig over the phoe ad the web. EXERCISE PROFESSIONAL JUDGEMENT RDs eed to determie whe it is ad whe it is ot appropriate to offer telephoe or web-based cousellig. There may be some istaces whe it would beefit a cliet to see the RD i-perso, at least oce, to establish rapport. I some cases, this may ot be possible ad the oly way for a cliet to have access to RD services might be by phoe or olie. With telephoe or web-based cousellig, RDs ormally rely o self-reported iformatio from the cliet ad eed to exercise the same professioal judgmet whe coductig a assessmet olie as they would i-perso. Services offered via Skype or some other web-based platforms, would help a RD address observable cocers (e.g. weight loss/gai, fluid retetio, ski itegrity, etc.). I the cotext of iterprofessioal practice, there are times whe other healthcare providers recommed utritio treatmets that coflict with a RD s professioal opiio. 13 Q UA L I T Y A SSU R A N C E P R O G R A M The Quality of Goal-Settig i the SDL Tools is Improvig Barbara McItyre, RD Quality Assurace Program Maager barbara.mcityre@collegeofdietitias.org 2016 RESULTS OF THE SELF-DIRECTED LEARNING (SDL) TOOL SUBMISSIONS All members are expected to submit their SDL Tool with their reewal iformatio each year. The College has a process i place to radomly scree 2.5% of the SDL Tools to make sure that members have completed them as required ad that the learig goals stated are SMART (Specific, Measurable, Achievable, Realistic ad Timely). SDL Tools that do ot meet the requiremets are referred to the Quality Assurace Committee. Whe members are required to resubmit their learig goals, it is usually because they are ot specific eough or do ot have a outcome which shows how it will ehace their dietetic competece. Whe learig goals are resubmitted ad still foud to be iadequate, members are required to atted a webiar o Professioal Goal Settig. Though may members were required to resubmit their goals this year, we were pleased to ote that, overall, the quality of the goals has improved over the last 5 years. Whe we first started to review goals five years ago, members were developig goals such as Clea Filig Cabiets or Improve chartig. We o loger see that type of goal. Now, the goals are SMART ad more directly related to improvig dietetic competecy. SDL Tool Results 2016 Total Reviewed 217 # Completed Adequately 97 (45%) PEER AND PRACTICE ASSESSMENT RESULTS FOR 2016 The Peer ad Practice Assessmet (PPA) is a 2-Step Process: Step 1 ivolves a multi-source feedback method (ofte called 360 º feedback) to gather iput from peers, colleagues ad patiets (if applicable); ad Step 2 ivolves a behaviourbased iterview coducted by a peer assessor who is a experieced dietitia. A chart review is icluded if the dietitia provides direct patiet care. Dietitias whose results were below established orms proceeded to Step 2. We ofte hear that members did ot receive a , or that they thought a remider was ot for them so they deleted it. It is your resposibility as a member to make sure that the College has your curret cotact iformatio so that you receive ad read all College commuicatios. You will be held accoutable for your obligatios as a registered professioal eve if you do ot receive or read a . Here are five tips we hope will help members maage their s to meet their obligatios o time. 1. Make sure College s are ot o a list of spam or low priority s May strategies for maagig s to save time iclude tips o filterig or skimmig your s to remove spam or low priority messages. Be careful ot to iclude College s o your list of spam or low priority s. Members are resposible for esurig that their cotact iformatio is updated ad that they read all College commuicatios. AMENDMENT TO THE COLLEGE FEES BY-LAW NO. 2 Coucil approved chages to By-Law 2, which were required to support the implemetatio of the ew Practice-Based Assessmet (PBA), the secod step i the ew Prior Learig Assessmet ad Recogitio (PLAR) process for assessig the educatio ad traiig of iteratioally educated applicats. The chages icluded removig from the by-law the registratio fee of $400 for the KCAT because this fee is ow admiistered exterally ad addig the registratio fee of $2300 for the PBA. The proposed chages were approved, i priciple, at the December 2016 Coucil meetig ad circulated to members for 60 days, from Jauary 13 to March 13, SEXUAL ABUSE PREVENTION PLAN Coucil approved the College s Sexual Abuse Prevetio Program. The Sexual Abuse Prevetio Program is desiged to comprehesively state the strategies, policies ad programs for the prevetio ad hadlig of sexual abuse that the College has i place to protect the public. I the cotext of this pla, Coucil also approved that a miimum period of oe year be required before a cosesual sexual relatioship is permitted betwee a dietitia ad a former cliet. I 2014, the Miistry of Health ad Log-Term Care (the Miistry) created the Task Force o the Prevetio of Sexual Abuse of Patiet ad the Regulated Health Professios Act, The Task Force submitted its report i September 2016 with several recommedatios which icluded defiig clearer boudaries to prevet sexual abuse. Oe of the recommedatios was to set a miimum period of at least oe year before a health professioal could have a romatic relatioship with a former cliet. DRAFT BOUNDARY GUIDELINES Coucil approved, i priciple, the Draft Boudary Guidelies for Registered Dietitias i Otario, for circulatio to College members. Although the College had several excellet resources regardig boudary issues, we did ot have comprehesive boudary guidelies i oe documet. The ew documet fills this gap. PROFESSIONAL PRACTICE STANDARDS AND GUIDELINES: CONFLICT OF INTEREST Coucil approved the Professioal Practice Stadards ad Guidelies: Coflict of Iterest. The Coflict of Iterest Stadard was circulated to members for commet from Jauary 10 February 10, Feedback from the cosultatio was icorporated after beig reviewed by the Legislative Issues Committee (LIC) at the February 2017 meetig. 18 DYSPHAGIA COMPETENCIES Coucil edorsed the proposed 2017 Competecies for Dysphagia Assessmet ad Maagemet i Dietetic Practice (see p. 6 for more details). The ew 2017 Dysphagia Competecies are the results of a workig group established by the Alliace of Caadia Dietetic Regulatory Bodies (the Alliace), to idetify the competecies required for safe, ethical, ad effective practice i dysphagia-related care. College Staff participated i this work. These competecies relate to the College s strategic goal of havig A Robust Regulatory Framework for the Quality ad Safety of Dietetic Practice, ad also may be used i discipliary proceedigs ad for quality assessmets, such as a practice-based assessmet. PROFESSIONAL PRACTICE STANDARDS: RECORD KEEPING Coucil approved, i priciple for the purpose of cosultatio, the proposed Professioal Practice Stadard: Record Keepig for Registered Dietitias i Otario. COUNCIL ELECTION RESULTS This year, electios took place i Districts 2 ad 4. O Jauary 19, 2017, electio otices were set to geeral members by ad posted i the ews sectio of the College website. The omiatios period was closed o February 17, 2017, at 5pm. There was oly oe omiatio for each district. Therefore, the members omiated for Districts 2 ad 4 were elected by acclamatio.
