1 BSN Application Supporting Documents Checklist Your Application Number is: Print, complete and sign this page. This page must be submitted as the top sheet, along with all BSN Application materials (Part C). Please make a copy of all submitted documents for your personal records. Supporting documents must be submitted at the SAME time and in the SAME envelope and mailed or hand-delivered to the address below. BSN Application supporting documents (Part C) must be received or postmarked by the deadline. If the BSN application deadline falls on a weekend, government holiday, or the university is closed due to unforeseen circumstances, the deadline will be 5 pm the next business day. Late applications will not be considered. 2 SlideRoom upload instructions: After you have created your SlideRoom account, open the browser. Go to [link to be provided] to upload your items. Print a copy of the SlideRoom submission confirmation . Submit the following items via SlideRoom: Video Presentation: What characteristics do you have that make you the best applicant for this program? 1. Maximum three-minutes in length. 2. State your name and G# first. 3. Your face must be visible during the presentation. 4. Record your video in landscape orientation. 5. You may only submit items to SlideRoom on time. 6. Visit the SlideRoom webpage if you need further information. Essay/Writing Assignment: Please describe the most eventful experience of your life. What important thing did you learn from the experience? 1. Must be your own original writing sample. 2. Indicate name, G#, page number and question at the top of each page. 3. Format instructions: Times New Roman, 12-point font, double-spaced with one-inch margins, two-page maximum. 4. Submit your writing sample in PDF format. Resume: Submit in PDF document format. (RN to BSN, Mason Veterans Program, and Coenrollment Program applications submit paper copy with Part C) Part C - To Be Mailed or Hand-Delivered to CHHS. Unofficial Transcripts Transfer Students: One copy from each institution attended Mason Students: One copy from each institution attended, including: all institutions attended prior to Mason your Mason transcript from Patriot Web. PLEASE NOTE: official transcripts are acceptable if submitted with Part B. Transcripts may not be submitted directly from an institution for BSN Application. 3 Copy of Current LPN License Copy of Active Military ID Copy of any communication from BSN Applications RN to BSN applicants only: Copy of Current US-based RN License Must be submitted to BSN Admissions no later than August 15th for fall admission, or January 15th for spring admission. Veterans Co-enrollment BSN Nursing Program (VCE) Applicants only: Copy of DD-214 (Member 4 or Service 2) Proof of Acceptance and enrollment in a partner community college AAS-Nursing program. Co-enrollment BSN Nursing Pathway, non-veteran (BCE) Applicants only: Proof of Acceptance and enrollment in a partner community college AAS-Nursing program. Accelerated Second Degree and Traditional applicants only: Copy of HESI A2 Exam Results - Must have been taken within the past year. Only scores dated after January 15, 2017 will be accepted. Must submit a copy of your one-page Admissions Assessment Report reflecting your HESI A2 (version1) test scores. This page indicates the breakdown of Reading Comprehension, Grammar, Vocabulary and Knowledge, Anatomy and Physiology, Math, and Cumulative Score. Copy of video submission confirmation from SlideRoom If enrolled in a previous nursing program, the School of Nursing reserves the right to request further documentation regarding your academic status.

I understand that late applications and documents will not be accepted and incomplete applications will not be considered. I understand that I will receive two separate decision letters one from the university indicating my admission status into George Mason University and one from the College of Health and Human Services indicating my admission status into the nursing program. I understand that failure to disclose all the institutions I attended, currently attend, or plan to attend on the George Mason Undergraduate Application prior to the start of the nursing program will jeopardize my BSN Application and the transfer of courses into the university. I understand the Supporting Documents Checklist, along with all supporting documents, must be submitted in one envelope at the same time, and received or postmarked by BSN Application deadline. I understand all prerequisites must be complete by the end of Mason s spring semester (May 20) for fall admission no exceptions. Prerequisite courses may not be completed during the summer session. I understand if I am offered admission to the BSN program and I fail to complete the nursing, Mason Core, and elective prerequisites by May 20 th the School of Nursing will rescind the offer of admission. Professional Conduct Policy: I understand all CHHS students are expected to conduct themselves professionally at all times. This means that certain behavior is prohibited, including excessive absences, verbal abuse, insubordination, and behavior that threatens the safety of a client, another student, a faculty/staff member, or other health care provider when the behavior occurs within the context of the academic program. CHHS reserves the right to place on probation, suspend, or dismiss any student in its programs who engages in such conduct. Students disciplined for such reasons have the right to appeal to their department chair or director. Academic Grade Standards: I understand that a passing grade is C+ (77%) in the BSN program. The grade of C- or lower in a nursing course may prohibit further progress in the major until the course is repeated and a satisfactory grade of C+ or higher is earned. Only one repeat is allowed. A second failure of a nursing course leads to termination from the BSN program. Alternate Education Plan: I understand that meeting eligibility requirements only allows my BSN Application to be reviewed and does not guarantee admission into the program. I understand admission to the BSN program is competitive and for this reason I have made an alternate (backup) education plan. Guaranteed Admission Agreement (if applicable): I have read and understand the Guaranteed Admission Agreement as described on the university main Admissions Office website ( I understand that it is my responsibility to make certain I comply with the university requirements for GAA admission. Mason Core Waiver (if applicable): I hereby affirm and understand the Mason Core Waiver is awarded by the Mason Office of Admissions, not the CHHS, Office of Student Affairs or the School of Nursing. If I intend to seek a waiver, it is my responsibility to contact Mason s general Office of Admissions to confirm that I am eligible to receive the waiver. I understand if a waiver for general

5 education and elective course requirements is not awarded, I will not be eligible for BSN Admission since all BSN prerequisite course requirements will not be met.
