1 Medical & Academic Partnerships (MAP) Program Rheumatology Fellowship and Visiting Professorship I. Background Pfizer s External Medical Affairs (EMA), US Medical Affairs (Inflammation) and Independent Grants for Learning & Change (IGLC) organizations are proud to present the Medical & Academic Partnerships (MAP) program. Since the inception of the MAP program in 1984, Pfizer has funded more than 1000 projects. This program is a reflection of Pfizer s commitment to advancing research, innovation, and medical education for academic centers and schools of medicine, nursing, osteopathic medicine, pharmacy, and public health. The MAP program provides career-building opportunities for practicing clinicians and academic researchers in outcomes and patient-oriented research that is vital to improving healthcare. Partnerships with organizations such as the National Institutes of Health (NIH) and Centers for Disease Control and Prevention (CDC) help develop future medical leaders and drive advances in patient care. Through the MAP program Pfizer is supporting Fellowships and Visiting Professorships in Rheumatology. Fellowship funding supports research proposals for programs and initiatives that seek to uncover mechanisms for improving the organization, financing, and delivery of public health strategies that promote health and prevent disease and disability on a population-wide basis. Organizations interested in research fellowships must submit proposals for programs and initiatives that focus on research aimed to further understanding of rheumatic diseases, and address scientific questions in clinical, translational and basic sciences as well as excellence in patient care related to rheumatology. The purpose of the fellowship grant is to help fund fellows who need to complete their research and/or do a 3rd year in the program focused on research. The Visiting Professorship is a reflection of our commitment to advancing research, innovation, and education in academic medicine, health disparities, nursing, pharmacy, health literacy, health policy, and public health. Pfizer offers the Visiting Professorship to provide financial support for host organizations to coordinate educational exchanges so healthcare practitioners, researchers, and patients can benefit from the knowledge and on-site presence of distinguished medical experts who might otherwise be inaccessible. 2 Applicant Eligibility Criteria: Fellowship The following requirements are in place for the Fellowship: Must be an accredited U.S.- based academic organization. The organization cannot host another recipient of this same award during the proposed term. Senior staff, division heads or department chairs should work on behalf of their organization to prepare and submit an application for consideration. Do not include the name of the potential Fellow in application materials. Visiting Professorship The Visiting Professorship is open to the following organizations: U.S. Medical Schools Academic medical centers with dedicated rheumatology departments State rheumatology associations Do not include the name of the potential Visiting Professor in application materials. 3 Expected Approximate Monetary Range of Grant Applications: Fellowship 3 Fellowship grants up to $80,000 per year for two years Funding is to be used as salary supplement for the recipient Funding will be awarded to the organization, which will select the recipient. U.S. Physician Payments Sunshine Act ( Sunshine Act ) and other transparency law requirements must be satisfied with respect to meals, payments and other transfers of value. Visiting Professorship Up to 12 grants at up to $10,000 each Funding is to be used to cover the visiting professor s honorarium, travel expenses, and other direct expenses incurred by host organization in conducting program activities, including luncheon/dinner for the event, if applicable. U.S. Physician Payments Sunshine Act ( Sunshine Act ) and other transparency law requirements must be satisfied with respect to meals, payments and other transfers of value. Key Dates: The dates listed below are applicable to both the Fellowship and Visiting Professorship. Those payments or transfers of value include honorarium or other compensation, meals, consulting fees, reimbursement of travel and other expenses and items of value (items that possess a discernible value on the open market, such as textbooks) provided to faculty, attendees and awardees, if those individuals meet the definition of Covered Recipients. The organization must submit the required information during the reconciliation process or earlier upon Pfizer s request, so that Pfizer can meet Sunshine Act reporting obligations. Pfizer will not make any payments to individuals; grant funding must be paid directly to the organization. Appendix A: MAP Fellowship Application Requirements The following information is required and must be consolidated into two documents. The first document should include items A thru H in Word or PDF; single-spaced using 12-point font and 1-inch margins. Further, items A thru F may not exceed 10 pages in total. The second document should be in Excel for item I, budget. No more than 2 documents may be uploaded as part of your application. A. Research Project Name 4

Budget Requirements for Prospective Fellows Prospective award recipients at the organization cannot apply directly for the award. However, a fellow may assist the senior staff, division head or department chair at the organization in the development of the application. Fellows supported by a Pfizer MAP fellowship must have the opportunity to carry out supervised research with the primary objective preparation for their respective discipline and subspecialty. Upon receipt of the award, the fellow selected by the organization must meet the following criteria: U.S. citizen or foreign national with permanent U.S. residence Hold an advanced degree (e.g., PhD, MD, DO, NP, PharmD, etc.) Be enrolled in fellowship program (clinical fellowships only) or hold a non-tenured, junior faculty appointment (research fellowships only) Requirements for Proposals Proposals involving study of any pharmaceutical product are ineligible for MAP awards. Basic science and/or fundamental research (in animal model, in-vitro, etc.) are not permissible. Appendix B: MAP Visiting Professorship Application Requirements The MAP Visiting Professorship template must be completed as part of your application. The template can be downloaded from GMS. After completing the template, save the file to your computer and then upload the completed file to GMS. The required template must be used to capture the information listed below.
