2 About the service The Rapid Response Service helps care for you in your own home when you are ill but do not need to be in hospital. The benefits of being treated at home include reducing the risk of infections, making it easier for you and your family to be involved in the decision-making process and allowing for assessment and treatments to be based around your specific requirements and environment. A senior healthcare professional will assess you at home and coordinate services that can help to support you there. In some circumstances, you may be offered a bed in our community unit. We are available 9.00am 12.30am, seven-days-a-week including public holidays (care support 8.00am 9.00pm). The team is based in Ickenham. We support people over the age of 18 years old who are registered with a Hillingdon GP and live no further than two miles outside the borough. We work closely with The Hillingdon Hospitals NHS Foundation Trust s A&E Department and Homesafe Team to support patients discharged from hospital. We can support your choices around end of life care, if this applies to you.

3 What we offer We provide short-term intervention to help stabilise your condition. This may include nursing, mental health support, occupational therapy, physiotherapy and care support. Once your condition has stabilised, we will identify and arrange further services, as required, to support you in your return to health and wellbeing. These services include: District nurses Social services Medequip (equipment provision) Specialist practitioners e.g. heart failure nurses, diabetes nurses Diagnostic services Community matrons Community Night-sitting service. How do I get referred? You can be referred to this service by a healthcare professional, such as a GP, district nurse, community matron, nursing or residential home staff or the London Ambulance Service. You may also be referred from The Hillingdon Hospitals NHS Foundation Trust s A&E Department or Homesafe Team. We work closely with your GP and other healthcare professionals to identify the most appropriate services to meet your needs.

Feeling valued We will: Acknowledge and treat you as an individual Treat you with dignity and respect Deal with issues in a confidential manner Behave with kindness, compassion, sensitivity and concern.

Data protection Everyone working for the NHS has a legal duty to maintain the highest level of confidentiality. As a healthcare provider we need to collect information about you. We will store this securely and ask your permission to share information outside of the organisation. If you require further information, please ask a member of the team for a copy of our leaflet Your information: How we keep your information safe. Tell us, we re listening! Our staff want to know how they are doing. Tell us what you think at and then we ll know what we have to do. If you are happy with the service you have received, please speak to a member of staff or contact our Patient Support Service. If you are unhappy with our services, please speak to the service manager in the first instance.
