1 Emergency Appeal Operation Update Ebola Virus Disease Emergency Appeals (Guinea, Liberia, Sierra Leone and Global Coordination & Preparedness) Combined Monthly Ebola Operations Update N o July 2016 Current epidemiological situation, country-specific information + Post-Ebola systems strengthening Guinea declared an end to Ebola virus transmission on 1 June. On 9 June the World Health Organization (WHO) declared the end of the most recent outbreak of EVD in Liberia. Having contained the last Ebola virus outbreak in March 2016, Sierra Leone has maintained heightened surveillance with testing of all reported deaths and prompt investigation and testing of all suspected cases. The ultimate goal of post-evd recovery plans is to re-establish the conditions for a quick return to a healthy society, with viable livelihoods, psychosocial well-being, economic growth, and overall human development. At the same time, the immediate priority is to end the epidemic, and address the adverse conditions that enabled a localized epidemic to escalate into a national crisis with regional and global ramifications. Over confirmed, probable, and suspected cases were reported in Guinea, Liberia, and Sierra Leone, with deaths since the onset of the Ebola outbreak. The majority of these cases and deaths were reported between August and December 2014, after which case incidence began to decline as a result of the rapid scaleup of treatment, isolation, and safe burial capacity in the three countries. IFRC continues to closely monitor the Ebola response key performance indicators of the phase 3 Ebola response framework. The objectives of these indicators are to interrupt all chains of transmission, prevent new infections, detect timely and respond rapidly. 1 A single combined operations update is produced for the 4 Ebola operations on a monthly basis. Daniel Wesseh, a Moto-taxi driver living in Clara Town, Monrovia noted that life was difficult during Ebola, as people were afraid to ride motorcycles for fear of contracting the disease. This affected his business, as he was not getting passengers to earn money to enable him to feed his family. Ahmed Jallanzo, IFRC IFRC s Ebola Virus Disease (EVD) strategic framework is organised around five outcomes: 1. The epidemic is stopped; 2. National Societies (NS) have better EVD preparedness and stronger long-term capacities; 3. IFRC operations are well coordinated; 4. Safe and Dignified Burials (SDBs) are effectively carried out by all actors; 5. Recovery of community life and livelihoods. Helping stop the epidemic, the EVD operations employ a five pillar approach comprising: (i) Beneficiary Communication and Social Mobilization; (ii) Contact Tracing and Surveillance; (iii) Psychosocial Support; (iv) Case Management; and (v) Safe and Dignified Burials (SDBs) and Disinfection; and the revision has included additional sectors on recovery basically covering food security, livelihoods and Disaster Risk Reduction. Six emergency appeals were launched to respond to and combat EVD outbreaks in Guinea, Liberia, Sierra Leone, Nigeria and Senegal. Those in Guinea, Liberia and Sierra Leone are still active whilst coordination and technical support continues at the regional level. The Ebola emergency appeals have been revised to anticipate a longer-term vision as operations head toward recovery. The revised appeals can be found at and are currently planned to end in December Smaller preparedness and response operations were financed by the IFRC Disaster Response Emergency Fund (DREF) in 11 countries. In total, 16 countries in Africa launched emergency operations relating to this outbreak.

The Federation played a critical role in the surveillance component through rapid diagnostic tests, Communitybased surveillance and the surveillance around survivors (SA-Ceint). The current security situation is calm despite occasional outbreaks of violence. An increase of basic prices (oil, flour, rice) has however been noted. There are also reported corruption accusation around mining contracts involving country senior leadership and family relatives. As a result, there is expectation of an increase in petty crime (theft, intrusion in residential, petty theft in the streets) as the school holidays begin and people need money (to visit their families abroad for example). In addition, it is anticipated that local elections (municipal) should be held in 2016, no official date has been provided yet. Human Resources: There are currently 11 delegates and 35 national staff. The downsizing process is still ongoing for national and international staff due to the lack of funding and the end of EVD operations. Capacity building efforts are ongoing for national staff by providing internal trainings in HR and Logistics topics. Logistics: There is continuous dismantling of operations warehouses and transfer of all remaining material to the GRC as well as the transfer of additional 8 VRP vehicles to Dakar after the end of contract. Capacity building of logistics staff is also ongoing.

Future Planning: - Conduct of radio broadcasts. B. Psychosocial Support Highlights: Continuation of community dialogue sessions. During July 2016, a total of 19 sessions (Dialogue Therapeutic Community) were held in 7 prefectures: Guékèdou (4), Nzerékore (4), Dubréka (3), Forécariah (1) Kéréouané (3), and Macenta (3) Kindia (1). These activities have provided PSS to a total of 245 community members. Challenges: There are still difficulties encountered in assessing remote field locations during the rainy season. Future Planning: Provision of recreational activities for orphans and other children affected by EVD and other diseases. Continuation of the community healing dialogue sessions. C. Contact Tracing and Surveillance Highlight: After the launch of Surveillance Around Survivors (SA Ceint in French) in the 3 Red Cross targeted prefectures in high Guinea (Kankan, Kouroussa and Siguiri), the activities are currently underway. A total of 5 semen samples (2 in Kankan and 3 in Siguiri) were sent for laboratory testing. In addition, 4 alerts for illness among survivors was escalated. A Vulnerability and Capacity Assessment (VCA) was conducted in the affected communities in order to design a community resilience program in such areas. The general director of the National Agency for Health Security submitted an official request to IFRC for the extension of surveillance activities in all Upper and middle Guinea in relation to the quality of the monitoring and the achievement of the objectives in areas covered by Guinean Red Cross. The Community Event Based Surveillance activities are still ongoing in two prefectures (Boké and Forécariah) with the mobilization of 337 Community volunteers with 773 alerts escalated and 2,654 persons sensitized during door to-door awareness activities. Challenges: Access to some communities during the rainy season is difficult.

Case Management in Red Cross Ebola Treatment Centres Macenta No activities were reported during this reporting period. E. Safe and Dignified Burials and Disinfections of Houses Highlights: 527 samples for Rapid Diagnostic Tests in Conakry, Forécariah, Yomou, Kindia, and Lola Guékèdou, 480 PCR in Conakry, 19 volunteers, Quantification of biomedical waste in 14 Kindia health centers, Planning development of tools and documents for the implementation of projects to support hospitals and medical centres in the collection and destruction of biomedical waste. Challenges: Validation of the body scrubbers proposal submitted to CDC. Future Planning: Continued negotiation for the positioning of items in the SCRC and CRBC. Monitoring of the support project activities to hospitals and medical centres in the collection and destruction of medical waste. Monitoring of RDT activities. Development of the project to support hospitals in Guinea Forest region. Implementing response activities to fight against hepatitis E outbreak. LONG TERM EPIDEMICS PREPAREDNESS PLAN THROUGH THE PREPARE 2 PROJECT IN GUINEA A workshop on the development of a supervision guide for alert teams and response to epidemics (ERARE) 3 took place from 18 to 22 July 2016 in Conakry. The workshop was attended by 13 trainees and 4 representatives from the Ministry of Health mostly from Regional Health Districts (or DRS 4 ). It was anticipated that completion of the first version of the Guide would take place during the week of July 25 to 31, This developed Guide would then be tested in Conakry and Kindia from August 3, This will result in the ccompletion of the second version of the guide that takes into account the observations recorded during the test 2 PREPARE: Projet d appui à la mise en place des Equipes Régionales Polyvalentes d Alerte et de réponse aux Epidémies in french acronym 3 Expertise France: A French government affiliated organization 4 DRS: Direction Régionale de la Santé

6 P a g e 6 Validation of the Guide by the Ministry of Health and its partners is envisaged to take place towards the end August The result will be the completion of the first edition of the Guide which will accept comments from the validation meeting. Training of people who will be involved in this training supervision will take place from September 26 to October 1, Liberia Highlights of key RCRC actions for the EVD Operation Community health awareness messages disseminated by the Social Mobilisation team (31,889 persons were reached) from May 8 - June 9, The Red Cross was responding to the Government of Liberia's appeal to partners to continue with Social Mobilisation and PSS activities/follow-up for a further 21 days. This was in a bid to lead the country to its 42 days with no EVD cases. The PSS team followed up people who completed the first 21 days of observation including the survivors and also carried out community healing dialogues. A. Case Management in Red Cross ETC, Macenta Recovery Activities Community-based Health Trainings: Community-Based Health and First Aid (CBHFA) A total of 122 LNRCS volunteers and two supervisors were trained in Montserrado County; 122 LNRCS volunteers and two supervisors were trained in Bomi County; 30 LNRCS volunteers and two supervisors trained in Gbarpolu County; 30 LNRCS volunteers and two supervisors trained in Grand Cape Mount County Community Event-Based Surveillance (CEBS) A total of 24 LNRCS volunteers and two supervisors were trained in Montserrado County, facilitators from MOH and LNRC chapter; 24 LNRCS volunteers and two supervisors trained in Bomi County, facilitators from MOH and LNRC chapter; 24 LNRCS volunteers and two supervisors trained in Gbarpolu County, facilitators from MOH and LNRC chapter; 24 LNRCS volunteers and two supervisors trained in Grand Cape Mount County, facilitators from MOH and LNRCS chapter; In addition, there was Distribution of CBHFA manuals to target chapters (Facilitator s guides, volunteer s manuals and tool kits); MoU signing with all communities in the targeted chapters; CEBS job aids printed and distributed.

7 P a g e 7 PSS: Volunteer s training on PSS in four out of five targeted chapters Livelihoods: Beneficiaries registered for livelihoods in Bomi and Gbarpolu (EVD survivors, widows, widowers, and orphans) as follows: Backyard guarding seeds and tools support in Gbarpolu (182), Livelihoods Skill empowerments for Widows, Orphans and Survivals in Gbarpolu (40) Backyard guarding seeds and tools support in Bomi (157), Livelihoods Skill empowerments for Widows, Orphans and Survivals in Bomi (66) Procurement of basic inputs for agricultural support (1,000 families with seeds and tools) is on-going. DRR: Training of CBATS in (DRR) in the five recovery chapters of Cape mount, Gbarpolu, Margibi, Montserrado and Margibi (178 CBATs Members); Verification of warehouses for pre-positioning of recovery materials; Ongoing procurement of assorted materials (to CBATS) for use in community based mitigation activities to reduce the impact of risks and hazards faced within their respective communities; Ongoing procurement of sanitation materials for community DRR activities. The Plan of Action up to September 2016 is as follows Health Community household survey by volunteers; Health promotion sessions in communities; First Aid training of community structures; Establish linkages between health facilities and community volunteers or reactivate existing referral system for CEBS; CEBS implementation in targeted communities; Monitoring and supervision visits to branches and communities by chapters and HQ; Conduct WASH needs assessment; Establish or reactivate community WASH committees; Train community water committees. DRR Livelihood skills training including training and apprenticeship; Livelihoods and cash transfer training for chapters and HQ staff; Procurement of selected emergency response materials including PPE for emergency response; Hazard and risk community mapping in disaster prone communities; Creation and strategic posting of modified community hazard maps in collaborations with BenComs; Community level micro risk reduction activities (based on existing hazard and risk).

8 P a g e 8 Development of a food security training curriculum in collaboration with relevant government authorities. Post distribution monitoring. Pre-positioning of response materials for 100 families in each chapter. Establishment of community DRR committee in the 12 communities in each recovery chapter. Establishment of school-based DRR committees in two primary school in two chapters (multihazard prone). Carry out safe school environment activities (fire prevention). Review of the National Society contingency plans. Funding HKD 500,000 in un-earmarked funds from Hong Kong Red Cross. PMN signing in progress. Sierra Leone SITUATION UPDATE The directorate of Disease Prevention and Control (DPC), MoHS summoned implementing partners to set up a Technical Working Group (TWG) committee, to formulate a smooth transition from Community Event- Based Surveillance (CEBS) to Community Based Surveillance (CBS). CBS will contribute to the reduction of mortality and morbidity related to a range of infectious diseases such as viral haemorrhagic fever, Acute Watery Diarrhoea, measles, etc. as well as natural disasters such as floods and wildfires in three districts in Sierra Leone. At the same time CBS will strengthen and empower communities to be in charge of surveillance and reporting within their community; it will contribute to strengthening the Red Cross auxiliary role with the MoHS and within the coordinated national surveillance and response system. The current Swedish Red Cross support of CBS is set to run until the end of 2016 after which the new funding will be needed to support programmatic and running costs in the same geographic location for additional nine months. In this regard, Red Cross has made tremendous contributions to achieve smooth transition; making useful inputs on training and integration plan, transforming disease case definitions to illustration, making it user friendly. Sierra Leone is currently preparing to roll-out CBS countrywide. UPDATE SUMMARY With Port Loko still on paper-based reporting focusing on Ebola specific case definitions and other sickness that affect people in their respective communities, a total of 213 alerts were received from the community based volunteers. 21 alerts were death alerts, while 192 were suspected sick cases including body pain, cough, fever, diarrhoea, headache, malaria, abdominal pain, stomach pain, typhoid skin rash among others. 97 of the sick notifications reported were male, with females making up the remaining 95. Also, 10 of the death notifications were male with 11 recorded for the female gender. 1 and 24 for alerts death and sick notifications respectively were recorded for children below the age of 1 year, with 1 for death and 36 for sick were children within the age bracket of 1-5 years. However, 19 and 127 for death and sick respectively were recorded for those age above 5 years. After assessment by supervisors, a few of the sick notifications were taken to PHUs for treatment. 10 P a g e 10 Age group for notification sick death MAGPI 0-1year 1-5yrs above 5 years SMS alerts were sent and recorded in both volunteers and supervisor s database as cases that needed investigation and response from different communities. A total of 112 excluding zero alerts were recorded, including 1 for suspected VHF, 7 for suspected AWD, 7 for suspected measles but no floods were reported so far. However, the 7 escalated measles notifications from follow-up with supervisor stated samples collected and were sent to DHMT for further investigation and their outcome is awaited. Communities with the suspected measles cases are Nyande in the Yowbeko chiefdom, Bendu town and Gbahama, Mobefa all in the Benducha chiefdom. The single escalated suspected VHF alert was reported from the Mobinty community in the Bonthe district. The summary of SMS alerts received is shown below.
