1 Presentation to support the: Immediate actions following an adult in-patient fall flow-chart: familiarisation with flat-lifting and use of Hoverjack Vulnerable Adults Risk Management Group June 2014

2 Aims of session Familiarisation with: Immediate actions following an adult inpatient fall guidance and flowchart flat-lifting equipment (Hoverjack and scoop) triple immobilising log-rolling Understanding responsibilities and working within competencies

4 Immediate actions Correct actions following a fall will: promote safe and quality care reduce harm from a fall screen for injury and escalate comply with National drivers Translating evidence into practice

5 How to Immediate actions flowchart gives: the assessment and screening process how to escalate for assistance to triple immobilise and flat-lift information eg neurological observations

6 Immediate actions following an adult in-patient fall illustration of the flowchart on each ward wall

7 Where to find information Flowchart - laminated A3 poster on ward wall Written supporting guidance on intranet and hard copy on ward E-learning programme available on LED intranet site

8 Assess the situation Keep patient still

9 Ask the screening questions on flowchart: Escalate if concerns

10 Escalate For medical assistance fast-bleep or UHW/L call site practitioners For equipment (Hoverjack and scoop via porters) Collar/head blocks/tape local CB arrangements Know what is where and who!

11 Hoverjack and scoop Obtained via the porters Check Hoverjack before use in case of damage and that valves are not leaking Do not place on sharp objects Move using handles - not straps Do not let patient roll off H-Jack

12 Obtain collar, head blocks and tape: responsibility of each Clinical Board

13 Care of patient with actual/possible spinal injury Only competent staff trained in triple immobilising to: hold head in neutral position measure and attach collar tape forehead and chin apply head blocks and lead log-rolling

14 Familiarisation with measuring a collar

15 Measure collar

16 Attach collar

17 Place scoop alongside patient to measure length of scoop required

18 Click out to lengthen scoop as required

19 Push both buttons out to split scoop into 2 halves

20 Log-rolling The competent lead person supports head and neck at all times Ward staff team in height order 1 manages scoop Lead gives the orders to roll

21 Competent lead holds head and shoulders Ward team organised in height order

22 One method of log-rolling by crossing hands Lead decides method

23 Change sides to log-roll onto other side. When patient on back, click scoop buttons in well top and bottom

24 Triple immobilising Lead holds head at all times until patient is immobilised onto scoop Triple immobilising = collar, tape on chin and forehead attached to the scoop with head blocks Strap onto scoop Competent staff undertake procedure

25 In scoop with head and shoulders held prior to tape and blocks in place

26 Tape on chin and forehead with rolled towels. Ferno head blocks are preferred means to immobilise

27 Hoverjack placed with valves at patient's feet

28 Ensure all 4 red deflation caps are screwed in firmly

29 Patslide in place for move onto Hoverjack

30 High to low kneeling manual handling technique to transfer #1

31 High to low kneeling manual handling technique to transfer #2

32 Attach Hoverjack straps loosely round patient

33 Air supply into valve 1 and hold in place

34 Inflate other valves in correct numerical order

35 Inflate to level required and ensure all white inflation caps are screwed in firmly

36 Bring trolley/bed near to inflated Hoverjack with Patslide

37 Patslide and glidesheet to transfer onto trolley

38 When on trolley log-roll off scoop. Continue immobilisation process with patient on trolley

39 Flat lifting suspected lower limb fracture, including hip Keep limb immobilised and comfortable Escalate for medical assistance Request Hoverjack only (scoop not necessary) Use Patslide and glidesheet to transfer

40 Hoverjack post-use Deflate Hoverjack release air from all 4 red deflation valves Clean when inflated (following instructions attached to the trolley); ensure Hoverjack is dry and then fold Replace Hoverjack with air supply into trolley and return via porters

41 Fold up Hoverjack carefully after cleaning as per instructions

42 Pack Hoverjack onto trolley

43 Pack air supply onto trolley and return via porters
