2 RFP BED BUG EXTERMINATION SERVICES QUESTIONS AND RESPONSES QUESTION 1: How many facilities are there total? RESPONSE 1: MHMR consists of approximately sixty (60) facilities. QUESTION 2: Which facilities will be quarterly? RESPONSE 2: The facilities for quarterly inspections are listed on Exhibit 1: Quarterly Bed Bug Service Locations (page 26 of 26). QUESTION 3: Are there diagrams of the facilities? RESPONSE 3: There are diagrams for some of the facilities. QUESTION 4: Will diagrams be made available for the bid? RESPONSE 4: The diagrams that exist for the quarterly inspection facilities are included in this document (see Attachment). QUESTION 5: How many facilities are there outside of the quarterly facilities? RESPONSE 5: There are approximately 55 facilities outside of the quarterly facilities. QUESTION 6: Who is the incumbent vendor? RESPONSE 6: MHMR does not have a current contracted vendor. QUESTION 7: Are any of the other facilities as large as the 70 bed facility (E El Paso)? RESPONSE 7: No, the 70 bed facility is the largest facility. QUESTION 8: Is MHMR s goal for bed bugs to be exterminated or controlled? RESPONSE 8: MHMR goal would be for extermination. However, due to the transient nature of our business, MHMR s goal will be for control. QUESTION 9: Are there any protocols for new clients or residents at the facilities? RESPONSE 9: The facilities have individual protocols for new clients or residents. QUESTION 10: Will there be a single protocol in place? RESPONSE 10: Because of the different regulations and restrictions by the various governing entities over MHMR facilities, there will not be a single protocol in place. QUESTION 11: Is part of the scope of services by the vendor to include various protocols? RESPONSE 11: Vendors may include protocols in their response. QUESTION 12: With an infestation treatment, will we be able to put a checklist in place? RESPONSE 12: The awarded Contractor will be able to put a checklist in place with an infestation treatment.

3 QUESTION 13: Of the all the facilities, how many are residential, crisis, clinical, and administrative? RESPONSE 13: MHMR facilities consist of approximately 22 residential, 4 crisis, 14 clinics, and 19 administrative facilities. Some facilities are multi-functional. QUESTION 14: Do all beds have bed bug covers? RESPONSE 14: Not all beds have bed bug covers. QUESTION 15: Are all the beds the same size? RESPONSE 15: Not all beds are the same size, depending on the type of facility (i.e. residential group home). All MHMR provided beds are twin size beds. QUESTION 16: Does MHMR provide all the beds? RESPONSE 16: MHMR provides beds for the facilities on the quarterly service list. MHMR does not provide all beds at other residential group home locations. QUESTION 17: Do you have bunk beds? RESPONSE 17: There are some bunk beds located at 815 S Jennings and at 1501 E. El Paso. QUESTION 18: Do you want covers on all the beds? RESPONSE 18: Covers for beds will be addressed by each facility with awarded contractor. QUESTION 19: How will the quarterly inspections be scheduled and who will be making up the schedule? RESPONSE 19: Quarterly inspections will be scheduled between awarded contractor and facility manager and/or Property Management.
