2 Welcome Dear Candidate Thank you for your interest in the role of Associate Medical Director for Primary Care at the NHS Wales Informatics Service. This is an organisation with an important role in the delivery of world class, integrated care to patients and citizens. It s true to say that our health service in Wales depends on the national services we provide. As the Chief Clinical Information Officer for Wales and Medical Director at NHS Wales Informatics Service I am consistently impressed with the strong commitment in Wales to collaboration and partnership. While we face many similar challenges to the NHS elsewhere in the UK, the health policy in Wales is different from that operating in NHS England, which is based on a competitive, market-driven system. In Wales, policy is the responsibility of the Welsh Government and the emphasis is on working together, collaboration and coproduction. We support the improvement of patient care through collaborative provision of high quality information, systems and technologies; 'Once for Wales.' In practice this means that we work with our partners in health boards, primary care and other partners in NHS Wales, the public sector and voluntary sector to develop new e-health and care services that are designed to make it easy for health professionals to carry out their daily tasks regardless of the health care setting with the aim of technology and information facilitating the deliver the best possible care to patients. As an organisation we are ambitious and are involved in leading the delivery of Wales e- health and care strategy. As Associate Medical Director for Primary Care, you will be joining a committed team who view the use of information and technology as paramount to enable the transformation of health and care services in Wales. As such we are looking for an exceptional candidate who is committed to making a real and sustained difference to informatics in Wales. 3 The NHS in Wales The health of the people of Wales continues to improve. Significant progress has been made against the big killers of cancer and heart disease in the past 20 years. Male premature deaths from heart disease have reduced by 60 per cent since There has been considerable success in concerted efforts against smoking, alcohol and substance misuse and risk behaviours. This has resulted in year on year improvement in health outcomes as a nation. The recent Nuffield Report comparing the health systems in the UK provides a good overview: Please click on this link to view the report in full: Despite this progress, we are not satisfied and there remains a commitment and aspiration to work together to further reduce the impact of poor health decisions. There are however considerable challenges in levelling up the health status for the whole population. Traditional pockets of deprivation exist, largely in the South Wales Valleys, but also in dispersed communities across the country. It is the stated aim of the Welsh Government to tackle the inequalities that exist in Wales by pursuing a health economy policy approach, supported by coordinated and determined research and intelligence and the use of new technologies. Wales is very proud that one of its sons, Aneurin Bevan, was the father of the NHS in the United Kingdom. Wales is also very proud of its creativity and ingenuity. In 2009 the NHS in Wales was reconfigured with the formation of seven Health Boards taking responsibility for the planning and delivery of all services in their areas from primary to tertiary care. The reforms put in place are designed to guard jealously Bevan s legacy of a health service which is free at the point of delivery on the basis of need but to do so recognising the huge pace of change in healthcare, the rapidly changing nature and scale of demand, and the overriding desire to deliver a citizen focused service. Underpinning the NHS in Wales is: A desire to re-establish the key and close links between health service planning and delivery and the specialist public health function The delivery of a high quality all-wales public health service focused on improvement of the health of the people in Wales and working seamlessly with the reconfigured National Health Service Recognition that in a country of three million people it is possible to use a planningbased approach to the design and delivery of service 3
4 An absolute imperative to move services closer to people and by doing so rebalance the priorities afforded to acute, primary and community based care The establishment of new accountabilities between health service organisations, the people who lead and work in them and the people they serve The imperative of high quality performance and the need to manage that performance effectively and rigorously The absolute requirement to build a health service which is sustainable into the future An introduction to NWIS There are seven health boards in Wales and three trusts Velindre NHS Trust, Welsh Ambulance Trust and Public Health Wales The NHS Wales Informatics Service has responsibility for the design, deployment and management of digital health services for NHS Wales. 4
5 Our work covers the development and procurement of new national products, in collaboration with health boards and health professionals, and the delivery of operational technology and information services. We also have a national role in supporting the use and uptake of new technologies by health professions and the professional development of informatics staff. Our strategy is based on the principle that effective and efficient information systems will give: Care providers the information they need to deliver better outcomes, more safely and more efficiently, and Welsh citizens access to information they need to support their own health and wellbeing. We employ around 500 people based in offices in Cardiff, Bridgend, Swansea, Carmarthen, Pontypool and Mold, and have an annual budget of around 57 million. Key business areas include: Designing and developing the new integrated digital services required for the improved quality and outcome of care, this includes the current procurement of a new Community Care Information System Supplying over 70 operational ICT services and service support to the NHS in Wales covering both primary and secondary care Providing the technology infrastructure and architecture to support national systems Analysing NHS data and making it available in easy to use formats to support planning and decisions Setting the direction for robust and integrated primary and community care systems Setting the standards for health informatics professional development Working collaboratively to develop information governance processes that maintain the balance between patient confidentially and information sharing Integrated Health Record Like many other countries across the world Wales is in the process of enabling an integrated healthcare record for patients. A record that contains the health information that is most important to assist patients as they move through the healthcare system, so that the right information is provided to the right people at the right time, wherever care is delivered. All components of the integrated health record are fully implemented or in the process of implementation across Wales. The key components of the single integrated health record are: Welsh Clinical Portal brings together information held in the many hospital computer systems that doctors, nurses, clinicians and pharmacists use on a day-today basis. It puts all the information about the patient in one place, saving time and making it easier for hospital health staff to do their job. It is used to view documentation, order pathology and/or radiology investigations and to see the results, and for medicines management in the hospital 5
6 Welsh GP Record - enabling clinical staff to see the important part of the GP record outside the confines of the local GP surgery Welsh Clinical Communication Gateway - secure data transfer between primary and secondary care using a secure information service Master Patient Index - identifying the right patient at every contact and a unique ID - for every member of staff, to provide the information services they need individually Networked services - to support pathology and radiology requests and results My Health Online allows patients to order repeat prescriptions and book GP Appointments using the internet Next steps are to build on the foundations described above to reflect current and emerging models of health care and build and support an integrated healthcare record For more information on our projects and services go to: Strategic Change NHS Wales faces a period of unprecedented large scale, complex change. Welsh Government has made very clear it is commitment to improving the health of people in Wales, to create 21 st century healthcare and to take tough action on health inequalities over the next 5 years. Together for Health In 2012 Welsh Government launched a five year vision for the NHS in Wales Together for Health. This set out a decisive shift in the balance of services away from a high cost model based on dealing with people whose health is already deteriorating to one based on local, early action to promote and protect health and independence. It included a commitment to one system for health; with more personal care and supported by state of the art information systems. 7 For further information relating to Prudent Healthcare please visit NHS Wales Informatics Service Approach The Informatics Programme for Wales is coordinated and implemented by the NHS Wales Informatics Service in partnership with the health care providers. The over-arching principles we use to plan the way forward are: Doing things Once for Wales - creating a single integrated common informatics platform for Wales Undertaking clinical and technical design together Focusing on information services not systems Making the best use of what we already have Doing things in common by design or agreement Ensuring safe migration to new services. Ensuring that common national information services are patient centric working seamlessly across organisations and care sectors. Priorities identified in our three year plan are: The development of common infrastructure, including data centres, data networks (e.g. Public Sector Broadband), mobile platform, data storage facilities including private cloud capabilities extending across the NHS in Wales Continued roll-out of national systems to accurately identify patients and staff across Wales Further development of national systems that share essential information and messages between services, such as the Welsh Clinical Portal, Welsh GP Record (WGPR)) and Welsh Clinical Communications Gateway (WCCG) Progress adoption of systems that give better access to patient with systems such as My Health Online (MHOL) Further roll-out of common systems, such as new GP Clinical Systems, PACS for Radiology and imaging sharing services Developing requirements to improve document management and provision and sharing of the patient record, such as the Welsh Care Records Service Progress national adoption of a Welsh Community Clinical Information System (WCCIS) that bridges the information gap between Health and Social Care for Health Boards and Local Authorities Across Wales, plans for the development of future safe delivery of services to patients continue and information and technology developed and delivered in partnership is key to any service restructures. We will continue to work with Welsh Government, NHS organisations and all other partners to ensure that their requirements, in the light of changing services, are met and aligned with national informatics policy. 7
8 Associate Medical Director for Primary Care The successful candidate will be supported by the Medical Director/Chief Clinical Information Officer Directorate with expertise in both Information Governance and Safety and Design. The structure will be reviewed with the new appointment and can be augmented with other key roles to reflect the strengths and competencies of the successful candidate. Health and Social Care Informatics Team The Health and Social Care Informatics Team designs and creates requirements that culminate in products that can enhance patient safety. Using their experience and expertise the team ensure Informatics Service products and services support the delivery of safe care. The Informaticists and Clinical Design Leads engage with colleagues in NHS Wales, Social Care, Third Sector, Professional Groups, and the public to design products that; Do no harm Promote safe clinical process Contribute to better clinical and patient outcomes. Functions of the Health and Social Care Informatics Team Clinical risk management/patient safety The Patient Safety Team supports and challenges each product and service in the Portfolio to ensure that it promotes safety. Any risk associated with design, implementation use or change is well managed according to agreed process and standards. In addition the team supports the organisation in managing any incidents that occur, collaborating with care providers, regulators, suppliers and Welsh Government. As the use of Informatics increases in the health and social care arena the need to manage risk becomes greater. The team works across all directorates to raise awareness and improve competency with regard to clinical risk management and contribute to the aims of our learning organisation. Information requirements Due to the shift from traditional process measures to more meaningful outcome measures the team focus on Quality Metrics. Supporting a diverse range of policy makers, planners and senior executives we are involved and leading on a number of projects, some of which have already delivered and many of which are conceptual. Knowledge management Designing informatics solutions on evidence is shown to be best practice. The knowledge manager provides support from literature reviews, evidence searches and advice on strategy and policy. The knowledge manager exploits an extensive network of people and organisations to keep abreast of new care and service models. This has proved invaluable in 8
9 horizon scanning in terms of international innovation and keeping the whole organisation up to date with a monthly Health Informatics digest. Supporting implementation and system deployment The team provides support and guidance to project and services in realising benefit through clinical and business process change management. We support the design of implementation and work with health providers and clinical colleagues to get the most out of the informatics solutions. Clinical design/leadership Clinical Design leads are all still working in clinical posts in the Health Service. They support the technical teams in interpreting requirements from our Clinical Reference Groups and workshops into a solution that provides real benefit to the user and patient. Clinical systems assurance The team on behalf of NHS Wales Informatics Service control the assurance process for the National Architecture Design Board which includes three stages from Strategic Fit to Authority to implementation. The assurance includes Information Governance assurance, Patient Safety and Technical. Clinical engagement A major part of our work is keeping in touch with the realities of delivering care. The team have regular meetings with professional and regulatory groups, local teams, clinical networks and senior clinical colleagues. This information is used to provide leadership and support to the organisation s projects, programmes and other departments to ensure what we deliver meets clinical and service need. 9
10 Information Governance Team Functions of the Information Governance Team The Information Governance team supports both the strategic and operational compliance with Information legislation such as Data Protection Act and Freedom of Information Act. The team interprets the legislation into requirements on the collection, processing and sharing of personal information in a health and social care setting. It also manages the Welsh Government sponsored Wales Accord for Sharing Personal Information (WASPI) Programme, which assists in the sharing of information across all public sectors. Information Governance Strategy for NHS Wales Informatics The team performs privacy impact assessments for all of the NHS Wales Informatics Service projects and programmes. It develops standards that help inform software design, based on good information governance approaches in making new and existing technology more privacy focused. The team also supports the developments in the wider use of NHS Wales patient information for secondary uses. Information Governance Strategy supporting all of NHS Wales The team assists Welsh Government in the development of the NHS Wales IG Standards Model to include Health and Social Care Code of Confidentiality Wales Accord for Sharing Personal Information (WASPI) NHS Wales Records Management Code of Practice In addition to the above the Head of Information Governance and his team supports the strategic direction of the Wales Information Governance Board (WIGB) Operational Support for the NHS Wales Informatics Service The NHS Wales Informatics Service Information Governance team supports the role of the Caldicott Guardian by providing intelligence and professional advice on the uses of patient information. Requests for information from NHS Wales National Databases used for secondary uses require scrutiny to ensure the data outputs remain anonymised. The Information Governance team provides support for that process. Some of these requests for information manifest themselves through the Freedom of Information Act (FOIA). The IG team manages for whole FOIA compliance process. Operational Support for NHS Wales The Information Governance team provides a forum for NHS Wales Health Boards/Trusts Heads of Information Governance to agree and communicate risks and issues collectively. That forum is called the Welsh Health Information Governance Network. The Informatics Service Head of Information Governance attends meetings with stakeholders from primary and secondary care to broker discussions on the strategic use of the electronic patient record with privacy in mind. The team also provides an advice service for colleagues in the Shared 10
11 Services Partnership and for those General Practitioners not already supported by the Health Boards/Trusts. Wales Accord for Sharing Personal Information (WASPI) The WASPI team, managed by the Head of Information Governance, provides support to public services within Wales on robust information sharing practice. Its role is to embed the WASPI principles within public services. 12 The Role The Associate Medical Director for Primary Care supports the Chief Clinical Information Officer with a defined responsibility for all matter relating to Primary Care Informatics. We are looking to appoint a credible clinician who can provide clinical challenge to design assumptions, inform technical and project approach, support external Senior Responsible Officers (SROs) and hold the overall design of the combined programmes and services for Primary Care. Working with the General Practice Council Chair to ensure all products are subjected to appropriate scrutiny, assessment and assurance prior to release and that all risks are managed appropriately. Influencing NHS Wales at strategic and operational levels to ensure Patient Safety and Clinical Governance are part of everyone s core business. This is a role that is responsible for Information Governance for Primary Care and promotes safe use of information through ICT Health and Social Care Solutions. Designing for robust Information Governance standards is essential in order to meet the privacy expectations of professionals and the public. The extent of the Information responsibility extends across information used for direct patient care and onward strategic use of information nationally. National use of patient derived information requires a sound knowledge base on the impact of secondary use strategies for national statistics, commissioning and clinical coding standards. Supported by the Head of Information Governance the role extends to providing the Wales Information Governance Board with expert advice and guidance on items and issues presented with a focus on Primary Care. 12

Improving quality and outcomes Accountable to the CCIO for ensuring the Patient Safety Assurance process is evidence based, legally compliant and adhered to for National Informatics Solutions whether developed procured or managed for Primary Care. Influencing NHS Wales at strategic and operational levels to ensure Patient Safety and Clinical Governance are part of everyone s core business within Primary Care. To take a key leadership role in the delivery of patient information priorities as set out in the Informed Health and Care: A Digital Health and Social Care Strategy for Wales with particular focus on the development of an online citizen portal to enable safe patient access to records held within NHS Information systems. Professional responsibility for the Incident Management Process in relation to Primary Care To provide strategic advice on innovation in information science and clinical informatics with a focus on quality and value based on the Principles of Prudent Healthcare for Primary Care services. To support the improvement of outcomes for patients and drive up the quality of care by providing clear clinical leadership in the use of information, including information governance. To ensure collaboration between leaders in informatics within Primary Care clinical groups and provide strategic professional input across the NHS Wales, in particular the: Clinical design and implementation of the e-health strategy. Subsequent commissioning of safe and effective IT delivery programmes. Support the development of Clinical informatics capability and provide facilitation for national clinical informatics networks.
