1 School of Nursing REINSTATEMENT AND INTERNAL TRANSFER POLICY AND ACTION PLAN
2 School of Nursing 16.2 Grading: Progression Students who receive grades of C, D+, D, F, or a W in NUR 102 or NUR 125 or any PNU course will be dismissed from the program and may reapply for admission. Students who reapply are not guaranteed a seat and must meet all admission requirements. Students who receive grades of C, D+, D, F, or a W in any subsequent NUR course may apply for reinstatement to repeat the course. Nursing students requesting reinstatement due to interruption of the specified progression through the program of study, for any reason, are not guaranteed a seat in the course requested. Reinstatement is dependent on clinical space availability and a review of the student s academic eligibility. Students can be reinstated into a NUR course only one (1) time. Reinstatement must occur within one (1) year from the last clinical course or the student must reapply to the program as a new student. Reinstatement forms and rubric used to determine academic eligibility are available on the School of Nursing website at A total of two (2) unsuccessful attempts resulting in a grade of C, D+, D, F, or a W in any NUR course will result in dismissal from the nursing program. Associate degree students who have been dismissed may apply for internal transfer to the Practical Nursing program. Requests for internal transfer must be made in writing to the Director of Admissions within four (4) weeks of program dismissal. Internal transfer is dependent on clinical space availability and a review of the student s academic eligibility.
3 School of Nursing Reinstatement Action Plan Students who are applying for reinstatement into the nursing program will be evaluated based on the reinstatement rubric and be required to complete the re-instatement action plan. Reinstatement is dependent on clinical space availability and academic eligibility. Students requesting reinstatement to a nursing course who received a medical leave of absence (MLOA) are exempt from completing the reinstatement action plan. Students who were unsuccessful, or withdrew with a failing average must have reinstatement action plan completed within two weeks of leaving the course. Please follow the steps below to be considered for reinstatement. 1. Meet with the course faculty lecturer after withdrawal/failure to discuss a remediation plan for such course that student was not successful. 2. Write a formal letter of intent to the Academic Chairperson to be considered for reinstatement. 3. Complete a one-page reflective journal documenting track to re-instatement. Items to be included: summary of meeting with course faculty, reflection on strengths/weaknesses in unsuccessful course, student s plan for remediation, and objectives to be met upon reinstatement. 4. Complete ATI assessment exam and focused review as directed and complete a remediation template for each topic to review from unsuccessful course. 5. Complete Nursing Program Change Form indicating the nursing course that needs to be repeated. 6. Submit all documents to Nursing Department secretary within two weeks of leaving course: a) formal letter of intent b) one-page reflective journal c) ATI assessment and focused review with remediation templates d) Nursing Program Change Form

Provide documented reflective journaling to include: meeting with course lecture faculty, reflection on strengths/weaknesses in unsuccessful course, remediation plan, and objectives to meet upon reinstatement.
