Only one General Permit Registration form should be used for the processes in the selected option. Each facility may possess only one GP-015 at any time. If a facility wants to make a change to a source which has already been registered under GP-015, a new General Permit Registration is required, unless the changed source would remain within the class of sources which qualify for GP-015, and the source would continue to be operated in accordance with the parameters set forth in the option chosen in the Registration. Indicate below how the equipment in this general permit is being registered by selecting one and only one of the three permitting options listed on Table 1 on page 3. Additionally the registrant must identify the stack that was used to select the permit option. Once chosen, this stack becomes the option-designated stack. This stack must be vented continuously to the outside atmosphere while the plant is operating. 5 INSTRUCTIONS FOR COMPLETING THE REGISTRATION FORM FOR NON-MACT PLATING OPERATIONS SECTION A: FACILITY PROFILE Facility (ID Number & Name) - Enter the New Jersey air pollution facility identification number (ID) followed by the facility name for which the General Permit is being registered. The ID is a five-digit number assigned by the New Jersey Department of Environmental Protection. If you have other air pollution control permits for your facility, you can obtain the ID number from those permits. If you do not have any air pollution control permits, leave this space blank. Your facility name is the one registered with the New Jersey Secretary of State, under which your facility does business. Include the Case Number assigned by the Bureau of Underground Storage Tanks, if known. Street Address - Enter the address of the facility where the equipment is physically located. Mailing Address - Enter the facility s mailing address. If it is the same as the facility location, check the box provided. Mailing Instructions & Information - Once the General Permit Registration Form has been completed, it should be mailed along with the appropriate fee to the New Jersey Department of Environmental Protection at the address listed on the front page of the Registration form. Make checks payable to: Treasurer, State of New Jersey. County - Enter the county in which the facility is located (not the mailing address county). Industry Information - Enter the facility s four-digit primary and secondary (if any) Standard Industrial Classification Codes (SIC) or equivalent. Use the codes registered with the US Department of Labor. You may also refer to the Standard Industrial Classification Manual from the U.S. Office of Management and Budget or equivalent industrial classification code. Location Description - Describe the facilityis location if it is difficult to find using the street address. If you have to give a visitor directions to your facility, consider showing them here. (Example: Two miles down the access road that leaves state highway 29 at mile marker 10.) Facility Contact - Check the box indicating the facility contact person for this General Permit Registration. If the owner and operator are the same, check both boxes. Enter the name, title, phone and fax numbers, other phone numbers and type of number (Example: pager, toll free, cell phone), address, the organization that the contact person works for, the type of organization (federal, local, public, private, state or utility), the New Jersey Employer Identification Number (EIN) and the contact mailing address. SECTION B: REASON FOR APPLYING This section of the General Permit Registration Form provides the Department with the reason the General Permit Registration Form is being submitted. New Plating Process(s) or Existing Plating Process(s) - Check this box if the operations are for a plating process(s) for which no current permit exists. Page 5 of 6

6 Change in permitting option - Check this box if the Operation is already permitted under an existing GP- 015 and the Registrant wishes to change the permitting option (e.g. Option C to Option A). Be sure to list the present permits that will be superseded by the General Permit. Existing Permitted Plating Process(s) switching to General Permit - Check this box if replacing an existing permitted plating process(s) with a General Permit. Be sure to list the present permits that will be superseded by the General Permit. Other - Check this box if no other selection is appropriate, and provide a brief explanation. SECTION C: PERMITTING OPTIONS This General Permit covers basic material finishing operations at plating facilities. Select only one option. In the designated area, write that option and the unique identification/name given to the stack that you select to be the "Options Designated Stack". Once chosen, this stack becomes the optiondesignated stack. This stack must be vented continuously to the outside atmosphere while the plant is operating. Other stacks that vent the facility must be as high or higher than the "Options Designated Stack" and must also be vented vertically upward without any raincap. However those other stacks may be vented or closed as needed for employee comfort. Registration under this General Permit covers all equipment listed in Section III Applicability, of the General Permit. Only one General Permit Registration form should be used for the processes in the selected option. Each facility may possess only one GP-015 at any time. If a facility wants to make a change to a source which has already been registered under GP-015, a new General Permit Registration is required, unless the changed source would remain within the class of sources which qualify for GP-015, and the source would continue to be operated in accordance with the parameters set forth in the option chosen in the Registration. Notes: i) Only one General Permit will be issued per facility. ii) iii) iv) Only one OPTION, either A, B or C may be selected from the OPTIONS TABLE. For facilities having multiple stacks with different stack heights, selection must be based on the lowest stack. Do not average stack heights. For example, if operations were vented out of stacks with emission point heights of 22 ft, 20 ft, 30 ft and 50 ft, the option available would be OPTION B, which requires a minimum stack height of 20 ft. All stacks or chimneys must be vented vertically upward with no rain hats installed. v) The minimum stack diameter at the emission point shall be greater than or equal to 2 feet. vi) vii) Adding, removing or changing plating processes are allowed so long as the limitations specified in the OPTIONS TABLE and the COMPLIANCE PLAN are complied with at all times. Recordkeeping to demonstrate compliance must be maintained to show compliance with the applicable requirements. SECTION D: CERTIFICATION Page 6 of 6

Individual with Direct Knowledge - Individual listed as the contact person or any person with direct knowledge of and responsibility for the information contained in the General Permit Registration Form. This may or may not be the same person who signs as the Responsible Official defined below. Responsible Official - A facility official responsible for the General Permit Registration. A Responsible Official as defined in N.J.A.C. 7: is as follows: For a corporation: a president, secretary, treasurer, or vice-president of the corporation; any other person who performs similar policy or decision making functions for the corporation; or a duly authorized representative responsible for the overall operation of a facility (plant manager, etc.). For a partnership: a general partner. For a sole proprietorship: the proprietor For a government agency: either a principal executive officer or ranking elected official. Page 7 of 6
