3 ENTRY SUBMISSION All omiatios must be submitted i the requested format o later tha 12:00 AM EST (USA), Jue 23, is the preferred method of receipt. Sed to The SQF Istitute at awards@sqfi.com. All omiatios must iclude the evidece as outlied. Icomplete submissios or those that do ot follow the submissio rules will ot be evaluated. PRIZES We will hoor the top fialists from both categories durig the 2018 SQF Iteratioal Coferece, where award wiers (oe i both categories) will receive a crystal award. The award wiers will receive complimetary registratio to the 2019 SQF Coferece. All fialists will receive a reduced rate registratio to the 2018 SQF Coferece. The award wiers will be recogized through a variety of SQFI marketig ad advertisig vehicles the etire year followig. SCORING CRITERIA Etries will be scored o overall cotet ad the omiee s demostrated achievemets i: Cotributig sigificatly to achievig the SQFI Missio. Commuicatig the goals, missio ad beefits of the SQF program to both iteral ad exteral stakeholders. Provig his/her uderstadig ad commitmet to the SQF program. Demostratig a ogoig persoal commitmet to improvig the awareess of food safety withi the global commuity. RULES & ELIGIBILITY All omiators ad omiees must be affiliated with the SQFI program. Examples of appropriate affiliatios iclude, but are ot limited to registered auditors, cosultats or traiers, licesed certificatio bodies, registered ad certified suppliers, licesed traiig ceters, etc. All omiees must have show ogoig ad dedicated support for the Safe Quality Food Istitute Program durig the 2017 ad/ or 2018 caledar year. The etry form must accompay your omiatio ad all sectios must be filled out legibly. Narratives must be 1,000 words or less. Nomiators must obtai permissio from the omiee prior to submittig a omiatio. Self-omiatios will be accepted. Specific evidece ad supportig documetatio must be icluded for all commets ad accolades give by the omiator o behalf of the omiee. Examples of supportig documets iclude published articles, idustry awards, ews articles ad 3rd party testimoials. These materials will ot be retured to you. No videos will be accepted. JUDGING CRITERIA Oly complete, legible etries will be cosidered. Icomplete submissios will NOT be cosidered. Etries will be scored ad judged by a pael cosistig of a maufacturer/distributor/ primary producer, past SQFI Award wiers, ad a executive from a food retailer. Fial award decisios will be made by members of SQFI for each specific award category. Fialists will be otified o or before August 15th, All fialists will be reviewed to esure they are i good stadig with SQFI.

The omiatio submissio for this award must iclude: q Completed applicatio form. q Cofirmatio that the omiee is a curret, registered auditor i good stadig with SQFI. q A executive summary (1000 word maximum) that illustrates how the omiee demostrates leadership, acts as a metor ad geerally strives to improve the auditig credetial ad promotio of auditors. q Evidece of traiig activities to promote professioal developmet ad demostrate cotiuous improvemet withi the idustry. q Examples of how the omiee works withi as well as outside the orgaizatio to drive food safety ad quality. q Maximum two letters of referece with oe referece from a certificatio body. All refereces shall iclude the cotact ame, address ad phoe umber for verificatio purposes.
