23 All HPTP Coaches Meetings Occur monthly Recordings are made available if coaches are unable to attend The HPTP Coach is responsible for the information contained in the meeting regardless of his/her ability to attend the meeting live 23
24 Meetings with HPTP Sites The expectation is that HPTP Coaches meet with each clinic site at least weekly Standardized documentation must be used for these meetings Meetings are focused around the clinic s goals and how the SCAETC can help them achieve those goals Training Technical Assistance Etc. 24
25 Quarterly Check-In Calls Central Office has a conference call with each clinic on a quarterly basis To ensure that what is being reported to the Central Office by the HPTP Coach is accurate To address any contractual issues To offer assistance which may need to come from the Central Office To ensure that the needs of the clinic are being met 25
