8 Location 3: The Health Centre is a multi-purpose town centre facility which is fully compliant with current regulations on accessibility The Dental Department is situated on the 2 nd floor and can be accessed by lift and stairs. It comprises 7 surgeries, (one of which contains a tilting platform for patients who use a wheelchair) and recovery room, a 2 roomed decontamination suite (compliant with HTMO105), x ray room, office and storage accommodation, reception and waiting area. The service provides dental care for adults and children with special care needs, minor oral surgery and intravenous and inhalation sedation for anxious patients on referral. The department also provides a base for epidemiology and the Oral Health Promotion services. There is a GP Extended Hours Service that provides none urgent/routine appointments for patients outside normal practice hours, between 6-8pm midweek and sat 8am to 8pm. Patients access via referral from their GP practice. The service has been set-up in order to improve access to primary care provision, at times that are more accessible to patients. The Warrington Borough provides its GP Out of Hours from this location. A range of community health services are also provided including District Nursing treatment room and Single Point of Access. Healthcare and dental services are provided on site from dedicated facilities. 11 A Walk-In Centre, based within the new Leigh Health Centre adjacent to Leigh Infirmary, offering healthcare advice and treatment without having to make an appointment. The Walk In Centre is open seven days a week from 07.00am in the morning to 9.00pm at night and provides treatment for minor injuries and illnesses including wounds, eye wounds, skin complaints, advice on coughs and colds, ear, nose and throat (minor infections) and emergency contraception. Dental service - Leigh Health Centre is a purpose built Health Centre on 2 storeys. There is a purpose designed dental suite on the second floor. There is a large lift and good wheelchair access. There is a large waiting area. Inhalation sedation is provided. Location 9: Bridgewater CHCFT Lower Ince Health Centre Claire House, Lower Ince Health Centre Phoenix Way, Ince Wigan Lancashire WN3 4NW A purpose built, 2 storey, LIFT building, opened in May 2005, in the Ince area of Wigan. The premises accommodate 2 GP practices as well as providing a range of community health services, including an Out of Hours Service. Routine dentistry treatment for residents of Greater Manchester West Mental Health trust, including patients with special needs. Bridgewater CHCFT St Helens NHS Walk In Centre Millennium Building Corporation Street St Helens Merseyside WA10 1HJ Walk in centre provided in St Helens town centre close to local amenities. The WIC is shared occupancy with The Millennium Centre who owns the building. 13 The Widnes NHS Urgent Care centre service is delivered from the Widnes Health Care Resource centre which is a purpose built health care facility constructed under the NHS LIFT initiative. The building was designed and constructed to offer flexibility for service provision and is fully compliant under the Equality Act The service is open 7 days a week, 7am pm, and provides care for patients who attend with undiagnosed conditions. The service has access to x-ray and other diagnostic facilities. The dental department has three surgeries on the first floor, which has excellent wheelchair access. 19 Location 6: St Catherine s Secure Centre is a 12 bedded secure unit providing residential care to vulnerable children (age 10-17yrs) who have been placed in care under welfare orders. The Centre is situated in St Helen s and is part of the charitable organisation Nugent Care. The Centre s secure site is Marydale Lodge and this consists of 2 residential units Austen which has 6 places exclusively for girls and Bronte with 6 places for a mix of girls and boys. Greater Manchester West NHS Foundation Trust will be providing the Mental Health and Substance Misuse services in partnership with Bridgewater Community Healthcare NHS Foundation Trust. 28 Location 2: Chat Moss Eccles Manchester M30 7RL Barton Moss Secure Care Centre is a twenty four bedded unit within the Secure Estate. The Centre consists of 4 residential houses; Beech, Rowan, Maple and Willow, with each house having the capacity to accommodate 6 young people. Rowan and Beech House cater for admissions whilst Maple and Willow both operate as longer term units. The principle aim of the healthcare team is to provide high quality primary care services that are equivalent to that available to patients in the community, within the constraints of a secure setting. 29 Location 3: The Health Centre is a multi-purpose town centre facility which is fully compliant with current regulations on accessibility The Dental Department is situated on the 2 nd floor and can be accessed by lift and stairs. It comprises 7 surgeries, (one of which contains a tilting platform for patients who use a wheelchair) and recovery room, a 2 roomed decontamination suite (compliant with HTMO105), x ray room, office and storage accommodation, reception and waiting area. The service provides dental care for adults and children with special care needs, minor oral surgery and intravenous and inhalation sedation for anxious patients on referral. The department also provides a base for epidemiology and the Oral Health Promotion services. A District Nurse treatment clinic is also held at this location. There is a GP service that provides none urgent/routine appointments for patients outside normal practice hours, between 6-8pm midweek and sat 8am to 8pm. Patients access via referral from their GP practice. The service has been set-up in order to improve access to primary care provision, at times that are more accessible to patients. The Warrington Borough provides its GP Out of Hours from this location. A range of community health services are also provided including District Nursing treatment room and Single Point of Access. 32 The Avenue Leigh Lancashire WN7 1HR A Walk-In Centre, based within the new Leigh Health Centre adjacent to Leigh Infirmary, offering healthcare advice and treatment without having to make an appointment. The Walk In Centre is open seven days a week from 07.00am in the morning to 9.00pm at night and provides treatment for minor injuries and illnesses including wounds, eye wounds, skin complaints, advice on coughs and colds, ear, nose and throat (minor infections) and emergency contraception. Leigh Health Centre is a purpose built Health Centre on 2 storeys. There is a purpose designed dental suite on the second floor. There is a large lift and good wheelchair access. There is a large waiting area. Inhalation sedation is provided. Location 10: Bridgewater CHCFT Lower Ince Health Centre Claire House, Lower Ince Health Centre Phoenix Way, Ince Wigan Lancashire WN3 4NW A purpose built, 2 storey, LIFT building, opened in May 2005, in the Ince area of Wigan. The premises accommodate 2 GP practices as well as providing a range of community health services, including an Out of Hours Service. 32 P a g e

33 Location 11: Bridgewater CHCFT Padgate House Vulcan Close Padgate Warrington WA2 0HL Padgate House is a care home owned by Warrington Borough Council which includes an intermediate care facility (step up and step down beds). This is an integrated service between Warrington Borough Council and Bridgewater Community Healthcare NHS Trust. Location 12: Bridgewater CHCFT Prestwich Dental Clinic Bury New Road Prestwich Manchester Greater Manchester Dental service to be provided from the main Prestwich hospital site utilising their facilities. Dental department comprises of one surgery, waiting area and an office. Routine dentistry treatment for residents of Greater Manchester West Mental Health trust, including patients with special needs. Location 13: Bridgewater CHCFT St Catherine s Secure Centre Blackbrook Road St Helens Merseyside 33 P a g e

34 Location 14: WA11 9RJ St Catherine s Secure Centre is a 12 bedded secure unit providing residential care to vulnerable children (age 10-17yrs) who have been placed in care under welfare orders. The Centre is situated in St Helen s and is part of the charitable organisation Nugent Care. The Centre s secure site is Marydale Lodge and this consists of 2 residential units Austen which has 6 places exclusively for girls and Bronte with 6 places for a mix of girls and boys. Greater Manchester West NHS Foundation Trust will be providing the Mental Health and Substance Misuse services in partnership with Bridgewater Community Healthcare NHS Foundation Trust. Bridgewater CHCFT St Helens NHS Walk In Centre Millennium Building Corporation Street St Helens Merseyside WA10 1HJ Walk in centre provided in St Helens town centre close to local amenities. The WIC is shared occupancy with The Millennium Centre who owns the building. 35 Cheshire WA8 7GD The Widnes NHS Urgent Care centre service is delivered from the Widnes Health Care Resource centre which is a purpose built health care facility constructed under the NHS LIFT initiative. The building was designed and constructed to offer flexibility for service provision and is fully compliant under the Equality Act The service is open 7 days a week, 7am pm, and provides care for patients who attend with undiagnosed conditions. The service has access to x-ray and other diagnostic facilities. The dental department has three surgeries on the first floor, which has excellent wheelchair access. A full range of dental services are provided from this facility including the provision of sedation services.
