1 Policy Name: Supersedes: Policy Deployment Disciplinary Action Institution: Policy Involuntary Interruption in Program Progression Division: Duke University Health System (DUHS) Duke Regional Hospital (DRH) Policy Level: WSON Student Department: Watts School of Nursing (WSON) Owner(s) Dr. Peggy Walters Contributing Departments: Approved by: WSON Faculty Committee Original Effective 6/2011 Date: Revision Date: 1/2017 Scheduled Review Date: DEFINITIONS Manual Name: 1/2020 Scheduled Review Interval: Student Handbook 36 months Probation - Specified time period in which the student must meet certain conditions as mandated by the Committee on Academic Standing and Professionalism (COASP) and demonstrate satisfactory improvement in designated areas. Probation will expire at the end of the semester unless continuation deemed necessary by COASP. Failure to follow directives of COASP may result in dismissal from the program. Suspension - Separation of a student from the program for a definite time period by administrative and/or COASP decision after which the student is eligible to return Administrative program withdrawal and eligible for readmission - Resignation from the program by administrative and/or COASP decision. COASP will determine length of withdrawal and conditions for return. Dismissal - Permanent severance of a student from the program by administrative and/or faculty decision. Disciplinary Action Form - Form used by faculty to initiate disciplinary action and submitted to COASP by the involved course coordinator. Report Form - Form used to document an infraction of policy.
2 Probation Contract Form used by COASP to initiate probation. POLICY Students are expected to meet standards of professional conduct which include, but are not limited to, complying with school policies, the honor code, and federal, state, and local laws. If a student fails to comply with the school s standards, administration and/or faculty will initiate disciplinary action. If at any time a student s conduct is determined to be of an egregious nature, the Director of Nursing Education has the immediate authority to suspend or dismiss the student. Details of any action taken will be retained in the student s record for a predetermined period of time as determined by the Director of Nursing Education. The student has the right to appeal disciplinary action through the appeals process. PROCEDURE A Disciplinary Action Form is completed by the course coordinator. The form is submitted to the Director of Nursing Education (DONE). The Director of Nursing Education reviews the Disciplinary Action Form and determines whether further steps are deemed necessary. If disciplinary action is deemed necessary, the Director of Nursing Education will call a meeting with the Committee on Academic Standing and Professionalism (COASP). The student will be asked to attend this meeting. COASP and the DONE will determine the specific action to be taken. These could include: o Probation o Suspension o Administrative program withdrawal and eligible for readmission o Dismissal. In the case of probation or suspension COASP and the DONE will determine the cause, length, and conditions. The DONE notifies the student of the decision and of his/her right to appeal. The Director of Nursing Education has the authority to dismiss a student.
3 Appeal Procedure: A student desiring to appeal a decision relating to disciplinary action must submit an electronic letter with a statement of reasons for appealing to the Director of Nursing Education (DONE). This statement must be filed within three (3) business days following notification of decision. The DONE will contact the student to discuss, in person, the appeal procedure and reason for appeal. The student will remain in current active status until the appeal procedure is completed. The Director of Nursing Education will present the student s request to appeal to the Faculty Organization for review and decision within ten (10) business days. The student will be asked to appear before the Faculty Organization. The DONE will notify the student of the Faculty Organization decision within three (3) business days. The Faculty Organization decision is final.
