3 2 NEWS HEADLINES Polio spread feared over mass displacement The News, 24 th June 2014 Dengue: Mosquito larva still exists in Pindi The Express Tribune, 23 rd June 2014 Lack of coordination hampering vaccination of NWA children Daily Dawn, 23 rd June, 2014 Delayed security nods affect polio drives in city Daily Dawn, 23 rd June, 2014 DETAILS ISLAMABAD: The World Health Organisation (WHO) has feared the spread of polio virus from North Waziristan Agency to other parts of the country due to the ongoing mass displacement triggered by the launch of a military offensive against militants there.unvaccinated children are moving out in large numbers of the restive tribal area, where anti-polio vaccination was suspended in 2012 due to threats from the Taliban.Most of new polio cases are reported in North Waziristan and thus, turning out to be a real spoiler for the nationwide fight against the crippling virus. Though the mass displacement offers an opportunity to vaccinate thousands of children, who were out of reach of polio teams for more than two years, the threat of polio expansion is real as people displaced from there will mix with host communities in other parts of the country, said WHO coordinator on polio eradication in Pakistan Dr. Zubair Mufti.At the start of this month, the government had made it mandatory for all local residents, both adults and children, to carry vaccination certificates for flying abroad. The move came shortly after the World Health Organisation announced travel guidelines for Pakistan, Syria, Cameroon and many other countries to check spread of poliovirus internationally. RAWALPINDI/ ISLAMABAD: A health official has cautioned that the dengue mosquito larva still existed in some areas of Rawalpindi, urging to make concerted efforts to raise awareness about the virus. Rawalpindi DHO Dr Khalid Randhawa said on Sunday that the larva has been found in different localities recently which could cause dengue among the area inhabitants.he said that larva was found in Rata Amral, Hazara Colony, Dhoke Mangtal, Fouji Colony, Bangash Colony and Dhoke Naju, located near Pirwadhai bus terminal.he said that the larva was found during indoor surveillance. He advised the people to adopt precautionary measures by avoiding breeding ground to the virus. PESHAWAR: Lack of sufficient staff and proper coordination between the Khyber Pakhtunkhwa health department and administrations of southern districts are hampering immunisation of the displaced children arriving from North Waziristan Agency. Officials said that all children coming from North Waziristan Agency should be vaccinated on their arrival by deploying sufficient staff in Bannu and other districts. We need to repeatedly vaccinate the children who have not been vaccinated since June Their immunisation is of utmost importance because there is a severe outbreak of polio in North Waziristan, a vaccinator said. North Waziristan has recorded 50 of the 83 polio cases in the country so far. An estimated 160,000 children have been unvaccinated since Taliban banned immunisation 20 months ago. These children posed serious threats to the children in host communities in different cities of Khyber Pakhtunkhwa. The health department in Bannu would earlier give oral polio vaccine (OPV) to children under 10 years, but three days ago it had launched general vaccination of people regardless of age on the recommendations of the World Health Organisation.The officials said that the health department hadn t yet appointed a full-time deputy director of Expanded Programme on Immunisation (EPI) after suspension of the former a week ago, which was also one of the factors hampering the vaccination programme of displaced people in the southern belt. It is the best opportunity to immunise the people here because they are going registration points and fixed routes, they said. Shortage of staff with the health department in southern districts especially Bannu which had received a bulk of the uprooted people is another problem. There is no additional staff with the district health officers in these districts and the existing employees have been giving OPV to the people. The DHOs in southern districts have deployed most of the health workers in medical camps set up for IDPs, making it humanely impossible to deploy vaccinators for longer periods at the registration points. We want to vaccinate the displaced persons before their entry into Khyber Pakhtunkhwa, the officials said. The upcoming Ramazan and hot weather could make matters worse for the health department which will not be able to reach all the people in southern districts and elsewhere.officials in Bannu said that the health department should coordinate with the Pakistan Army to ensure that all the displaced people were administered vaccines on their way from Waziristan. They also want the directorate of health, Fata, to coordinate with the army and KP health department to ensure vaccination of all displaced people to prevent transmission of poliovirus to population in the province.health officials in some areas said that there was a complete coordination between them and the Pakistan Army to ensure immunisation of children. They said that the army had informed the health department of new families arriving in Lakki Marwat, who were immediately vaccinated. We should immunise the people frequently because poliovirus is in full circulation in the areas from where they have arrived, said a health worker. KARACHI: Authorities in the provincial health department find it hard to go with their plans to administer polio booster, especially to the children who had potentially missed out earlier campaigns because of being residents of sensitive neighbourhoods, for they received no goahead signals from the officials who provide them security, it emerged on Sunday.Some senior officials hinted at the fact that they had planned another two-day phase of polio campaign in the city from Tuesday (tomorrow), but they were still waiting for a nod from the security officials to be adequately prepared for the day. We have plans for a campaign in the city from Tuesday in which we ll target the sensitive areas in particular. However, we have not got a signal to go ahead with our plans from the police and other law enforcement agencies, said a senior official among those overseeing health-care system in Karachi.Similarly, certain officials in the expanded programme on immunisation (EPI) in Sindh had little knowledge of the plans to combat the crippling disease that caused travel sanctions on the country by the World Health Organisation (WHO) for Pakistan s staggeringly huge share in polio cases

4 3 Combating dengue: Fumigation carried out in rural areas Express Tribune, 22 nd June 2014 U.A.E. polio campaign vaccinates 2.5 million children in 21 areas in Pakistan Emirates News Agency, 21 st June 2014 Children suffer as Pakistan battles measles epidemic detected worldwide this year.the EPI is engaged in administering oral polio vaccine (OPV) to international passengers at airports and in government hospitals. Certificates are also issued to the people who intended to travel from Pakistan in four weeks time.the authorities do not allow for polio campaigns in Karachi for the fact that volunteers have repeatedly come under attack in Gadap and other neighbourhoods, compelling the authorities to suspend the immunisation campaigns quite often.while the Sindh government initially ordered the law enforcement agencies to provide security to polio workers during such campaigns, a strategy was later worked out to provide better security to volunteers and ensure the maximum coverage during anti-polio campaigns. On Jan 21, 2014 three polio workers, including two women, were killed in Qayyumabad. Following the deadly attack, the Sindh government had decided that motorcycle riding be banned for eight hours in 24 union councils of Karachi on Sundays for the safety of the vaccinators participating in the anti-polio drive in these areas during these hours.for three months or so the special polio campaigns were run in selected areas of the city on Sundays with proper security cover. The city, which has recorded six cases till the middle of 2014, had seen eight victims last year. Pakistan now carries a huge burden of 82 polio cases out of more than 100 worldwide. ISLAMABAD: Fumigation has been carried out at 300 places in the capital s rural areas including schools, markets, roads and streets during the ongoing fumigation drive. District Dengue Control Cell In-charge Dr Muhammad Najeeb Durrani said the district health department had constituted six anti-mosquito teams which are conducting fumigation in the union councils of Rawat, Bhara Kahu, Sihala, Tarlai and Shah Allah Ditta. Sanitary inspectors and lady health supervisors taking part in the anti-dengue drive have been imparted training on modern lines. Lady health workers are also visiting door to door and distributing pamphlets in various communities for raising awareness about preventive measures against dengue.the main focus of the drive is to mobilise the community through a public health education campaign. The department is also arranging lectures at the public places about preventive measures against dengue and stressing the need for a clean environment.the administration has also asked the staff of union councils to ensure cleanliness in the area and timely disposal of solid waste. The ongoing drive was started on June 15 as part of preventive measures against dengue. ISLAMABAD : Two and a half million children in Pakistan were vaccinated last week against polio in 21 areas in Khyber Pakhtunkhwa province and tribal regions, in the second week of a U.A.E. mass campaign launched by the U.A.E. Pakistan Assistance Programme (UAE PAP), in implementation of the directives of President His Highness Sheikh Khalifa bin Zayed Al Nahyan, and as part of the initiative of His Highness General Sheikh Mohamed bin Zayed Al Nahyan, Crown Prince of Abu Dhabi and Deputy Supreme Commander of U.A.E. Armed Forces.U.A.E. Pakistan Assistance Programme said between 7-19 June, nearly five thousand teams had worked intensively in nine areas in Khyber Pakhtunkhwa province, and in 12 areas in tribal regions to vaccinate more than two and a half million children aged under five years against polio as part of the campaign using health centres at local communities and mobile units to reach remote areas.the three-month U.A.E. polio vaccination campaign which targets more than 3.6 million children in 24 areas in Pakistan is being carried out in cooperation with Pakistan's army, Pakistan's ministry of health, representatives of World Health Organisation (WHO) and Pakistan's regional governments. On 24th April 2013, one day ahead of the Global Vaccine Summit in Abu Dhabi, His Highness General Sheikh Mohamed bin Zayed Al Nahyan, Crown Prince of Abu Dhabi and Deputy Supreme Commander of U.A.E. Armed Forces, pledged a donation of AED440 million to support global efforts to eradicate polio by Abdullah Khalifa Al Ghafli, Director of the U.A.E. Pakistan Assistance Programme, described the polio vaccination campaign as "an exceptional success" that further recognises U.A.E.'s humanitarian efforts under the wise leadership of President His Highness Sheikh Khalifa bin Zayed Al Nahyan, and proves that the country's initiatives to assist poor countries facing bad humanitarian conditions contribute to the effective building and development of communities for a brighter future. "The campaign is the latest addition to U.A.E. leadership's humanitarian initiatives to protect children worldwide from diseases and epidemics." He underscored the initiative of H.H. Sheikh Mohamed bin Zayed which contributes to efforts to eradicate polio in one of the world's main hotspots. Al Ghafli noted that the ongoing U.A.E. polio campaign has overcome geography which posed a big challenge as all previous local and international polio vaccination campaigns failed to reach these areas, which accounted for 95% of Pakistan's polio cases in the past six months, resulting in wider spread of the disease into other areas.sound planning and well-organised execution contributed to the success of the campaign which received wide response from the locals who affirmed their confidence in U.A.E. and the humanitarian initiatives launched by its leadership for the good of their children. The locals of the targeted Pakistani areas who expressed their thanks and appreciation for all the efforts of H.H. Sheikh Khalifa bin Zayed Al Nahyan, and H.H. It has drawn unprecedented response" LAHORE: In the intensive-care ward of Lahore's Mayo hospital, pale, spotty children cry in the intense heat of the Pakistani summer -- victims of a devastating measles outbreak.punjab, Pakistan's most populous province of which Lahore is the capital, has borne the brunt of the epidemic. It has 18,000 of the 25,000 cases reported around the country by

5 4 The Nation, 20 th June, 2014 provincial health authorities and more than all children -- of the 495 deaths.the province is the country's richest and most developed, but health experts say ineffective vaccination programmes and poor monitoring of the disease have led to the contagious disease running rampant. "Initially people were unaware about this epidemic, so they tried to treat the disease at home and using local street doctors," doctor Iftikhar Mirza, a spokesman for Mayo, Lahore's main public hospital, told AFP. "They were even unaware about the vaccination. So, when they came to us, the children were in a critical condition and many had already died." Health dept starts registering IDPs to halt polio spread Daily Dawn, 19 th June, 2014 Asad Abbas, a labourer, tried to treat the disease, spread by droplets from the nose or mouth of infected people, with traditional methods and sought proper medical treatment only just in time. "The red spots appeared on my six-year-old grandson's body some days back. We took it lightly and got him treated from a homeopathic practitioner," he said. "Then he started vomiting and his energy vanished. When we brought him here, he was about to die. But after treatment over here, he is okay now." Doctor Muhammad Younas, an official from the Directorate General of Health in Punjab, told AFP the province had seen 17,985 measles cases this year, with 158 deaths. "This is the worst situation in five years and we can confidently say that the number of these cases is much higher than during the previous five years," he said. Last year there were 310 measles deaths in Pakistan, according to the World Health Organization, described in January by a health ministry official as "a record high". The WHO uses a more conservative count of cases and deaths, but even by its measure 2013 is on track to be far worse than Since the start of the year the WHO has recorded 12,951 measles cases and 290 deaths across Pakistan -- compared to 14,984 cases and 310 deaths in the whole of last year. The WHO says three consecutive years of severe flooding, which put Pakistan's health system under severe strain, have helped measles cases rocket from 4,321 in 2010 to the current alarming figure.pakistan has a network of vaccination centres for mothers and newborns, and issues vaccination cards for children who are born in hospitals or seen by doctors after birth.but many women -- like Asad Abbas's daughter -- give birth at home and do not see a doctor either during or after pregnancy.efforts to eradicate polio in Pakistan have been hampered by longstanding conspiracy rumours that the vaccination droplets contain a sterilising chemical as part of a Western plot to reduce the Muslim population. There has also been opposition from militant Islamists who see the vaccination drives as cover for espionage, and numerous health workers on polio campaigns have been gunned down in recent months.the United States fuelled the conspiracy theories by using a hepatitis vaccination drive in the Pakistani city of Abbotabad as cover for gathering intelligence on the whereabouts of Al-Qaeda founder Osama bin Laden. Measles programmes have not been affected by these problems, but even where families are aware of the dangers of the disease, efforts to get children immunised have struggled with youngsters not always showing up for jabs. The measles vaccine is cheap -- less than a dollar per child according to the WHO -- but two shots are recommended to prevent outbreaks, as around 15 percent of children do not achieve immunity with a single dose. An official with the UN Children's Fund in Punjab said missed vaccinations had built up over the years to create the current outbreak. "Some children could not be immunised even after vaccination and many others have issues of malnutrition, so all these cases accumulate after five years and the disease turn out as an epidemic. This is what has happened this time," the official told AFP on condition of anonymity. Jaffer Ilyas, a spokesman for the Directorate General of Health in Punjab, said the province had vaccinated 2.6 million children in response to the epidemic.punjab Chief Minister Shahbaz Sharif has declared a "war against measles" and the province hopes to have the outbreak under control within weeks, Ilyas said. RAWALPINDI: On the directives of the Punjab government, the health department has started registration of families, especially children, coming from North Waziristan to administer anti-polio drops in the light of the military operation. There are reports that families from North Waziristan are arriving in Rawalpindi and Attock districts. There is a fear that the children might bring polio virus with them, so the government has started registration of all such families to vaccinate their children up to five years, said Executive District Officer (EDO) Health Dr Zafar Iqbal Gondal, while talking to Dawn.He said although the transit camps have been established at entry points of the two districts, there was a need to take extra precautionary measures, as summer is the ideal season to for the transmission of polio virus.he said a total of 195 teams have been formed for the district, at union council level. Each team comprises of vaccinators and community development officers, and they will keep an eye on the arrival of newcomers in the area, he said. This is essential if we are to achieve our goal of stamping out the disease, said Mr Brende. CEO Seth Berkley of GAVI is in Oslo to meet the Foreign Minister and sign the agreement. The international community is at a pivotal moment in its efforts to eradicate polio. Norway s support to the GAVI Alliance is critical in our efforts to strengthen routine immunisation and contribute to the polio end-game strategy, both of which will protect future generations of children, said Mr Berkley.

14 13 DANGER: UNTRAINED VACCINATION TEAM AHEAD Express Tribune Article June Over the past three weeks, hundreds of children in Khyber-Pakhtunkhwa (K-P) have fainted after reportedly having a reaction to the measles vaccine while nearly 10 children have died. Regrettably, the provincial government did not make much of an effort to put the drive on hold at once. On May 19, the K-P government launched a 12-day measles vaccination programme to ensure that an outbreak does not occur as it did last year. However, on the very first day of the campaign, 25 students of a government school fell unconscious after they were given the vaccine by health teams. This continued for the next few days with such reports coming in from several districts. However, the K-P government seemed to have turned a blind eye to the reports and pressed ahead with the vaccination drive when clearly there was something wrong. In a civilised society, relevant authorities kick into action and begin an investigation at the first sign of trouble. But the provincial government continued to ignore the signs and it took the death of several children for it to wake up from its slumber. After the death of children, the provincial health minister formed a top-level investigation committee made up of local and foreign health experts. Sadly, for nearly two weeks there was no wind of what this supposed committee was doing and what it is investigating. There are several reports floating around that the people who were administering the vaccine to unsuspecting children were not trained and thus injected the medicine incorrectly. A doctor from Charsadda claimed that the vaccine had gone bad because of prolonged power outages in the hospitals it was stored at. But nothing conclusive about the vaccine s efficacy has emerged from the government s quarters. The country s polio campaign is already a target of militants and refusal cases abound because of several reasons. I m afraid that the negligence of governments both federal and provincial will also alienate people from the measles vaccination drives and parents would refuse to get their children vaccinated against this fatal disease, creating fresh challenges for the state. How can we expect a country which allocates a measly amount to health in its budget to progress? Governments remain negligent and following their lead the relevant health authorities also ignore their duties without fear of punishment. This needs to stop as soon as possible.

16 15 MEASLES TAKES LIVES OF 157 CHILDREN Express Tribune Editor: Sameer Mandhro, Sehrish Wasif It s not just polio. Pakistan is suffering from another outbreak of measles that has taken the lives of 157 children so far. The intensity of the spread of measles has significantly increased, with 727 confirmed cases this year alone. Khyber-Pakhtunkhwa and Sindh are the worst hit. In Sindh, there were cases of measles and ensuing child deaths for the second consecutive year. Health experts say that measles kill children in the province mainly due to malnutrition. In February and March this year, Sindh launched a measles vaccination drive aiming to inoculate 3.9 million children between the ages of nine months and 1o years. We have achieved our target, up to 103% as the population has increased after the last census in 1998, said the provincial head of the Expanded Programme on Immunisation (EPI) programme, Dr Mazhar Khamisani. The situation is under control now. Similar to the situation in Khyber-Pakhtunkhwa, several children fainted in different villages of Sajwal during the vaccination. But there were no deaths reported due to the vaccination. However, many children have died of the disease itself. According to government officials, only 14 children have died of measles this year, but unofficial reports put the number to almost 100. When inoculation campaigns are not being conducted, the only places to go are EPI centres and public sector hospitals. But a majority of these health facilities are far away from villages and so most of the poverty-stricken villagers avoid visiting hospitals only for vaccination. It needs a whole day and expenditures for transportation and meals, a villager Ayub said. It would be better if teams visit us. Another villager near Sajawal, Parvez Ali, too, avoids visiting hospitals for vaccination only. I know vaccination is important for a child and a mother but the system is unfavourable here, he said, adding that one can t get his children easily vaccinated without contacts. His threemonth-old daughter, Madiha, died in February because of measles. According to the provincial health department, a total of 13.9 million children were administered measles vaccine, which is free of charge. There are many expectations from the measles supplemental immunisation activities (SIA), which have been recently completed in Sindh and K-P, a federal EPI official told The Express Tribune. This campaign was supposed to be carried out last year when it was recommended by international donor agencies. However, the lethargic attitude of the government that only concentrated on accumulating funds caused the delay, he added. He explained that the success of the measles campaign will be clearly reflected through a significant decrease in the number of cases from K-P and Sindh starting from the month of July. We are expecting at least a 95% decline in the number of measles cases from the two provinces after the drive.
