3 The North Country Telehealth Partnership is northern New York s leading agency focused on increasing access to health care through the innovative use of telehealth and telemedicine technology. Mission: To provide local health care providers the technical assistance they need to develop and expand telehealth services, which improves access to high-quality health care for medically under-served and rural North Country residents. Services: The North Country Telehealth Partnership (NCTP) is your telehealth/telemedicine resource, focusing on assessing need and readiness of organizations looking to implement telehealth into their existing workflows. The NCTP also researches relevant articles and data, connects organizations to specialists, provides workflow and infrastructure support, assists with finding vendors, provides insight on equipment, and stays current on state and federal reimbursement and regulations. The North Country Telehealth Partnership is a joint project between Fort Drum Regional Health Planning Organization (FDRHPO) and Adirondack Health Institute (AHI). The NCTP serves an 11-county region: Clinton, Essex, Franklin, Fulton, Hamilton, Jefferson, Lewis, Saratoga, St. Lawrence, Warren, and Washington. 5 Presentations Diversified Partnerships in Telehealth Dr. Irina Gelman Keynote speaker, Dr. Gelman, is working on her dissertation entitled, Telehealth Initiatives in Rural Areas. Her focus on improving access to care, thereby directly impacting population health, culminated in a pilot project geared towards a larger countywide telehealth initiative in Fulton County, NY. This telehealth initiative includes Fulton-Montgomery Community College, which is at the epicenter of the project. The college acts as a spoke site with a telehealth location in the physical education building. Other spoke site locations include the Fulton County Office for the Aging and Youth and the health department. This keynote session will dive into the collaborative efforts put forth by numerous stakeholders across multiple counties to achieve greater access to care as a part of the Triple Aim. Telemental Health: Overcoming Challenges in the Rural Landscape Jay Ostrowski Many individuals who live in rural or underserved areas cannot easily access mental health or behavioral health care. Utilizing telehealth can often address these needs by enabling the provision of high-quality, real-time telemental health and telebehavioral health services to many who might not otherwise receive it. This session is for planners and providers who are interested in learning what is involved in planning and implementing telemental health or telebehavioral health services. Areas covered will include readiness assessments, laws, ethics, technology, reimbursement, referral development, provider and staff training, direct-to-consumer services, and more.

6 Presentations Virtual Care: The New Front Door of Health Care Keith Algozzine, Dr. Patrick Michel, Michael Walsh Virtual care is becoming the new front door to our health care system. Attendees of this session will learn how virtual care can help assist employer groups by reducing cost, improving quality, and increasing patient satisfaction. Virtual care is provided at the right place at the right time, and requires the highest trained medical professionals to be front and center. Attendees will see real-life examples of this happening in our region today. New York State Telehealth Regulation and Reimbursement Update Rae Ann Augliera, Joan Sicard, Ronald Bass, Keith McCarthy, Sara Osborne, Deborah Egel Moderated by Dr. John Morley This afternoon panel brings together key decision makers from three New York State government agencies the Department of Health (DOH), the Office of Mental Health (OMH), and the Office of Alcoholism and Substance Abuse Services (OASAS). Panelists will discuss current telehealth regulations and how all three agencies are working in concert to expand these regulations to make reimbursement and access to care easier for practices and patients around the State.

7 Presentations Tele-Collaboration: Working Together to Drive Patient Engagement and Reduce Hospitalizations Bonnie Ohmann, Jennifer Newberry, Mark Spiezio Moderated by Courtney Shaler Discover how a rural Northern New York hospital pulled together various health care agencies to implement and deploy remote patient monitoring technology to help reduce avoidable hospitalizations; one of the main goals of the NYS Delivery System Reform Incentive Payment (DSRIP) Program. Learn how the collaborative efforts of the panelists have led to effective in-home interventions, reduced avoidable hospital use, and increased patient engagement, empowering patients to take charge of their own health care. Panelists include individuals from varying entities such as hospitals, public health departments, and EMS agencies focusing on community paramedicine. Teledermatology and Clinical Image Security Dr. Julie Lin Dr. Julie Lin, Associate Professor of Medicine in the Dermatology Division of UVM Health, will discuss the uses of teledermatology, and why the security of storing and transmitting clinical images is so important. Telehealth in the Home Setting: Lessons Learned Elizabeth Zicari, Kris Smith, Kari Charette-Furnia Explore how Upstate New York home care agency HCR Home Care has expanded their home monitoring program from ten to more than 200 units. HCR will discuss the model they use and the strategies they have implemented to achieve success in a 22-county region. Speakers will share the lessons they have learned over the last several years of growth, including how not every patient is the right fit for telehealth, how the nurse can make a difference, target patient population, and the hidden costs.

9 Speaker Biographies Keith Algozzine, CEO United Concierge Medicine and Management Mr. Algozzine is a board-certified Physician Assistant (PA) and co-founder and CEO of United Concierge Medicine and Management (UCM). Before creating UCM, he was the Chief PA of Emergency Medicine for St. Mary s Hospital in Troy, NY, as well as part of the national ER start up management team for Pegasus Emergency Medicine. He was also a co-founding member and Director of Patient Services for a nationwide telemedicine company. Mr. Algozzine currently represents UCM on the American Telemedicine Association Accreditation Advisory Board and has been appointed as a member/advisor to the Clinical Practice Guidelines Committees dealing with telemedicine considerations in the practice areas for primary care and urgent care. Rae Ann Augliera, Health Program Administrator New York State Department of Health Ms. Augliera is a Health Program Administrator in the Office of Primary Care and Health Systems Management at the New York State Department of Health. She is the Department's internal lead for the Regulatory Modernization Initiative (RMI) Telehealth Workgroup, charged with creating a more favorable policy environment to advance the use of telehealth in New York State. Rae Ann has 15 years of experience in New York State government and holds a Master of Science in Health Policy from the University at Albany School of Public Health. Ronald Bass, Director, Bureau of Medical, Dental and Pharmacy Policy New York State Department of Health Mr. Bass is the Director of the Bureau of Medical, Dental and Pharmacy Policy in the Division of Program Development and Management for the New York State Department of Health. He is responsible for the oversight of payment policy for the New York State Medicaid Program, including integration of telehealth services as a program benefit. Kari Charette-Furnia, RN, BSN, Director of Patient Services North Country, HCR Home Care Ms. Charette-Furnia is the Director of Patient Services for the North Country at HCR Home Care, and has been with the agency for more than five years. She has 20 years of professional experience in health care nursing, home health care and care transitions. As a native of the North Country, Ms. Charette-Furnia received her education and training locally through the CVPH School of Radiology, Clinton Community College, and the State University of New York at Plattsburgh. Prior to her nursing career, she was a Radiologic Technologist working at CVPH. Ms. Charette-Furnia began her nursing career at CVPH Medical Center as an Oncology Nurse and later as an Operating Room Nurse before pursuing her passion in home care where she has spent the last 15 years of her career. Ms. Charette-Furnia is active in the Adirondack region on the Delivery System Reform Incentive Payment (DSRIP) leadership committees. Deborah Egel, Associate Attorney New York State Office of Alcoholism and Substance Abuse Services Ms. Egel earned her undergraduate degree in Nursing from Molloy College. She practiced as a nurse from 1994 through She then completed her Juris Doctorate (JD) degree at City University of New York School of Law. Ms. Egel is admitted to the New York Bar and is currently an Associate Attorney with the Office of Alcoholism and Substance Abuse Services (OASAS), where she provides legal assistance and support to the General Counsel and other senior OASAS administrators. Dr. Irina Gelman, MPH, DPM, PhDc, Director Fulton County Public Health Dr. Gelman completed a Master s degree in Public Health at Nova Southeastern University College of Osteopathic Medicine. With her preliminary medical background in surgery and clinical care, Dr. Gelman was appointed the Director of Public Health for Fulton County, NY in January of Currently a PhD candidate, Dr. Gelman is working on her dissertation entitled, Telehealth Initiatives in Rural Areas. Her focus on improving access to care, thereby directly impacting population health, culminated in a pilot project geared towards a larger countywide telehealth initiative in Fulton County, NY. Dr. Julie Lin, MD, Dermatologist and Associate Professor of Medicine University of Vermont Health Network Dr. Lin has been the Associate Professor of Medicine in the Division of Dermatology at the University of Vermont Larner College of Medicine since Dr. Lin earned her undergraduate degree at the University of Pennsylvania and her medical degree from McGill University Faculty of Medicine. She completed her dermatology residency and fellowship at the University of Pennsylvania and is board certified in dermatology and clinical informatics. Keith McCarthy, Director Bureau of Inspection and Certification at New York State Office of Mental Health Mr. McCarthy began his career as a NYS Senate Graduate Fellow in After four and a half years with the Senate Committee on Health, he moved onto the NYS Department of Health, working first with the Office of Continuing Care as a Health Program Administrator, then the Office of Health Systems Management as the Acting Director of the Bureau of Project Management. Mr. McCarthy joined the Office of Mental Health in 2013, becoming the Licensing Director in He has both his graduate and undergraduate degrees from the University at Buffalo, with an undergraduate degree in Economics and Social Sciences, and a Master s in Social Sciences.

10 Speaker Biographies Dr. Patrick Michel, HFM BOCES District Superintendent of Schools After seven years as District Superintendent of Hamilton-Fulton-Montgomery Counties BOCES, Dr. Patrick Michel has left an indelible mark on education in the upstate New York region he oversees. Prior to his appointment to HFM, Dr. Michel served as Superintendent of the Monticello Central School District in downstate New York. His educational administrative experiences include positions as Associate Superintendent at Clarkstown Central School District and as a Principal in Hyde Park and Pine Bush Central School Districts. Under his leadership, HFM BOCES has increased the role of technology throughout its programs and services, and strengthened local partnerships with Fulton-Montgomery Community College (FMCC), Fulton Montgomery Chamber of Commerce, and area businesses through student internships. Dr. Michel holds a Doctorate in Education from Columbia University, along with Masters degrees from Brooklyn College and Columbia University, a Certificate of Advanced Study from SUNY New Paltz, and a Bachelor of Arts degree from Manhattan College. Throughout his career, Dr. Michel has served on the Boards of the YMCA, Boys and Girls Clubs, Boy Scout Council, and The Fulton Montgomery Chamber of Commerce. Dr. John Morley, Chief Physician Executive, Population Health Adirondack Health Institute Dr. Morley is a highly-regarded Board-Certified Anesthesiologist who has been in practice for 30 years. He is also Board-Certified in Internal, Pulmonary, and Critical Care Medicine. Dr. Morley has served in a number of high-level administrative roles, including NYS DOH Medical Director for the Office of Health Systems Management, VP for Clinical Affairs for the Healthcare Association of New York State (HANYS), and Vice President of Medical Affairs and Medical Director at Albany Medical Center Hospital. Before joining AHI, he held the position of Chief Medical Officer at Jacobi Medical Center in Bronx, NY. Dr. Morley has also held a number of academic positions, most recently as an Associate Professor of Anesthesiology and Surgery at Albany Medical College. Dr. Morley earned a Bachelor of Science degree in Biology from Iona College and an MD degree from St. George's University. He received Residency training in Anesthesiology at Saint Barnabas Medical Center in Livingston, NJ, and Internal Medicine at Albany Medical College. He was also a Fellow in Critical Care at Cooper Hospital University Medical Center in Camden, NJ, and a Pulmonary/ID Fellow at Albany Medical College. Jennifer Newberry, Director of Patient Services and Information Systems Manager Essex County Public Health Ms. Newberry has been in home care for 31 years. Prior to moving to New York, she spent 23 years as a Director of Home Care in two separate agencies in West Virginia and North Carolina. She taught at Mountain State University until 2008, teaching a patho-pharmacology course to nursing students. When Ms. Newberry moved to New York, she was a developmental partner with the first Hudson Headwaters EMR in She was the Supervising Home Health nurse with Essex County Public Health for six years prior to becoming the Director of Patient Services. Since 2010, Ms. Newberry also owns Home Health Survey Solutions, LLC which provides training in compliance and survey readiness, medical billing and chart auditing services to home health care practices, health departments, and physicians. Bonnie Ohmann, Grants and Strategic Projects Manager Adirondack Health Ms. Ohmann has been with Adirondack Health since Prior to Adirondack Health, she worked for 12 years as the Saranac Lake Central School District s Chief Information Officer. She has also worked as an administrator for a Massachusetts based orthopedic practice, and earned early experience on Capitol Hill as a legislative liaison. Ms. Ohmann holds a Master s degree in Public Policy and Women s Studies from George Washington University. Sara Osborne, Associate Attorney New York State Office of Alcoholism and Substance Abuse Services Ms. Osborne is an Associate Attorney for the NYS OASAS and has been with the agency since Focus areas of practice for the agency include regulatory development, legislation, and capital projects. Ms. Osborne co-authored an article in 2010 for the New York State Bar Association Government, Law and Policy Journal on issues distinguishing state certified residential treatment from privately-leased properties often called sober homes. Prior to working at OASAS, Sara was an attorney for the NYS Legislative Bill Drafting Commission and in private practice as a court appointed law guardian in Saratoga and Warren counties. Ms. Osborne has taught disability law for Empire State College, worked as a court stenographer, and owned a resume writing business in Boston, MA. Ms. Osborne is a graduate of Syracuse University Maxwell School of Citizenship and Public Affairs (American Studies, BA) and received her Juris Doctorate from Albany Law School. Jay Ostrowski, MA, NCC, LPC-S, DCC, ACS, CEO Behavioral Health Innovation and Adaptive Telehealth Mr. Ostrowski is the CEO of Behavioral Health Innovation and Adaptive Telehealth. His passion is research and development in the telemedicine field. Thus far, Mr. Ostrowski has led development of eight HIPAA-secure telemedicine software platforms, created nine peer-reviewed telemental health training courses for national certification, and created many innovative niche treatment programs. He also serves as a consultative partner with the Mid-Atlantic Telehealth Resource Center, providing telemental health technical assistance helping organizations start-up and scale telemedicine services including telemental health, telebehavioral health, and telepsychiatry.

11 Speaker Biographies Courtney Shaler, Adirondack Rural Health Network Manager Adirondack Health Institute Ms. Shaler has been employed by Adirondack Health Institute (AHI) as the manager of the Adirondack Rural Health Network (ARHN) since 2014, a NYS DOH grant. ARHN functions as a seven-county multi-stakeholder, regional coalition that informs on planning assessment, and provides education and training to further the NYS Prevention Agenda. Additionally, she oversees the Community Health Assessment (CHA) Committee supporting the efforts of 13 hospitals and local public health departments across seven counties with their community health assessment needs. She has a Bachelor of Arts degree in Organizational Leadership and Business from the College of St. Joseph of Vermont, and an Associate degree in Applied Science in Physical Therapy from Herkimer County Community College. In 2012, she was honored as one of the region s leaders with a Post-Star s 20 Under 40 award. She is also certified as a Telehealth Liaison and Rural Grant Writing Specialist. Joan Sicard, Manager, Basic Benefit Unit, Bureau of Medical, Dental and Pharmacy Policy New York State Department of Health Ms. Sicard is a Registered Nurse by trade. She manages the Basic Benefit Unit in the Bureau of Medical, Dental and Pharmacy Policy in the Division of Program Development and Management, New York State Department of Health. This unit is responsible for reviewing new and emerging services and technologies for inclusion in the Medicaid benefit package and for developing coverage policies/guidance. Currently, they are working on the integration of telehealth as a Medicaid program benefit. Kris Smith, RN, COS-C, Director of Patient Services Finger Lakes, HCR Home Care Mr. Smith is the Director of Patient Services for HCR/Finger Lakes region. He also oversees the statewide Telehealth Program of HCR Home Care, and has been with the agency for more than six years. He has nine years of professional experience in nursing, telehealth, leadership and staff development. Mr. Smith began his nursing career at the Wyoming County Community Hospital and University of Rochester Medical Center, as a Registered Nurse. His roles in home care include Community Health Nurse, Assistant Director of Nursing, and Director of Patient Services. Mr. Smith received his RN from Alfred State College, and a Bachelor s of Science in History from the University of Buffalo. Mark Spiezio, Mobile Health Care System & Cambridge Valley Rescue Squad Operations Chief Mr. Spiezio has more than 28 years of active service as a volunteer and career EMS provider. He founded the Southern Washington, Northern Rensselaer County Mobile Health Care System. He is a certified Paramedic, Instructor Coordinator, and Program Director for the New York State Department of Health, Bureau of EMS. Mr. Spiezio has spent 27 years as the Chief Operating Officer of the Cambridge Valley Rescue Squad, and is a member of both the National Association of EMS and the New York Mobile Integrated Health Care Association. He is a champion of community paramedicine and has recently utilized remote patient monitoring technology to enhance the existing Community Check Program established by the Cambridge, Salem, and Greenwich Rescue Squads. Michael Walsh, CEO NYSTEC Mr. Walsh is the President and CEO of NYSTEC, an independent, not-for-profit technology consulting company with offices in Albany, New York City, and Rome, New York. Four of NYSTEC s seven Practice Areas are centered on Health-IT, assisting government agencies, health care providers, and institutions with technology planning, acquisition, project oversight, and security. He is also the co-founder of IgniteU NY: an independent program focused on developing the entrepreneurs of New York State. Since its founding in 2015, IgniteU has sponsored two health-centric events: The Health-IT Forum and Showcase, and the Population Health Innovation Summit. Mr. Walsh is passionate about supporting, encouraging, and promoting technology innovation in health care, which fuels creative solutions to improve the patient experience and health care affordability for all New Yorkers. Elizabeth Zicari, President HCR Home Care Ms. Zicari is the Administrator and President of HCR Home Care, New York s most geographically-expansive home care provider, serving 22 upstate counties. She has 30 years of professional experience in nursing, administration, and business management, including 15 years at HCR, and has expertise in home health care, transcultural care, regulatory compliance, and quality improvement. Ms. Zicari began her nursing career as a commissioned officer in the U.S. Public Health Service/Indian Health Service and has specialized in community health care in both urban and rural settings since that time. She currently serves on the Board of Directors of the Home Care Association of New York State, the New York Organization of Nurse Executive and Leaders, and is President of the Finger Lakes Organization of Nurse Executives. Ms. Zicari is active statewide on the Delivery System Reform Incentive Payment (DSRIP) Program leadership committees, is a project champion of Hospital-Home Care, and is a member of the MSSNY-HCA joint task force to remove obstacles to access community-based, long-term care. She previously served on the NYS Clinical Advisory and Value-Based Purchasing (VBP) Groups and holds a Certificate in Executive Nursing Practice (CENP), and is a Johnson and Johnson/Wharton Executive Nurse Fellow.
