1 COMMISSION ON PUBLIC HEALTH MINUTES OF MEETING 10/19/14 12:00 AM 3:00 PM NYSAFP Office Albany, NY Members Present: Drs. Heather Paladine (Chair), William Klepack (Vice-Chair), Margarita DeFederecis, Scott Hartman, Raymond Harvey, Mark Kratowski, Linda Prine, Wayne Strouse, Ron Rouse (staff), Yorgos Strangas, (resident) Emily Holt (resident) Agenda Item Discussion Action Introductions All members introduced themselves. No further action. Behavioral Health (mental health, alcohol, substance abuse) Discussed the importance of family doctors providing BH screening and counseling services to their patients. These services help improve the health of our patients, but increasing the number of doctors providing BH services will enhance the reputation of NYSAFP. We need to obtain information from our Members on which obstacles they face in providing BH services, and which ones are interested in doing more with BH.. Discussed edits to the draft survey. Other items discussed: - benefits of co-locating counselors in doctors offices. We could work with county mental health centers, but some may be under-staffed. - We need more counselors - reimbursement issues Education on methadone prescriptions and potential harms - Separate Model #2 into 2 models one for Colocation of counselors and another one for referral. (Rouse) - Edit Member Survey as discussed; speak with Vito on how best to disseminate it among all Academy Members. (Rouse) Article for Family Doctor (Harvey) 1

2 Single Payer We need to convince MSSNY to support Single Payer, but at their recent COD they voted it down; however, the vote was closer than in the past and at least it is being debated. Discussed how we can motivate our members to be more involved with MSSNY SP debates; also, need to send our student and resident delegates to MSSY meetings. AAFP is starting member various interest groups, including one on SP. It will have its first meeting at the Scientific Assembly. Chelsea Maguire is active. The Affordable Care Act will accept State waivers in 2017 to establish state-operated SP systems. We should use this waiver timeline as a reason to promote SP. Make sure NYSAFP members who are also MSSNY members are vocal about SP. Write a how-to blurb for our Members on getting involved with MSSNY. (Strouse) Explore whether we can we have a joint AFP/MSSNY membership mechanism. (Rouse) Prepare materials for our members who are part of the AAFP SP Interest group. (Rouse) Disseminate information about how to join the AAFP s interest group (Prine, Paladine) Verify when Wayne s article on New Zealand s SP system will be published by NYSAFP (Strouse) Continue to write articles, letters, op ed pieces that promote SP. (Rouse) List of Preventive Services We are approaching the point where all Americans will be covered for comprehensive prevention services, and many of these services must be provided at no cost to the patients. The ACA is playing an important role in reaching this milestone as are Medicare and Medicaid. Doctors and their patients need information on this comprehensive list of prevention services. Many insurance companies and self-insured plans are required to provide preventive services at no cost, but some are circumventing this provision. Reviewed the comprehensive list of preventive services required by the Check to see if NYS has a consumer hot line for complaints about insurance companies and selfinsured corporations not providing coverage for preventive services. If so, publicize it among our Members and their patients. (Rouse) Disseminate the preventive services list among 2

3 End of Life Care Hydrofracking Update AAFP & Coca Cola Women s Health Tobacco Cessation Grant ACA. We need to add HPV vaccine for boys and men age Many different forms are available for doctors and patients, which is confusing to them. Bill Klepack wrote a NYSAFP Journal article that was published. Other resources: NY state Attorney General Bill gave a brief update on developments. Data continues to emerge on various concerns but no changes in the NYSAFP policy are merited at this time. Discussed the problems with AAFP accepting funds from Coca-Cola. Although the funds do help offset AAFP costs, its acceptance sends contradictory and even hypocritical messages. Orlando Sola, of our Commission, co-wrote an article about this issue. Some insurance companies are denying payment to family doctors for performing various women s health referrals (IUD, implant, colposcopy, prenatal care, etc.) because either they are not an Ob/Gyn or not the patient s PCP. Pfizer has announced an RFP related to tobacco cessation, and a Letter of Intent is due October 23. Heather has circulated the RFP and taken the lead in developing a program outline and the Letter. We discussed the broad ideas for an application; we will focus on residency programs and physician offices. If Pfizer is interested in our LoI, we will be invited to submit a full application. Funding would be awarded next our Members and create a version for patients. (Rouse) Develop a summary of all the resources and forms and put them on the NYSAFP web site; include the Journal article. (Paladine, Rouse) Monitor developments and take action as appropriate. (Klepack, Strouse) Develop resolution to stop AAFP acceptance of support from Coca-Cola. (Sola, Strouse, Paladine) Send a letter to NYS Department of Financial Services advocating payment to family doctors for performing these procedures and, if necessary, prepare materials for Lobby Day (Prine) Complete and submit Letter of Intent. 4 spring. Resolutions Discussed potential resolutions for the 2015 COD. Members should begin preparing resolutions now, and circulate them among the PHC Members before the March 2015 meeting. Shortage of physicians who are trained to insert IUD s despite its efficacy. Address barriers to training post-residents. (Prine) Raising the number of patients (100 now) for whom a doctor can prescribe suboxone. (Harvey) Removing methadone from state Medicaid formularies, ie, require prior authorization. (Harvey) Climate change refer to changes in climate that affect health. Make it broad. (Klepack, Strangos) Increase access to Hep C medications (Strangos Nuclear disarmament (Klepack) Improved public transit (Prine) Behavioral Health: Improved reimbursement & CME for behavioral health screening, counseling, & referral; more Residency training; recruit more doctors for suboxone, vivitrol prescribing; development of more effective addiction treatment medications; use law enforcement system to emphasize more treatment and less incarceration; seal arrest records of those completing treatment Begin developing resolutions now.
