1 Eastern Region DELTA EMERGENCY RESPONSE TEAM (ERT) COMMUNITY PREPAREDNESS AWARDS The Eastern Region Delta Emergency Response Team (ERT) Committee is pleased to accept applications for the 2017 Eastern Region Delta ERT Community Preparedness Awards, which recognizes the outstanding efforts of chapters throughout the region working to prepare their communities and chapter members for emergencies. These awards highlight innovative practices and campaigns that have made outstanding contributions toward making communities safer, better prepared, and more resilient Award Categories: o Outstanding Achievement in Youth Preparedness Presented to an organization, program, or jurisdiction that increases youth preparedness; engages youth to educate others in disaster preparedness; or, incorporates the voice of youth in development, outreach, and assessment of preparedness programs. The organization or program selected must align with Federal Emergency Management Agency s (FEMA) National Strategy on Youth Preparedness Education ( o Preparing the Whole Community Presented to the chapter that successfully engages the whole community to improve disaster preparedness and resilience; encourages and explores ways to develop whole community partnerships; and advocates a grassroots approach to emergency planning. o Outstanding Emergency Response Team (ERT) Initiatives Presented to a Chapter Emergency Response Team or Program that has made a significant contribution to response or recovery efforts during a disaster; has demonstrated innovation in operations, outreach, or training practices; or, excels in educating community members about disaster response skills and preparedness.

2 Delta ERT Awards Page 2 o America s PrepareAthon! in Action This award is presented to a chapter that has implemented a registered America s PrepareAthon! event, which incorporates specific actions taken to strengthen communities preparedness and resilience in the face of disasters. Activities may include any of the America s PrepareAthon! 10 Ways to Participate or other initiatives, which showcase preparedness. Applicants are asked to describe accomplishments that took place between July 1, 2016 and April 30, Applicants may apply for all categories but must submit only one application package. Application Process Application Form - All applicants must complete the Eastern Region Delta ERT and Community Preparedness Awards Application Form and Checklist. The Application Form also includes a Description of Achievement section, which should be no more than two pages in length and typed in Arial font at size 12. Description of Achievement: When preparing your Description of Achievement, please use the following headers, sequence, and apply the description guidance: 1. Problem Statement: Provide a clear and concise description of the problem/issue(s) that the work you are presenting in this application is meant to address. 2. Resolution: Briefly describe what you plan to do to address the problem/issue(s) out-lined in your problem statement. 3. Project/Program Description: Summarize your project/program activities and achievements, and how they helped you to resolve the problem/issue(s) outlined in your problem statement. 4. Results/Impact: Describe the results/impact your activities had on your community and its population. Additionally, indicate the participants your activities served (e.g., chapter members, students, community leaders, local elected officials). Supplemental Materials: Supplemental materials are welcomed, but not required. If included, supplemental materials should not exceed three attachments and your total application submission should be no larger than five megabytes (5 MB). Instructions for condensing files are provided below. You may also indicate website links to supporting materials, videos, and information in the Attachments section of the Application Form.

3 Delta ERT Awards Page 3 Acceptable file types include: o Application file: PDF or Word o Attachment file(s): PDF, Word, Excel, JPEG and PNG o Note: Temporary internet files that connect to an original file will not be accepted. Endorsements: To strengthen your application, you may wish to include a letter of recommendation or a written endorsement from a school, local government official or community organization, tribal leadership, a Citizen Corps Council, Partner Program, or Affiliate. Letters of endorsement are not required. Submission Instructions 1. Ensure that the name indicated in the Nominee Name field of the application is the name you want to use for the certificate, if awarded. 2. Save your Application Form using the required file name format ERDERTCPANomineeName. EXAMPLE: ERDERTCPWestParkAlumnaeChapter or ERDERTCPOmegaPhiChapter 3. Attach your completed Application Form (which must include the Description of Achievement) and completed Checklist to your submission as well as any relevant attachments. 4. The subject line of your should read: 2017 Eastern Region Delta ERT Community Preparedness Award Application. 5. your application and any supporting materials to emergencyresp@easternregiondst.org (one submission per applicant). 6. Ensure you receive an confirming receipt of your application. 7. 4 Delta ERT Awards Page 4 Review Process Reviewers will assess each application based upon the following criteria: Clarity: Is it clear what the applicant is trying to convey about the value and impact of the program, partnership, activity, initiative, etc. they are highlighting? Innovation: Does the applicant display originality, creativity, and/or the ability to make the most of limited resources or find alternative/unconventional solutions? Background: Does the applicant support their claims with quality results, sustaining documentation, findings, sufficient evidence or examples? Category Alignment: To what extent does the applicant s program, partnership, initiative, or activity align and support the category for which it is being considered? Remember, all applications and supporting materials must be submitted to the committee no later than 11:59 P.M. E.T. on May 15, 2017 in one submission in order to be considered.

All applicants are required to submit a completed checklist with their application submission. 1. ERT Awards Application Form Completed all requested fields/questions. Aware that the name indicated in the Nominee Name field will be used on the certificate, if awarded. 2. Attachments Provided the File Name and the Description for each attachment. 3. Description of Achievement Used the correct headers, guidance, and order to organize the Description of Achievement 4. Submission Instructions: Ensured that the total submission size is no more than five megabytes (5 MB). Named/Saved the Application Form per the required naming convention. Ensured that each file attachment is one of the following file types: Application Form and Checklist: PDF or Word Attachment file(s): PDF, Word, Excel, JPEG and PNG
