13 Access Health Ryan White Clinic Working directly with clinic staff regarding best practices in routine HIV testing Particular interest placed in patient/staff interactions Clinic serves predominantly racial/ethnic minority patients Staff is predominantly white Very economically disadvantaged population within a generally very economically advantaged area 13

14 Healthcare for the Homeless Houston Administrative Technical Assistance has been important to this clinic Clinic has a strong desire to work with other homelessserving clinics to develop and implement best practices Transient populations Populations at risk for HIV infection 14

15 Gulf Coast Health Center Currently are developing plan for educational activities Strong interest in improving HIV testing rates Primary goal for this clinic Unique challenges include: Louisana/Texas Medicaid differences High risk of infection among offshore workers 15

20 St. Gabriel Health Clinic This clinic is currently developing a plan for educational activities Strong support among staff for the success of this project Activities will mainly involve improving HIV testing rates, linkage to care, and administrative technical assistance 20

22 Wellness Pointe Strong relationships with clinic staff Very eager to try new ideas and perform rapid cycle quality improvement This clinic also has issues with Texas/Louisiana Medicaid differences Upcoming trainings to include Medical Assistants Plans to improve telecommunications capabilities to allow for more distance learning 22

23 All HPTP Coaches Meetings Occur monthly Recordings are made available if coaches are unable to attend The HPTP Coach is responsible for the information contained in the meeting regardless of his/her ability to attend the meeting live 23

24 Meetings with HPTP Sites The expectation is that HPTP Coaches meet with each clinic site at least weekly Standardized documentation must be used for these meetings Meetings are focused around the clinic s goals and how the SCAETC can help them achieve those goals Training Technical Assistance Etc. 24

25 Quarterly Check-In Calls Central Office has a conference call with each clinic on a quarterly basis To ensure that what is being reported to the Central Office by the HPTP Coach is accurate To address any contractual issues To offer assistance which may need to come from the Central Office To ensure that the needs of the clinic are being met 25
