3 Eligible Professionals Medicare Doctor of Medicine or Osteopathy Doctor of Dental Surgery or Dental Medicine Doctor of Podiatry Doctor of Optometry Chiropractor Medicaid Physicians (Medicine/Osteopathy) Nurse Practitioner Certified Nurse Midwife Dentist Physician Assistant who furnishes services in a Federally Qualified Health Center (FQHC) that is led by a Physician Assistant

4 Eligible Criteria Medicaid Required to have a minimum of 30% Medicaid patient volume for the prior year Pediatricians are required to have 20% Medicaid patient volume Practice is a predominantly Federally Qualified Health Center (FQHC) and have a minimum 30% patient volume attributable to needy individuals Both Programs Providers eligible for both Medicaid and Medicare incentive programs must choose between either one of the two and declare during the registration process Most Eligible Professionals would maximize their incentive payments through the Medicaid program

5 Who is Eligible for the Financial Federal Incentives? A Medicare Eligible Professional (EP) who successfully demonstrates Meaningful Use of a certified EHR system for a 90 day period in the first year is eligible A Medicaid Eligible Professional who either Adopts/Implements/Upgrades (AIU) to a certified EHR is eligible

7 Registration for EHR Incentive Program Medicare Registration Medicare registration is currently open Guidebooks are available through the CMS for the registration process for Medicare EHR incentive program Visit the CMS EHR Incentives program website and click on registration Medicare Eligible Professionals need not own their own EHR system to register for the incentive program

8 Medicaid Registration States are in charge of the Medicaid incentive programs However the registration still happens on the federal level CMS website This is the first step of the AIU attestation process Guidebooks are available through the CMS for the registration process for the Medicaid EHR incentive program

9 Registration Information Requirement by Eligible Professional National Provider Identifier (NPI) National Plan and Provider Enumeration System (NPPES) User ID and Password Call and ask for a NPI specialist if you don t remember Payee Tax Identification Number (if you are reassigning your benefits) Payee National Provider Identifier (if you are reassigning your benefits)

10 Requirements for Stage 1 of Meaningful Use For Eligible Professionals, there are a total of 25 Meaningful Use objectives. To qualify for an incentive payment, 20 of these 25 objectives must be met: There are 15 required core objectives that must be met. The remaining five objectives may be chosen from the list of 10 menu set objectives. There are six Clinical Quality Measures (CQMs)

11 Meaningful Use Core Measures Computerized Provider Order Entry (CPOE) More than 30% of unique patients with at least one medication in their medication list seen by the EP have at least one medication order entered using CPOE. Implement drug drug and drug allergy interaction checks EP and enabled this functionality Yes or No Maintain an up to date problem list of current and active diagnoses More than 80% of all unique patients seen by the EP have at least one entry as structured data E Prescribing (erx). More than 40% of all permissible prescriptions written by the EP are transmitted electronically using certified EHR technology. Maintain active medication list. More than 80% of all unique patients seen by the EP have at least one entry recorded as structured data.

More than 80% of all unique patients seen by the EP have at least one entry recorded as structured data Record demographics More than 50% of all unique patients seen by the EP have demographics recorded as structured data Record and chart changes in vital signs More than 50% of all unique patients age 2 and over seen by the EP, height, weight and blood pressure are recorded as structured data. Record smoking status for patients 13 years old or older More than 50% of all unique patients 13 years or older seen by the EP have smoking status recorded as structured data Report ambulatory clinical quality measures to CMS Successfully report to CMS ambulatory clinical quality measures selected by CMS in the manner specified by CMS

Capability to provide electronic syndromic surveillance data to public health agencies Performed at least one test of certified EHR technology s capacity to provide electronic syndromic surveillance data to public health agencies Implement drug formulary checks Yes or No The EP has enabled this functionality and has access to at least one internal or external formulary for the entire EHR reporting period Incorporate clinical lab test results into EHR as structured data Results of more than 40% of all clinical lab test results ordered by the EP should be entered as structured data. Generate lists of patients by specific conditions Yes or No Generate one report listing patients

15 Meaningful Use Menu Measures Send reminders to patients per patient preference for preventive/follow up care More than 20 percent of all patients 65 years or older or years old or yonger were sent an appropriate reminder Provide patients with timely electronic access to their health information At least 10% of all unique patients seen by the EP are provided timely access within 4 business days. Providing patient specific education resources More than 10% of all unique patients seen by the EP are provided patient specific education resources Medication Reconciliation The EP perform medication reconciliation who for more than 50% of transition of care patients Summary of care record for each transition of care/referrals The EP who transitions or refers their patient to another setting of care or provider of care provides a summary of care record for more than 50% of transitions

16 Clinical Quality Measures Must report on 6 total Clinical Quality Measures (CQMs) They are required to report on 3 core or 3 alternate core measure that must be meet. Core: NQF0013 Hypertension: Blood Pressure Measurement NQF0028 Preventive Care and Screening Measure Pair: a. Tobacco Use Assessment b. Tobacco Cessation Intervention NQF0421 Adult Weight Screening and Follow Up Alternate Core: NQF0024 Weight Assessment and Counseling for Children and Adolescents NQF0038 Childhood immunization Status NQF0041 Preventive Care and Screening: Influenza Immunization for Patients 50 Years Old

17 Clinical Quality Measures 3 additional measures must be reported on There are 38 to choose from based on your specialty and your system capability For Stage 1 there are no thresholds that need to be meet. The requirement is to be able to tack and report
