1 To: CSM Medical Staff, Residents, Nurse Practitioners & Physician Assistants From: Richard Shimp, MD Chief Medical Officer Robert Lyon, MD Chief of Staff Guillermo Martinez-Torres, MD Medical Staff President, CSM-M Bruce Rowe, MD Medical Staff President, CSM-O Date: August 5, 2015 Re: New Threshold Criteria to Admit/Attend Patients in Adult Critical Care and Acuity Adaptable Units at CSM FCCS Course Registration (to Fulfill Threshold Criteria) Effective July 1, 2016 (approximately 10 months from now), the Medical Staff at Columbia St. Mary s will enforce a new policy which requires physicians to meet certain threshold criteria before admitting/attending patients in the Adult Critical Care and Acuity Adaptable Units. This new rule also impacts residents, nurse practitioners and physician assistants who manage patients in the aforementioned critical care units under the supervision of or in collaboration with the admitting/attending physician. The new rule does not apply to subspecialists who consult critical care patients. The new threshold criteria include: Successful completion of a critical care fellowship; or Current board certification or eligibility in one of the following specialties or subspecialties by the American Board of Medical Specialties, or the American Osteopathic Association: cardiology, cardiothoracic surgery, critical care medicine, general surgery, obstetrics and gynecology, neurosurgery, or vascular surgery; or Successful completion of the course entitled Fundamentals in Critical Care Support (FCCS) or equivalent training as determined by the Adult ICU Medical Director(s). FCCS course recertification or equivalent is required every four (4) years. Please continue to read this important message if you wish to pursue FCCS course certification sponsored by the Society of Critical Care Medicine (SCCM). To achieve FCCS course certification, the candidate must successfully complete both of the following: Part I: FCCS on-line course, including post-test (may access anytime on-line see FCCS Course Enrollment Job Aid for instructions); and Part II: FCCS one-day, hands-on skills assessment (registration required via Docport see FCCS Course Enrollment Job Aid for instructions). Beginning August 12, 2015 and continuing through mid-june 2016, Columbia St. Mary s will provide free access to the on-line FCCS course. Columbia St. Mary s will also offer three (3) options for the one-day, hands-on skills assessment:

2 Columbia St. Mary s Fundamental Critical Care Support (FCCS) Page 2 Thursday, October 8, 2015 (Registration deadline on DocPort September 24, 2015); Thursday, February 18, 2016 (Registration deadline on DocPort February 4, 2016); and Thursday, June 16, 2016 (Registration deadline on DocPort June 2, 2016). Enrollment is open to all interested members of the CSM Medical Staff, residents, nurse practitioners and physician assistants. It s also open to other CSM associates who provide adult critical care services to CSM patients, space permitting. CME credits are available for both the on-line course as well as the one-day, hands-on skills assessment. The Society of Critical Care Medicine will provide CMEs for the on-line portion; CSM for the one-day, hands-on skills assessment. Refer to the FCCS Course Enrollment Job Aid for instructions regarding on-line course enrollment and separate registration process on Docport for the one-day, hands-on skills assessment.

Participants must return textbooks at time of skills assessment to avoid textbook replacement fee. To achieve FCCS course certification, the candidate must successfully complete both of the following: Part I: FCCS on-line course, including post-test; and Part II: FCCS one-day, hands-on skills assessment Part I: FCCS Online Course Enrollment You are invited to enroll in the Society of Critical Care Medicine s LearnICU Classroom. Please click or copy and paste the following Student Enrollment Link into your web browser to enroll in the Fundamental Critical Care Support (FCCS) course provided by Columbia St. Mary s: Follow the instructions provided below in order to create your Society of Critical Care Medicine account and enroll in the FCCS course. 1. If you ve clicked or copied and pasted the link provided above you should now be on the Society of Critical Care Medicine Web site where you can begin to create an account. 2. Create your Customer ID and logon information see Figure 1 below for reference. Page 1 of 6

4 Figure 1 4. Once you ve successfully created your account, please log your Customer ID and Password in a safe place. You will need the logon information every time you need to access the Society of Critical Care Medicine Site and Fundamental Critical Care Support Course. 5. To continue with your course enrollment, click on the link highlighted by the red arrow and text box shown in Figure 2 below. Figure 2 6. When you click the link you will be directed to the page shown in the Figure 3 below. To finish the enrollment process, click the Enroll Now button as shown in Figure 3. Page 2 of 6

5 Figure 3 7. Once you ve selected Enroll Now you will have successfully enrolled in the Fundamental Critical Care Support Course. 8. You will now have access to the course under the MY LEARNING tab that is located on your Society of Critical Care Medicine profile page. See Figure 4 below for reference. Figure 4 9. To enter the course, select the FCCS Industrialized Healthcare Online course located under your Learning List menu shown in the picture above. 10. Once you select the course you will be directed to a page shown in the screenshot below. If you select the Launch button the page will open to a welcome window where you can begin taking the course. 7 Figure 7 CME credits are available through SCCM for the online portion of the course. Columbia St. Mary s provides CME credits for the hands-on skills assessment that you will enroll in Step 2 below. Part II: Hands-On Skills Assessment Registration (via Docport) Click the Docport link below to access the registration form. Steps are outlined in the screen shots below. Please note: the registration deadline for the hands-on skills assessment is two weeks before the day of the scheduled event. You are only required to complete one of the hands-on skills assessment dates in order to achieve FCCS course certification. Deadlines to register for one-day hands-on skills assessment listed here: First scheduled hands-on skills assessment is to take place on October 8, 2015 deadline for registration is September 24, Second scheduled hands-on skills assessment is to take place on February 18, 2016 deadline for registration is February 4, Third scheduled hands-on skills assessment is to take place on June 16, 2016 deadline for registration is June 2, Follow this link to register On the Docport FCCS hands-on skills assessment registration page, please enter your information in the fields provided and press the Submit button.
