3 HSHS St. Elizabeth s Hospital Comprehensive Inpatient Rehabilitation Unit HSHS St. Elizabeth s Hospital s Comprehensive Physical Rehabilitation Unit has been helping patients with almost every type of disability since The Rehab program, accredited by CARF as a Comprehensive Integrated Inpatient Rehabilitation Program, offers a complete range of services. The 33-bed unit is one of the few CARF approved acute rehab programs in Southern Illinois and the only one in St. Clair County. More than 430 patients were helped last year to regain their highest level of function. The rehabilitation team cares for patients who need rehab due to major injury or illness. The unit s physiatrist, therapists, nursing staff, and social workers have helped patients either return to their prior lifestyle or live as independently as possible. Admission Criteria Patients, 18 years of age and older who are medically stable, non-ventilator dependent, and can do at least three hours of therapy throughout each day may be admitted in our Rehab program. Patients are evaluated and approved for admission by a physiatrist. Our Rehabilitation Referral Coordinator or Charge Nurse will assist with the admission to the unit. Patients admitted to Rehab may have (but are not restricted to) the following diagnoses: Brain Injury Spinal Cord Injury Stroke Amputation Neurological Disorder Hip Fracture Other Orthopedic Conditions Multiple Trauma Congenital Deformities
4 Rehab and Your Family After you are admitted to the Rehab program, the team members working with you will assess your function and develop a plan of care with your input. You and your family are important members of the team. We encourage a family member or close friend to be with you on the day of admission to learn more about the program and to help answer questions. One of the main goals of Rehab is to return you to your prior living arrangement. Therefore, family members are a crucial part of the process and are encouraged to attend therapy as scheduled by the rehab team. The therapists and nursing staff will teach your family and/or friends how to assist with activities of daily living and self-care. Visitors Social visits are important but should not disturb the therapy schedule. Therapy is held seven days a week with shorter sessions on Saturdays and Sundays. Only family members or friends serving as the patient s care or support person are allowed to attend scheduled therapy sessions. No children are permitted to attend therapy sessions. Your well-being is our primary concern and is the reason for limiting the number of visitors and the length of their visit. Please inform your nurse if you expect a large number of guests for a special visit. Children under the age of 4 are discouraged from visiting in patient care areas. Children under 12 are discouraged from visiting a patient in isolation. Visiting children must be accompanied by an adult at all times. Suggested Visiting Hours: 12:00-1:00 p.m. and after 4:30 p.m. Monday-Friday. After 12:00 p.m. on Saturday and Sunday
5 Your Rehab Team When you are admitted to our program, you will be treated by a multi-disciplinary team of professionals, led by a physiatrist, to assess your impairment with you and your family. Together, we will develop a care plan based on your needs and capabilities. Because we believe in treating the whole person - not just the impairment - your caregivers may include professionals from a wide range of disciplines, including: REHAB PHYSICIANS (Physiatrist) - Coordinates your Rehab plan - Participates in team conferences - Manages medical issues with consultation, if needed NURSING STAFF - Provides 24 hour nursing care - Provides education for the patient and their families - Encourages patients to work toward the goal of self management - Reinforces therapy recommendations from all disciplines 24 hours a day SOCIAL WORKER - Works with physiatrist and nursing to aid admission - Coordinates discharge services - Acts as a contact between patient/rehab team/ insurance/community resources - Participates in team conferences PSYCHOLOGICAL/COUNSELOR SERVICES - Available for assessment and support PHYSICAL THERAPY (PT) - Provides assistance and instruction with mobility - Provides instruction in safe transfer techniques - Provides training in balance activities - Provides proper technique for walking and stair climbing - Provides appropriate exercise programs for strength, flexibility, and coordination OCCUPATIONAL THERAPY (OT) - Provides training for activities of daily living (ADLs): eating, bathing, dressing, and homemaking - Provides activities that deal with high level thinking skills - Provides appropriate wheelchair seating/ positioning devices - Provides instruction in toilet, tub/shower, furniture and car transfers - Provides upper extremity exercises for strength and coordination SPEECH-LANGUAGE PATHOLOGY - Provides treatment for patients with difficulty speaking - Provides activities to improve memory, problem solving and high level thinking skills - Provides treatment for patients with feeding/ swallowing problems THERAPEUTIC RECREATION - Offers adaptations for leisure interests - Gives community resources
6 A fully accredited program with expanded facilities To reach your highest level of functioning after a major health crisis, it s important to get your rehabilitative care from a source you can trust and at a location that is convenient. Both are important factors in maintaining a commitment to rehabilitation. St. Elizabeth's 33-bed inpatient rehabilitation program is accredited by CARF, the Commission on Accreditation of Rehabilitation Facilities, to function as a Comprehensive Integrated Inpatient Rehabilitation Program. It is a CARFaccredited program - AND THE ONLY ONE OF ITS KIND CURRENTLY AVAILABLE IN ST. CLAIR COUNTY. Our rehabilitation program is designed with our patients needs in mind. Virtually every service you might need including physical, occupational, speech and recreational therapy is located directly within the unit itself. Once you are discharged from the inpatient program, we offer a variety of therapies on an outpatient basis. HAPPY PATIENTS SAY: The therapists really pushed me and that s why I m doing so well. Many thanks and you were great. What to bring Personal clothing is worn since the greater part of your day will be spent out of bed and out of your room. Dress is casual. Clothing should be machine washable. All clothing and belongings should be labeled to prevent loss. Patients SHOULD bring the following: Three sets of casual, easy-to-clean clothing that are suited for exercise. Underwear, T-shirts, short-sleeved shirts or blouses, and comfortable slacks with elastic waistbands. Sweatpants are recommended. Seasonal outerwear for leisure activities. Shoes with flat heels - A sturdy oxford lace-type or tennis shoe is ideal. New shoes should not be bought before asking the therapist. Socks, like those worn at home. Button-front sweater, if desired. Toothbrush, toothpaste, comb, brush, shampoo, lotions, and battery operated razor. A list of medicines. Dentures, glasses, hearing aid, braces, splints. Personal items like photos. A copy of insurance card, Medicare and Medicaid information. A copy of your Durable Power of Attorney for Health Care document. We recommend you do not bring jewelry or other valuables to the Rehab Unit. If you do, they may be kept in Hospital Security upon admission. Please do not keep more than $5.00 in your room at any time. You are asked to let your nurse know if you have hearing aids, dentures, glasses, contact lenses, etc., so they may be listed on your chart. FINANCIAL INFORMATION St. Elizabeth s Hospital accepts Medicare and Medicaid. In addition, we are contracted with and included in the networks of several commercial payers (list available upon request).
7 Amenities and Special Touches At Your Request Patient Meals. Room Service Dining is offered for all patients. This program allows you to select meals from a restaurant-style menu designed to make you feel as though you are at home or at your favorite restaurant. Menus are located in your room. Once you ve made your selections, dial ext to have your meal delivered to your room within 45 minutes. Room service for visitors. Guest trays are also available for a nominal fee for visitors who wish to remain with the patient for meals. Cashonly payment will be collected by the Room Service Attendant when delivered. Dial ext to order. Cafeteria and Starbucks Our Cafeteria is located on the 1st Floor. Hours are 6-10a.m. for Breakfast, 10:30-1:30p.m. for Lunch, 4-6p.m for Dinner, and our Grill, Grab & Go is available until 6:30 p.m. Starbucks Coffee & Cafe is located in our Main Lobby, Monday-Friday 7:30 a.m.-7:00p.m. Walgreens Bedside Delivery Service. Medications and a limited number of over-the-counter supplies for patients can be ordered through Walgreens and delivered to your bedside during stay and before discharge. This added convenience can eliminate a stop on your way home. This service is optional and may not be compatible with all insurance plans. Daily Care Calls. We will call the designated spokesperson daily with an update. Once you re home, questions often arise. You can call us 24/7 at , ext Your call will be answered as soon as possible. After discharge, you will also receive follow-up phone calls within your first week and approximately 90 days later to check on your progress. Pastoral Care. Our Pastoral Care department is available to provide spiritual support for the family. Other amenities include: Wi-Fi access
8 Stroke Symptoms of Stroke Some people don t understand they re having symptoms of a stroke. Some may experience subtle symptoms, while others hope the symptoms will disappear with time. If people seek treatment within three hours of the first onset of symptoms, physicians have more options to treat strokes. The vast majority of strokes are caused by clots that block blood flow to the brain. For patients who meet treatment criteria, today s options include aggressive medical, surgical and radiological responses, including clot-busting drugs that can dissolve a clot before any lasting damage occurs. The key is time: the earlier you get to the emergency department, the more treatment options are available and the greater the likelihood that you will not suffer lasting effects of the stroke. St. Elizabeth s Stroke Telemedicine St. Elizabeth s partners with C3O Telemedicine. Patients with on-set stroke-like symptoms within 24 hours now have the opportunity to have their test results shared with a telestroke neurologist, no matter what time of day the incident occurs. That s 24/7 access to specialists at the bedside. STROKE is an EMERGENCY Act F.A.S.T. Face Arms Speech Time Ask the person to smile. Does one side of the face droop? Is one arm weak or numb? Ask the person to raise both arms. Does one arm drift downward? Ask the person to repeat a sentence. Is speech slurred? Is the sentence repeated correctly? Stroke is the fifth leading cause of death and the number one cause of long-term disability world-wide. Approximately 700,000 Americans will have a stroke this year; that s someone every 45 seconds. If the person shows any of these symptoms, call immediately. Types of Stroke Ischemic stroke accounts for about 83 percent of all cases. Ischemic strokes occur as a result of an obstruction within a blood vessel supplying blood to the brain. Hemorrhagic stroke accounts for about 17 percent of stroke cases. It results from a weakened blood vessel that ruptures and bleeds into the surrounding brain. Transient ischemic attacks, or TIAs, are minor or warning strokes. In a TIA, conditions indicative of an ischemic stroke are present and the typical stroke warning signs develop. However, the obstruction (blood clot) occurs for a short time and tends to resolve itself through normal mechanisms. Even though the symptoms disappear after a short time, TIAs are strong indicators of a possible major stroke. Steps should be taken immediately to prevent a stroke. STROKE SUPPORT GROUP Fostering a better understanding of stroke recovery, rehabilitation and prevention, one family at a time. The Stroke Support Group meets the 1st Wednesday of every other month at 2:00 p.m. in the Millennium Room of St. Elizabeth s Hospital. 10 Combining exceptional care with the comfort of home. HSHS Home Care Southern Illinois offers skilled health care to patients at home. The focus of home care is on the health needs of the individual in a family setting. Activities are directed toward helping patients to attain and maintain their highest level of independent living, and to help the family provide support. Efforts are also concentrated on the prevention of further disability, health teaching, and on appropriate referral to allied health professionals as needed. HSHS St. Anthony s Memorial Hospital, of the Hospital Sisters of the Third Order of St. Francis, doing business as HSHS Home Care Southern Illinois, is licensed by the State of Illinois, is Medicare-certified and accredited by The Joint Commission. Admission Criteria Your physician is responsible for recommending home care and for establishing a treatment plan. This plan is incorporated into the patient s record that is maintained by the Home Care staff. The physician must certify and acknowledge the following: Home Care services are necessary. A plan for treatment has been established. The patient is in need of the various services as outlined under the treatment plan. He/she will not falsify or conceal essential information. The physician will review the treatment plan periodically and re-certify that there is a continuing need for service. Planning Patient Care After the patient s referral has been accepted, the Home Care staff will assist the physician in planning for the patient s home care. An evaluation is made to determine the patient s needs for nursing services, physical therapy, speech therapy, occupational therapy, medical social services and home health aides. The need for special supplies and equipment is also determined. Family members are given an orientation and trained in their responsibilities in providing care for the patient. Physical, Occupational, and Speech Therapy Therapists provide prescribed exercises to develop body strength and coordination, improve mobility, as well as mastery of everyday skills to allow you to be as independent as possible in the home setting.

Virtually every service you might need including physical, occupational and speech therapy plus audiology is available at convenient locations throughout the metro east. These services are provided for a variety of patients - from pediatrics to geriatrics. Our dedicated therapists work one-on-one with patients, helping them to overcome limitations and achieve their full recovery potential.

Physical Therapy Physical Therapy focuses primarily, but not solely, on relieving pain, promoting healing, restoring function and movement, facilitating and adapting to the injury. Physical therapy also addresses ergonomics, fitness and wellness. St. Elizabeth s physical therapists main goal is to help you get back your quality of life and return to the activities you love to perform. We help patients improve motion and mobility and are trained in a variety of sub-specialties. This allows us to create individualized programs. We offer the following services: Fall risk reduction programs Pain control interventions Balance programs Lymphedema management Adult and pediatric urinary incontinence therapy Pelvic pain therapy Work injury therapy Aquatics program Cancer rehabilitation Why Physical Therapy? Restoring proper movement is essential to living a healthy life. Movement is key to prevention of obesity, which is linked to as many as 18% of adult deaths in the United States. Recent studies suggest prolonged sitting is linked to heart attack and stroke, and that the ability to walk more each day can help reduce that risk. The ability to move is necessary for maintaining a healthy system of balance, which prevents falls leading to significant and often costly injuries. Who can benefit from Physical Therapy? Physical therapists generally treat all ages and abilities to maintain, restore and expand motion capabilities. Current research suggests that Physical Therapy can often be used as a substitute for surgeries or prescription medication. This less costly alternative can help many conditions, from back pain and certain types of arthritis, to degenerative disc disease and injuries of the meniscus. Treatment plans are generally designed collaboratively with the patient, to address his or her specific goals and challenges. While highly effective, this form of treatment is rarely passive on the part of the patient. In fact, participation in your own recovery is an empowering experience. Patients develop a close, ongoing relationship with their Physical therapist while working to maintain their health and movement. Aquatics Therapy Aquatic therapy is available for patients according to their individualized plan. This treatment may be beneficial to patients that are not able to tolerate therapy on land. Therapists work with patients to develop a therapy exercise program to meet their goals. It is designed to address a wide variety of orthopedic, spinal and neurological deficits for
14 clients of any age. Occupational Therapy Occupational therapists help patients of all ages learn skills to lead productive, independent and satisfying lives. Therapy sessions help patients rebuild strength and motor skills, and re-learn activities of daily living such as feeding, bathing, grooming and cooking. Occupational therapy is not for adults only. A specialized program helps infants and children achieve age appropriate self-help, play and leisure skills. Using these activities, therapists strive to minimize the effects of disease, injury, congenital deficiencies or other disabilities. We offer the following services: Hand therapy Work modification educations Cognition and visual perception training Strengthening, coordination and joint protection Wheelchair evaluation Occupational Therapy benefits anyone who has a condition or injury, which limits his or her ability to participate in activities of everyday life. That includes those who have experienced health issues such as stroke, fractures, head injury, joint replacement, upper extremity sprain/strain or other debilitating conditions that impair function. and use problem-solving skills. St. Elizabeth s speech therapists specialize in pediatrics to include fluency, feeding therapy, articulation and language disorders. Also specializing in cognitive therapy, vocal cord dysfunction, aphesia therapy, augmentative communication devices, and full swallowing therapy options including NMES/VitalStim and modified barium swallow studies. Services include evaluation and treatment of: Brain injuries Stroke Alzheimer s or Parkinson s Disease Speech Fluency (Dysarthria, Apraxia Visual-Perceptual Deficits) Swallowing Dysfunctions Voice Disorders Laryngectomy Cognitive Linguistic Deficits Audiology Sometimes rehabilitation requires an evaluation from an audiologist to determine whether a patient has suffered hearing loss. St. Elizabeth s also meets the audiology needs of children through the Illinois Early Intervention Program and the Division of Specialized Care for Children. Speech Therapy Speech-Language Pathologists treat patients who have problems with speech, language, hearing and swallowing.
