While we try to provide you with accurate data, the information contained in this directory may change as participating providers join or leave the network, move, or retire. This online directory does not constitute a recommendation of any of the listed providers. Prior to receiving services, you should always verify that the provider you choose is still participating and accepting new patients. In addition, some benefit plans require that you obtain a referral from your primary care physician before receiving services from another physician or specialist. In order to receive benefits or to receive the highest level of benefits - check your schedule of benefits, your group administrative contract or call the Customer Service number on the back of your ID card to verify your benefits. If you receive services from a network hospital, the hospital may arrange with physicians and other providers to assist in your care. Network hospitals are encouraged to hold those hospital-based providers to the same standards for member billing. However, if a hospital does not require its hospital-based providers to comply with the same standards, we cannot influence the manner or amount that you may be billed by the hospital-based provider. Although this listing is continually updated, participation by practitioners as well as data regarding Patient Populations Treated and Areas of Expertise is not guaranteed and may be subject to change. Prior to receiving care, please ask your practitioner whether he/she is still participating in the network, accepting new patients, and providing services that meet the criteria you have selected. Pre-Authorization from Anthem Blue Cross may be required prior to seeking outpatient or inpatient mental health or chemical dependency services, except for emergency services. You or your provider should call the toll-free number on your member ID card to obtain pre- authorization from Anthem Blue Cross. HMO members do not need a referral from your Primary Care Physician to see a Behavioral Health specialist. HMO members can self-refer to a specialist in the Behavioral Health Network. Confirm that the specialist is a participating provider at the time and location of service. For more information regarding behavioral health services, please contact your benefits administrator.
