4 Background Health Human resources studies conducted by the Health Care Human Resource Sector Council of NS 1, the Prince Edward Island Advisory Committee on Health Human Resources 2 and by The Atlantic Advisory Committee for Health Human Resources (AACHHR) 3 identified a number of specific needs for professionally trained and licensed health care workers, in both provinces and across the Atlantic region. The focus of Health Human Resources Departments in the Atlantic Provinces has to date, been primarily on recruitment of Physicians and Nurses. Experience with the International Medical Graduate (IMG) activities will most certainly contribute to a knowledge base on the successful integration of IEHP s but as a whole, the provinces currently have very limited and dispersed knowledge regarding the needs and capacity to integrate these and other Internationally Educated Health Professionals. Sustainability of services to assist with the resettlement and integration of IEHP s and their families to NS ands PEI is another factor that will have major implications on the future of IEHP s as a source of trained and licensed health human resource professionals for NS and PEI. Price-MacDonald & Associates Consulting Inc. (PMA) has been contracted through the Department of Health, Nova Scotia, to conduct an Environmental Scan and Gap Analysis (ESGA) in relation to Internationally Educated Health Professionals (IEHP s) in Nova Scotia, Prince Edward Island and Newfoundland & Labrador. In addition to carrying out the ESGA project, PMA was asked to assist with overall project management and support for five other projects that are under development in the area of IEHP support and education and to help facilitate Atlantic wide communication and dialogue around IEHP issues. This interim report will review progress to date and discuss future directions for the IEHP projects. 5 The overall goals of the ESGA project are to: 1. identify key issues relating to integration of IEHP s and their families in NS and PEI 2. identify opportunities for better utilization of IEHP s in the health care systems of NS and PEI 3. implement and evaluate demonstration projects to facilitate recruitment and retention of IEHP s in the region; 4. develop a strategic plan and make recommendations to HHR Departments for recruitment and retention of IEHP s in NS & PEI 5. Facilitate Atlantic Canadian dialogue around HHR needs IEHP issues and initiatives. Project Plan A project plan was designed and presented to the Steering Committee in February of The plan involves five phases to the project over five years: Phase I: Gap analysis December 2005 to March 2006 Phase II: Consultations with Stakeholders - April 2006 to November 2006 Phase III: Strategic Plan & Evaluation Framework December 2006 to March 2007 Phase IV: Demonstration Projects, TBD - April 2007 to March 2009 Phase V: Final s & Regional Conference September 2009 to March 2010 Progress to Date Phase 1 of the ESGA project ended in March of In addition to assisting the Project Manager of DOH NS to plan and organize the IEHP project as a whole, deliverables for Phase 1 included: an annotated bibliography of a literature review of IEHP activities for posting on a project web site; a database of support services and analysis of gaps in services, to be posted on the planned web site; and initiation of a study of Education and Evaluation of International Medial Graduates (IMG)'s. These are further defined in following chapters of the report. In relation to the project management function, since April 1, 2006, PMA has: contracted a web development and hosting firm to design a visual identity and build a bilingual web page to support IEHP projects; facilitated the translation to French of deliverable documents for the ESGA project and the proceedings for the March, 2006, Project Lead/Steering committee meeting in preparation for posting to the web page; made available support in relation to secretarial services and translation of documents for project leads in NS and PEI; retained a consultant to facilitate stakeholder surveys in the summer and fall of

6 Project Stakeholder input will be sought through survey, focus group and interview between July and November A second interim report that will include data analysis and recommendations for demonstration projects to recruit and retain IEHP s in NS and PEI will be produced. The project work plan identifies the design of an evaluation framework for the projects and implementation of the projects by April 1, Project Demonstration projects will be implemented by April 1 st, 2007, and evaluated by August 31 st,2009. A final report with recommendations for governments and health human resources will be compiled by December 31, The final report will detail recommendations for governments An Atlantic Canadian Conference will be held in March of 2010 to disseminate findings and to create forward direction. 6

8 - Gap Identification Literature Review The literature review of activities relating to IEHP s in NS, PEI and Newfoundland & Labrador was conducted by way of an extensive search of IEHP activity across Canada and then the Atlantic provinces. 4 The data was derived from a number of sources that involved: internet and other access to federal and provincial governmental agencies, professional organizations, licensing bodies, educational institutions, and service and religious organizations involved with resettlement of immigrants and specifically with IEHP s. Annotated Bibliography The annotated bibliography detailing the results of the literature review is included as Appendix A. It details the contact information and involvement of each party with either immigrant support or IEHP education or licensure. Pan Canadian sources are cited but the focus is on Atlantic Canada, specifically PEI and NS. The data base will be translated and published on the IEHP web page and will be available for information of IEHP project leads and others, for reference and for dissemination of information. At this time the annotated bibliography contains a summary of the information available. More specific information to be provided through consultation with the organizations will be published on the webpage. The annotated bibliography and reference lists will be updated regularly as the research continues into the fall of Reference List The reference list details the number and type of reports and notes collected through the literature review and is included in Appendix B. These reports have been compiled into reference manuals and will be enhanced as the project progresses. The list will be circulated to IEHP project leads. Access to the reference documents will be made available to IEHP project leads and others through PMA, as requested. Authors will be requested to have their reports linked to the IEHP Atlantic Connection website. Services Review The review of services for immigrants and IEHP s in NS, PEI and Nfld., was conducted by telephone, web search and service directory review. Information on settlement and immigrant integration service delivery organizations, educators of health professionals, regulatory bodies and religious organizations was collected. The information was tabulated to include brief descriptions 4 New Brunswick and Newfoundland & Labrador are not formal participants in the current IEHP projects, however some data has been collected from each, with the hope that future initiatives will expand to include those provinces. 8

9 of services provided and reviewed for trends and gaps in service. The detail of services presently available to IEHP s in NS, PEI and Nfld. is included in Appendix C. Also reviewed was services provided to international students at NS Universities. The Health Human Resource Database for NS for Regulated and Non-Regulated Health Professions was another source of information. This document alphabetically lists regulated and unregulated health professions in NS and details educational institutions offering training for the specific profession, the basic entry to practice requirement and the regulatory body responsible for the particular worker. This service review documents will be translated and posted o the IEHP web page for use and reference of all IEHP project leads and to facilitate dissemination of information. Gap Analysis Initial results of the literature review and the service reviews provided information that was analyzed for patterns. A number of probable gaps began to emerge. In combination with information that will be gleaned through stakeholder surveys to be completed between July and November 2006, the data will be statistically analyzed to verify the initial patterns and identify further possibilities for demonstration projects. Information was obtained from a variety of service providers in Nova Scotia, PEI and NFL/Labrador. Additionally, these gaps have been compared to the scan of services to determine accuracy. This list is not presented in a particular order of priority however, all are important and deserve equal attention. Small Professional Groups Especially those on PEI, do not provide any information re IEHPs on their websites. Currently, several small professional groups do not have queries from IEHPs therefore there is a belief that there is little reason to spend limited resources on web enhancements. The reality is that many prospective IEHPs find their information via the web before they immigrate. This information helps them determine where in Canada they may choose to move too so no information on a website sends the wrong message. Recommendation: Identified groups would benefit from financial support to upgrade their websites. Interpersonal Skills for IEHPs Societal orientation is as important as information about the Canadian health care system. This certainly varies by region Atlantic Canada is different, in a cultural sense, than central or western Canada. Programs dealing with the Canadian work context are important and necessary, however there is also a need for immigrants to understand and appreciate other nuances of Canadian life. These interpersonal or softer skills are important in the successful integration for the immigrant especially in Atlantic Canada where there may not be a large community that shares their culture. The more difficult it is for an immigrant to fit in, the more likely it is that they will leave to seek 9

10 out a larger community. Nearly 75% of all immigrants to Canada choose to live in Toronto, Vancouver and Montreal. (Educator Roundtable Dialogue Halifax, 2005). Recommendation: Settlement agencies require funding to provide programs focusing on softer skills. A cost effective process Regardless of ones commitment to pursue recognition of their credential, there should be an opportunity for all IEHP's, regardless of discipline, to view the system, the work and the clients with no pressure to perform simply to observe. In some cases, an IEHP may decide to NOT pursue their profession. Therefore, this aspect of self-selection can be cost effective. These observational experiences could be developed for all the healthcare professions currently being studied. Recommendation: Create a program that supports this activity Observorships for IEHPs. This could become a key service to help all IEHPs, especially for those who need to determine whether pursuing recognition of their credential is worthwhile. Well informed Provincial governments Office(s) of Immigration in each province are in need of good information about the various health professions. These Offices are attending Job Fairs in a number of locales globally. The premise of their participation is to share information not to recruit nor represent potential employers. As these Offices are recent additions to government, part of their learning curve includes pertinent information from the professional organizations regarding policies/positions on active recruitment of IEHPs. Recommendation: Current videos/brochures in a number of languages that addresses health professionals would be beneficial. Furthermore, any message (regardless of provider) must be reviewed to ensure that we are not engaging in poaching of health care professionals from other countries. A soft welcome mat is most appropriate and the focus of communication for the IEHP regardless of the media used - needs to be on the Welcome vs. Yes, you can come but...here are the list of obstacles you will have to overcome. Limited Services Small professional groups (i.e. MLT, MRT, etc) provide limited services. These groups have little or no capacity/staff in their provincial associations to provide guidance for queries from IEHP's let alone a process to help one achieve a Canadian credential. Recommendation: Atlantic-wide partnerships amongst these groups created with a focus on creating pathways for interested IEHP's. Outcomes often take significant time IEHP s lose hope/accept underemployment/become discouraged and may decide that a current job is going to be their future i.e. cab driver. Recommendation: Review timelines attached to specific services to ensure that timelines are reasonable and achievable and are communicated to the IEHP at the outset. 10

11 Reliance on technology There is an assumption amongst a number of service providers that IEHPs are all computer literate or have unlimited access to services available on-line. Recommendation: All pertinent information and application forms etc. must be available in hardcopy. Creation of new materials should be shareable on different formats i.e. CDs, DVDs, downloadable to PDA s, etc Settlement agencies employ few staff and few IEHP s Having small numbers, it is difficult to justify resources expended for one of s i.e. one staff assigned to assist an IEHP Social Worker or an MRT. Atlantic Canada numbers are low therefore out of necessity - an interdisciplinary approach has been adopted by settlement agencies for ESL programs i.e. all health professionals take the program together. This is the preferable methodology and should always be considered when new programs are being developed. Recommendation: These agencies need to tie in with their provincial govt. as well as other settlement agencies to expand their capacity and know what they can (and cannot) offer to an IEHP before frustration sets in and the IEHP leaves the province. Lack of communication Many immigrants brought to Canada via the Provincial Nominee program are often not made aware of MISA/Newcomers groups etc. Additional supports and programs provided by these groups are critical in ensuring that the IEHP (and their family) can achieve integration into society. Recommendation: Offices of Immigration/Citizenship must provide provincially specific and current information to these Nominees to ensure they are aware of what is available in their communities and whom they may contact for further guidance. International students International students assocs. at universities/colleges are not linked to the immigrant settlement groups nor are they currently allowed to any of the services. Recommendation: Govt. needs to revisit this group and the regulations associated with their stay.these students are graduating in Canada with Canadian credentials. It could be very cost effective to invest in this population. Services centered in the urban areas. IEHP s must be encouraged to settle in rural areas also. The perception that necessary services are only available in larger centers may cause IEHPs to seek our larger cities rather than smaller communities. Recommendation: Settlement agencies should be supported to expand their services and provide other means such as: one day workshops in other parts of a province to connect personally with IEHPs. Ideally, interpersonal contact should be part of every program rather than relying solely on web-based delivery. 11

12 Atlantic Canada Website for IEHP s Provides portal for IEHPs to access current relevant information about Atlantic Canada regardless of their location allows data gathering from their country of origin or from other locales in Canada. Recommendation: While recognizing that face to face interactions are very important, there should also be one website for IEHPs interested in Atlantic Canada. This site should have links to all the projects/services available across the region. Low cost assessment tools There are a variety of methods used in health care to assess skills many of these processes are very costly and require considerable input from professionals. Clearly, not all assessments can or should be changed however looking outside of the health disciplines for some synergies should not be viewed negatively. Recommendation: Investigate processes utilized by other professions i.e. Professional Engineers create a professional diary. Flexible Residency programs For IMGs who have already practiced in their home countries, having to start over can be frustrating and counter productive. Many of these professionals may decide it is too onerous to gain an MD in Canada and opt for other employment. This is not to suggest that the IMG should be given credit for the entire program. Recommendation: IMGs should be able to enter these programs with advanced standing. Availability of clinical skill enhancement programs. Many IEHP s do need some upgrading that addresses knowledge/skill gaps etc. without requiring repetition of large segments of /or entire programs. Recommendation: Short term clinical skills enhancement programs need to be developed for all IEHPs (not just MDs and RNs). Accurate data on the number or the location of IEHPs in Atlantic Canada. In order to determine size/locations etc. of programs/services, an approximate number of IEHPs in the region would be helpful. Recommendation: Develop a directory of IEHPs perhaps at a settlement agency encourage IEHPs to stay in touch with updated contact information Large amounts of personal information required Immigrants (including IEHP s) have to submit a large amount of personal information/academic history to a number of groups for scrutiny i.e. regulatory bodies. Transcripts of post-secondary education, translated documents, resumes, language benchmark grades etc. are all necessary pieces of information that an IEHP will need to share with a number of agencies. Recommendation: All IEHP s should be afforded the opportunity to take a Portfolio course (usually 30 hours or less) that provides them with the purpose and process of compiling a personal Portfolio that is developed with a future employer in mind. 12

13 Employing agencies may not accept IEHP s. Atlantic Canada has not experienced large numbers of immigrants and there are few large communities of immigrants hence the draw of larger centers such as Toronto and Montreal. One of the by products of this inexperience is a lack of sensitivity towards others from away. While the term come from away could be considered to identify someone from another country, it is in fact a very common term used to identify people from other parts of Canada. Clearly, if segments of our population refer to other Canadians in a less than welcoming manner, there are some educational needs that should be addressed. Recommendation: Cultural Sensitivity workshops should be provided to employers/employees to increase awareness and hopefully support the IEHP s integration into that workforce. IEHP s are relatively isolated from one another. IEHP s that are integrating into communities and doing well could extend a helping hand to others as needed. Recommendation: Development of a non-profit organization for IEHPs perhaps housed at the settlement agencies but not administered by them. This would then become a place of support, information and networking for IEHPs and their families to meet and help one another. Start up funding would be required with the sustainability of the organization left for the IEHPs to manage. International Medical Graduates International Medical Graduate (IMGs) To address the specific issue of recruitment and retention of International Medical Graduate (IMGs) in NS and PEI a proposal has accepted from Med-Emerg Inc. to develop a strategic framework for the international medical education program in NS. The project will provide an improved understanding of the information sources, assessment process (e.g. credentialing) preparedness, integration and impact of implementing international medial graduates into Nova Scotia s health care system. The project also proposes an evaluation process and research plan to determine the impact and effectiveness of international medical graduates on health, system and provider outcomes. 5 The proposal is included in Appendix D. 5 Med- Emerg, Proposal For International Medical Graduates, DOH NS, March 24,

14 Initial IEHP Project Team/Steering Committee Meeting Proceedings of the Meeting The initial meeting of the project leads was held in Halifax in March of 2006, under the auspice of the Nova Scotia Department of Health. Minutes of the meeting have been translated and will be circulated to participants and posted on the webpage for information of interested parties. The team lead meetings will be an integral part of the ongoing project management plan for the implementation of the IEHP Atlantic project. Conclusion Phase one of the Environmental Scan & Gap Analysis project is completed as planned. Phase two is underway, with resources having been assigned and task lists identified. The visual identity package and webpage is under development and awaits approval of the steering committee. Documents are in process of being translate in preparation for posting on the web. The consultation process with stakeholders is in the planning stage and will be implemented in July of The next meeting of the project team leads is planned for fall 2006 in Prince Edward Island. The second interim report will be prepared for December 2006 and will include recommendations for demonstration projects to be implemented under the lead of individuals/ groups in both PEI and NS. The project has already generated useful information and the database will be expanded as new learning takes place. 14
