2 Introduction The Calvary Community Council has been established by Calvary Health Care Tasmania to advise on the management of its community funds. The Council s mission is to reach out to Tasmanians to provide assistance to people in need. This mission complements the founding philosophy of The Little Company of Mary of being for others. It also reflects our Catholic social justice tradition and the values of the Gospel. Calvary Community Council is one aspect of Calvary Health Care Tasmania s community benefit program which is an active way Calvary is committed to caring for marginalised and vulnerable people in Tasmania. Community Council Grants Program Calvary Health Care Tasmania is committed to assisting the Tasmanian community, not just through excellence in its hospital and medical services but through the financial support of projects which will build a healthier and happier community. Calvary Community Council offers grants up to $5, per grant in two (2) grant rounds per year. The Calvary Community Council assists Calvary Health Care Tasmania in identifying and funding projects that will be of benefit to the health needs of the Tasmanian community. The Calvary Community Council aims to support programs that will reduce the current health inequalities that exist in Tasmania. Because of this the Council has a particular focus on vulnerable and marginalised groups and on projects that will help Tasmanians take more control of their health and well-being. Community Council Grants Criteria It is advisable to address these criteria explicitly in your application. To ensure your application is well-founded it is recommended you check that you meet all these criteria before proceeding,: It aims to benefit persons from one or more of the specified areas of focus (see below); It aims to improve health outcomes of the Tasmanian community or the broader social determinants of health that underpin community development and wellbeing; It aims to benefit communities in Tasmania via organisations that are based in Tasmania; It does not aim to fund individuals, personal travel or private scholarships and bursaries, construction projects or commercial film making; It aims to trigger future current and future initiatives rather than retrospective ones; It is auspiced and approved by an organization with evidence of financial and general governance. The Calvary Community Support Program is unable to support applications which do not meet these criteria. 2
3 Areas of Focus The Calvary Community Council supports processes, events and activities that make a positive difference to the health needs of the community in one or more of the following focus areas. The Community Council particularly supports projects that are seeding projects or start up projects. The Council has a focus on organisations who are addressing identified and specific unmet, underresourced or under-serviced health needs in the community. To this end, it has chosen to focus on the following four areas, expanded further below: Marginalised and Vulnerable Groups Children and Adolescents (0-18) Young Adults (18-30) The Elderly Marginalised Groups The Calvary Community Council recognises that an indicator of how well a community acts is determined on how it treats its most vulnerable and marginalised members. The Catholic Church teaches that the marginalised deserve preferential respect because if a society is to provide justice for all, the poor, the powerless have the most urgent claim. The community is compromised when some of its members are marginalised and denied basic rights. The Community Council is committed to help those who are the most vulnerable, thereby strengthening the whole community. The Community Council will therefore consider support for projects which: Focus on improving the health of our most vulnerable and marginalised community members. Help to enhance the wellbeing of vulnerable and marginalised community members. Help to empower marginalised groups in our community. Target groups who do not have access to health care services that other community members have. Are justice initiatives which strengthen a marginalised group s capacity to live with dignity. Children and Adolescents The Calvary Community Council recognizes that experiences, education and behaviors in childhood form the foundations for health, development and wellbeing throughout life. The Calvary Community Council aims to support programs that enable children and young adults to break cycles of poverty and unhealthy living which enable them to live healthier more fulfilling lives. The Calvary Community Council will consider providing support to initiatives that recognise the particular problems faced by children and adolescents experiencing family instability or stress, social isolation, socio-economic disadvantage and/or lack of support services that have negative impacts. The Council aims to support projects that: Promote the health, development and wellbeing of children and adolescents Promote antenatal care and maternal nutrition Promote the supportive social relationships and positive networks 3
4 Promote the participation in positive community activities of parents, children and adolescents Promote initiatives that enable children and adolescents to live healthy and fulfilling lives and to break cycles of poverty and unhealthy living Young Adults The Calvary Community Council recognises that poor mental health and decreased physical wellbeing in young adults is a growing problem in Australia which contributes to social isolation, suicide, poverty and disadvantage. Research has shown that issues of poor mental health and reduced health and wellbeing in young adults have a significant adverse effect on the quality of life of many Australians. The Calvary Community Council aims to partner with health providers and community organizations to support programs that enable young adults to break cycles of unhealthy living and poor health and wellbeing and which enable them to live healthier more fulfilling lives. The Calvary Community Council will consider providing support to initiatives that recognise the particular problems faced by young adults experiencing instability or stress, social isolation, socioeconomic disadvantage and/or lack of support services that have negative impacts on their health and well-being. The Council aims to support projects that: Promote the health, development and wellbeing of young adults Promote supportive social relationships and positive networks Promote improved mental health for young adults Promote the participation in positive community activities for young adults Promote initiatives that enable young adults to live healthy and fulfilling lives and break cycles of poverty, poor mental health and unhealthy living The Elderly In order to help build a society in which older people lead satisfying and productive lives with maximum independence and wellbeing, the Calvary Community Council will consider providing support to initiatives that: Promote the dignity and worth of the elderly. Increase the participation of the elderly in the workforce, education, leisure and volunteering. Recognise the diverse needs of older people, including the diversity of gender, culture, language, geographical and socio-economic circumstances. 4
5 Advice for Applicants Attached to these guidelines is an Application Cover Sheet and a Checklist to ensure the Council receives all the information it requires. Only one application from any one organisation will be accepted for consideration in each funding round. Your application should be brief and to the point and run to a maximum of four pages in no less than 12 point font. It can include any number of attachments such as reports, evaluations, research findings, press clippings, videos or other relevant material to support your case. Your application should be divided into six sections with a heading for each of the criteria: 1. Overview statement, addressing the criteria 2. Background to the project 3. Objectives of the project 4. Project summary 5. Funding and Budget 6. Method of Evaluation and reporting 1. Overview statement This section should provide an overview of what area of focus your project is addressing. In your overview provide an explanation of how the proposal specifically relates to the Community Council s areas of focus? 2. Background to the Project This section should provide a context to the issue you are addressing. If possible, try and quantify the extent of the problem or need you would like to address. If your organisation has a history of addressing this issue, provide a brief history of this work. It is useful to cover the following: Why does the proposed work need to be done? What has been done in the area to address this issue and by whom? What is known about the issue and what research has been done on it? How well placed is your organisation to do this work? 3. Objectives of the Project We would like to see the three key objectives of the project. These can be short, medium and long term objectives. Please provide details on each objective in bullet points. 4. Project Summary This section should describe the project you are proposing and what difference it will make. It is useful to cover the following: What are you proposing to do and why? Who is going to do the work and are they qualified to do it? Where will the work be done? How long will it take? How is this project different from other attempts to address this issue? 5
6 How will the work continue after the current support period? 5. Funding and Budget You must provide a full project budget indicating which component or proportion you need support by the Community Council. Support for projects that will extend over more than two (2) years will not be considered. Your budget must show where you have received or expect to receive support in cash or in kind from other parties, including a contribution, in cash or in kind, from your own organisation. Do not include provision for GST. The Australian Tax Office has issued a Private Ruling to members of Philanthropy Australia Inc. which states that grants made to non-profit organisations will not incur GST. Accordingly, such organisations do not pay GST in respect of any grants made. You will need to supply a copy of your organisation s current audited Financial Statements and/or Annual Report. It is necessary for your budget to cover the following: What is the overall cost of the project? What non-monetary supports, ie: Calvary Health Care Tasmania Volunteer Network, are you requesting? (It may be possible for Calvary Health Care to provide you with medical or health advice or support that may assist you in achieving your goals.) Similarly our volunteer network may also be able to help your organization with personnel or voluntary assistance. Are you asking for core funding or support for activities? Have you provided for the cost of monitoring and evaluation? Have you provided for the cost of communicating about the project s outcome? 6. Evaluation and reporting Please provide information about how the work will be documented, monitored and reviewed. Documentation required in your application The completed application form below Statement addressing one or more of the areas of focus and the six elements of the application Evidence of incorporation as a not-for-profit body or similar status Evidence of Deductible Gift Recipient (if applicable) please supply a copy of the ATO notification letter) and Australian Business Number (ABN) (please supply a copy of the ATO notification letter) or ABN of sponsoring organization. 6
7 How and where to send your application Your application should be received by no later than 5.00pm on the date of closing for each round and should be addressed to: Attention: Director of Mission Calvary Community Council Calvary Health Care Tasmania GPO Box 1523 HOBART TAS 7001 Applications can also be hand delivered to the Calvary Community Council, Calvary Health Care Tasmania, C/- Executive Office, Lenah Valley Campus, 49 Augusta Road, Lenah Valley TAS You may also your application to Receipt of your application by the due date does not guarantee that it will be supported by the Community Council. How we deal with your application 1. We will acknowledge receipt of your application via 2. If your proposal is considered to be outside our guidelines or areas of focus, a letter of advice will be forwarded to you as soon as possible. 3. If your application is successful, you will receive advice as soon as possible. 4. Not all applications considered can be offered a grant. Applications may be offered support but not the total amount requested. Role of Successful Applicants All grant recipients are asked to provide, within two months of the conclusion of the support period a written grant report following guidelines outlined in the application. GRANT REPORT FOR SUCCESSFUL APPLICATIONS Following the completion of each successful project a 1-2 page report needs to be submitted to The Calvary Community Council. This report will be shared with our community members, staff and patients. The report will need to use photographs or diagrams to help people understand the great work that was done with the money from the Calvary Community Council grant. 7
8 There are 5 key areas required for the report. 1. Objectives Identify what the 3 key objectives of the project were 3. Evaluation Analyze if the three key objectives were met and report why or why not Identify the major success of the project Outline a key learning from the project 4. Future Outline any future plans that have been developed as a consequence of the project.
