2 Welcome to the annual report April 2011 to March It provides an overview of the work undertaken in this time period by the BBV and Sexual Health Promotion Team in NHS Lanarkshire. 3 Annual Report of the BBV & Sexual Health Promotion Team Condom Distribution Schemes (CDS) The CDS provides free condoms for all living, working or studying in Lanarkshire by operating three separate schemes. These are Free Condoms No Fuss- C-card, Free Condoms No Fuss Just Ask and Get Rubbered. Free Condoms No Fuss - C Card Scheme The C Card scheme continues to function well. There are currently 98 outlets registered with the C-Card scheme. There are 53 GP practices and Health Centres and 45 pharmacies registered. There has been considerable interest from pharmacies wishing to become C Card centres and a list has been created to keep their details on file. However, a number of reasons at present mean there will be no further C Card centres established. The new NHS Greater Glasgow and Clyde condom scheme known as Free Condoms, has now been rolled out. For more information, please visit This has seen some new pharmacies and health centres join and we now have 7 (increased from 4 previously) outlets within the Northern Corridor and Cambuslang and Rutherglen areas. However, these outlets currently remain under the remit of NHS Greater Glasgow and Clyde. There has been sustained communication with the Manager of the new scheme and literature regarding Lanarkshire sexual health services and the C Card scheme are to be made available within the outlets. The Way Forward - The proposed electronic C Card service is still at planning stage and will not be introduced until a CDS Co-ordinator is recruited. C-Card leaflet C-Cards and leaflets are being distributed widely throughout Lanarkshire via the usual outlets of GP practices, health centres and pharmacies. The Sexual Health leads identified within Unit health improvement teams have agreed to widen the distribution of the C Cards and leaflets to their partners within their localities. C Cards and leaflets have also been distributed to agencies on request as part of their Healthy Working Lives initiatives. 3 Free Cs Pharmacy Initiative this continues to function, though has required intervention from the Condom Distribution Scheme Manager and the Chief Pharmacist (Primary Care). A reminder has been to staff who deliver the free Emergency Hormonal Contraception (EHC) service about the ordering system of condoms required for dispense with EHC. This follows on from a low uptake of condoms in the third quarter and nil in the last quarter of the year. 3

4 Free Condoms - Just Ask No. of agencies Joined during Total left Rationalised It has not been possible to complete the review of the scheme that took place in Whilst undertaking the review, agencies were contacted to find that some had either ceased operation or changed their remit and no longer fitted the Just Ask criteria. Rationalisation occurred where 6 services came under one point of contact. This happened for 2 such services. Therefore, what were previously listed as 12 individual Just Ask services, are now counted as two services. There have been a number of new requests to become a Just Ask agency; however, the criteria required to join the service were not fulfilled. Just Ask GPs This service was launched in April 2009 and aimed at GPs with Special Interests (GPwSI). The service is aimed primarily at clients from vulnerable backgrounds or known to engage in what is considered to be high risk behaviour. GPs have a stock of condoms to dispense to clients attending them in a brief intervention setting. 11 GPs and 3 practice nurses have now registered with the service. This will not be rolled out to other practices in the near future. This scheme was launched as a pilot and was included in the general Just Ask review. However, as the review was not completed, there have been no recommendations made as to the validity of rolling out the pilot. Anecdotal feedback from the nurses and GPs providing the service indicate that it meets a need in their practice, therefore the service will be continued in its current form until further review can take place. Resource boxes of training materials have been distributed throughout all maternity settings. Discussions regarding uploading the training to FirstPort pages and the sexual health website have taken place and will be progressed further. The training package is also to be evaluated by NHS Education for Scotland (NES). 4

5 A Staff Midwife from Wishaw General Maternity Unit and a BBV Specialist Nurse from Lanarkshire HIV, AIDS and Hepatitis Centre (LHAHC) continue to deliver the training. A working group has been set up to look at the varying educational needs of Maternity Services staff in relation to BBVs and options as to how the training will be tailored to address these needs have been suggested. For further information on the Condom Distribution Schemes and BBV and HIV Antenatal Screening Training for Midwives, contact Jacqueline Martin, Senior Health Promotion Officer on or Social Marketing Vue Cinema, Hamilton over the course of the year C-Cards and leaflets continue to be displayed alongside the campaign posters within the washrooms. The General Manager is happy with the process and has received no complaints. MSM Social Marketing (Make Your Position Clear campaign) The audit work around the dissemination of the materials at a local level took place in March It asked a number of questions of those on the original campaign material distribution mailing list within Lanarkshire. Topics covered were: whether materials were received if the quantities received were adequate if the materials were displayed and if they received feedback from staff or members of the public if the materials were not displayed, some explanation as to why this decision was taken 317 self completion questionnaires were sent out in Lanarkshire and 46 responses were received (15%). Main points from the audit include: 5

6 Pharmacies were the most supportive with 77% of respondents displaying the materials. Their comments tended to be balanced and more in line with our equality and diversity values. GP responses were mixed with 50% of respondents displaying the materials. There were some supportive comments but it was clear there was a weighting towards less positive views such as we are a family practice and too blatantly sexual. Responses from dentists were least supportive with only 21% displaying the materials. Many felt the materials were totally inappropriate for the setting with most respondents citing the presence of children and families as explanation. A number felt the issue of HIV in gay men is irrelevant to dentistry, with one respondent stating they wished to improve oral health not sexual health. These responses highlighted a need to consider further work with dentists in the future to explore these issues. Professor Paul Flowers from Glasgow Caledonian University undertook a formal evaluation of the campaign. The evaluation focused on: Target group recognition of the campaign Target group understanding of the campaign Types and levels of exposure/interaction with campaign elements Impact of exposure/interaction with the campaign on attitudes, intentions and behaviours relating the core campaign messages. The final evaluation report for the campaign has been completed. The project team are planning a seminar with Professor Flowers with a view to working through each section of the report in detail. After this has happened the report will be signed off by the group and will be disseminated through the usual networks with the findings tabled for discussion in the relevant local, regional, and national fora. It is hoped that this exercise will help to inform future campaigns in terms of producing material that will be used in mainstream settings, reducing waste by supplying the correct amount and providing some basis through which to address issues around equality, diversity, and discrimination. The project team presented at the WellBeing In Sexual Health West of Scotland Regional event in September The presentation focused on breaking down barriers and the targeting of vulnerable populations, in this instance, men who have sex with men. The campaign received a Highly Commended award at the annual Communicating Health Awards which is run by the Association for Healthcare Communications and Marketing. The feedback received is that this was a very strong field with a wide range of UK health campaigns in the running so the highly commended award was a significant achievement. 6

7 Chlamydia Campaign The Chlamydia campaign continues to operate throughout Lanarkshire. It should be noted that, in addition to on-going publicity within the Chlamydia campaign, there have been Facebook adverts running since December These adverts target males and females aged between 16 and 24 years old across the NHS Lanarkshire area. The aim of this type of social marketing is to encourage those over the age of 16 to visit the NHS Lanarkshire sexual health website; where they can access information on the C Card scheme and view other pages related to sexual health information. The marketing of the C Card scheme on Facebook began early in 2011 and was undertaken with the Chlamydia and Long Acting Reversible Contraception campaigns. The campaign has proven to be successful in reaching its target group and the intention is to continue with this approach. The BBV and Sexual Health Development Officer at LANDED Peer Education Service co-ordinated a project on behalf of the team to roll out work in pubs and clubs across Lanarkshire to raise awareness of Chlamydia and encourage testing. A number of events were held and assessed as being useful at the time. This work is discontinued for the meantime. Between April 2011 and January Chlamydia postal testing kits (PTKs) have been requested via the sexual health website. This has included working within the Lanarkshire area to support the roll out of the campaign at the local level. This officer is an active member of the National Guidance Development Group for prevention of HIV transmission in MSM as part of the work on action 9 of the HIV Action Plan for Scotland. The drafting of the guidance has been completed and the document is now out for consultation. For more information on social marketing, work with voluntary organisations and Chlamydia please contact: Colin Anderson, Senior Health Promotion Officer on colin.anderson@lanarkshire.scot.nhs.uk or Sexual Health and Relationships Education (SHRE) for parents and carers The Training and Education Manager continues to play an active role in promoting the work of Speakeasy in Lanarkshire. Recent developments have seen the fpa (family planning association) secure a small grant which is being used to employ a former Speakeasy employee. The team had positive working relationships established with the fpa and will continue to foster these by supporting the worker and providing information, support and ensuring mutually agreed outcomes are met. 10

11 The team continues to support schools with their work aimed at parents and carers. This support is in different forms and ranges from providing information and resources to speaking at events. BBV and Sexual Health Training and Education Two Sexual Health and Relationships Education (SHRE) training courses, aimed at staff currently delivering, or who may be required to deliver, sexual health work in schools, have been delivered in the last year. The team recognised that it is difficult for staff to be released for training and shortened the course and numbers have stayed constant. This course offers an optional half day looking at what work is happening in the Education sector and this has been an ideal opportunity to showcase and consult on the new Curriculum for Excellence Relationships, Sexual Health and Parenthood (RSHP) materials. All sessions evaluated positively. Staff who attended came from a wide range of NHSL and partner agency backgrounds and all felt more confident and competent to undertake further sexual health related work. The team has been involved in both the support of others working to raise awareness of relationships and sexual health issues through training / information sessions and in the planning and delivery of bespoke training courses to meet needs such as Community Learning and Development and First Steps. This year saw the team decide not to organise any further Sexual Health Fast Track card information sessions in order to allow the Looked After and Accommodated Children and Young People SHRE policy and guidance supporting training to be prioritised. SHRE for Educational Establishments This has been a busy year for the team in connection with this work. New RSHP materials were created in partnership with both local authorities over the year and the pilot phase was a success. Forty four establishments took part and 37 in total submitted extensive feedback with the overwhelming majority (95%) being impressed with the draft versions and suggesting few changes. The team appreciated the level of feedback, engagement and comments made by teaching staff. The launch of the new Relationships, Sexual Health and Parenthood Frameworks and supporting resources for Early Years, Primary Schools and Secondary Schools took place in January The event was a tremendous success with more than 125 staff from across local authority and health in attendance. It focused on inputs from health and education practitioners on their experiences of delivering this topic within the various settings as well as a Scottish Government Support and Learning Unit representative. The new resources are currently being delivered via the local authorities to each establishment and will be completed prior to the end of the academic year. 11

12 SHRE teacher training programmes have been considerably revised this year to take time constraints into account. The team is confident that for most whole school settings, one 90 minute session covers the main issues and allows time for discussions. For in-service training, the content is now shortened to a three hour session and includes time for staff to familiarise themselves with the new materials. A positive move forward has been the SHRE teacher training delivery to primary NLC Probationer staff. A formal commitment has been made to both local authorities that the team will continue to support the delivery of training. In North Lanarkshire Council, two in-service SHRE training sessions for primary and secondary teachers were delivered. One course was delivered to primary probationer staff. Whole school training sessions have been delivered to 15 schools. Total number of staff and parents reached is 302. In South Lanarkshire Council, three in-service SHRE training sessions for primary and secondary staff have been delivered. Whole school training sessions have been delivered to 22 schools. Total number of staff / parents reached is 441. The team have secured funding to recruit two part time Curriculum Support teachers to further develop work in the RSHP field for 12 months. One will focus on supporting primary schools and the other will develop RSHP materials for Additional Support Needs schools that better suit children and young people in these settings. RSHP work, as required by the Curriculum for Excellence Health and Wellbeing section, has resulted in an increased demand for support from this team. The team has provided support to the local authorities to look at RSHP issues for multicomposite classes and plans to work with South Lanarkshire Council to scope out RSHP issues in the Catholic schools sector. It continues to do its best to provide the high levels of support for which it is known. Looked After and Accommodated Young People This has been a busy year for working on new developments to better support these children and young people and the staff and carers who look after them. May 2011 saw the launch of the SHRE Policy and Guidance for North and South Lanarkshire children s unit staff and foster carers and each unit and carer has received a copy. Much work has been done to plan and develop an appropriate training course to accompany the policy and ensure that it remains a relevant document. The course needed to be responsive to needs expressed, have staff and carers consulted as to content and relevance and awareness raised of dates. Partners have been in agreement regarding the importance of this training and this has been reflected by the training becoming mandatory for staff and carers. Training delivery will be via a training team which includes members of the team, Health Improvement Unit Sexual Health leads and staff from voluntary organisations 12

13 (LANDED and THT). The first course was delivered in April 2012 and evaluated very positively. A leaflet for looked after and accommodated young people highlighting the important work being done with staff to help them feel more comfortable talking about relationships and sexual health has been disseminated. Along with the leaflet, various resources are being distributed to the units for staff and young people to help further support this work. Foster carers will also have access to the resources via foster carer libraries. Development of new young person BBV and Sexual Health resource The Training and Education Manager is currently involved in the development of a new resource for children and young people, working in conjunction with NHS Glasgow and Clyde and NHS Highland. Two booklets looking at growing up and relationships are being developed: one for children 8-12 years and one for young people years. It is anticipated that the booklet content will be finalised in May 2012 and copies made available thereafter. It has now been moved to a new platform that will make it much easier for the team to make amendments on an on-going basis. The new website went live in March The table below shows the number of hits received on the website from April 2011 to March

14 A Facebook campaign highlighting the C Card Service, Chlamydia Testing and LARC (Long Acting Reversible Contraception) started in December 2011 and as a result, there has been a greater amount of traffic being driven to the site. The Facebook campaign is also ensuring that particular age groups and areas of Lanarkshire are being targeted. It is also worthy of note that the number of people accessing the site from a mobile device has increased significantly. The new site will be easier to view on a mobile device and this will be very useful given the numbers of people accessing the site via this medium. Partners are encouraged to contact the Health Promotion Officer if they think any information should be added to the website. Black and Minority Ethnic Resource for communities in Lanarkshire The booklet Using Healthcare Services in Lanarkshire, which was developed by Terrence Higgins Trust, has now been translated into Polish, Swahili, French, Punjabi, Urdu and Chinese. The booklets have been uploaded to the website and can be accessed by visiting clicking on the Resources page and then selecting Leaflets from the list of options. World AIDS Day 2011 Activities across Lanarkshire The BBV & Sexual Health Promotion Team continues to support and promote World AIDS Day on the 1 st of December each year across Lanarkshire. A number of awareness raising activities took place which were supported by NHS Locality Staff, Voluntary Sector Agencies and a number of staff members from NLC and SLC through the collaborative themes working group involvement. (The collaborative themes group incorporates Suicide Prevention Week, Alcohol Awareness Week, Scottish Mental Health Week and World AIDS Day.) Red Ribbon flags flew from North Lanarkshire Council Headquarters in Motherwell, South Lanarkshire Council Headquarters in Hamilton and University of West of Scotland in Hamilton. Events received press coverage from STV Lanarkshire in Motherwell, Hamilton and Airdrie areas. This was information on their local websites to raise awareness of the red ribbon flag flying in these areas and there was also general information around HIV. Further press coverage was noted in the Hamilton Advertiser. Resources were provided to a variety of agencies across Lanarkshire to support awareness raising events.
