3 Team-based health care is the provision of health services to individuals, families, and/or their communities by at least two health providers who work collaboratively with patients and their caregivers to the extent preferred by each patient to accomplish shared goals within and across settings to achieve coordinated, highquality care. 9 THE BEGINNING Initially 1 MD & 1 NP in 1995 Culture was started patient-focused; on-call & hospital responsibilities for NP expanded; 3 MDs & 3 NPs (1 male); worked in teams of expanded; 5 MDs & 3 NPs Interviewing included questions on team concepts, interactions with NPs & PAs; change concepts

Lori is their patient and their true north. She has a face and a name and is a real person. On each floor, on each board there is a picture of Lori.
