Diagnosis Interview Role Plays Role Plays Written Case Conceptualization Purpose of Informed Consent The purpose of this consent form is to provide you with essential information about supervision and to ensure a common understanding about the supervision process. The process of supervision and these guidelines will be discussed in detail at the first meeting. Questions may be asked about the supervision process at any time. Requirements for Practicum & Supervision Hours Practicum requires 150 total hours, with a minimum of 40 direct service hours (face to face). Direct service hours are defined as direct client care with the student as the primary therapist, with either individual clients, couples, or families. At least 16 hours (40%) of the direct service hours must be with these types of clients. Therapy or process group hours and/or interactive psychoeducational experiences may be included in direct service hours as well, as long as the minimum 40% individual clients is maintained. After the required hours are met, the hours can distribute however between individual & group. We encourage co-therapy experiences with supervisors; if a supervisee is doing co-therapy with a supervisory staff member or more experienced student in a supervisory role, then the supervisee must be the primary therapist or have a majority role during the session for the session to count as direct service for that student (simple observation would count under supervision hours). Indirect service hours may include supervision (site and Lipscomb), staff meetings, orientations, trainings, client paperwork, observation of another s client session/intake for training purposes, conversations regarding client issues, and time spent reading or watching videos of clinical topics required by the site or supervisor. See the Clinical Experiences Handbook for further explanation of the entire clinical experiences year. Client Load We expect that a student will meet with a minimum of 5 different clients on an ongoing basis throughout Practicum. We know that certain practicum sites do not work on a traditional therapy model. For any difficulties in meeting this requirement, you and your supervisor will determine the best course of action. Documentation We expect a student to become competent and proficient in client documentation skills. While we will instruct on documentation methods, you are expected to be compliant with your particular site s documentation protocol. You will be maintaining your time logs and a notebook to keep your site and supervisor information, your client summaries and progress notes, and any other paperwork. See the notebook guidelines for specific requirements. On your week to present a video, you will turn in sample documentation (e.g., progress notes and treatment plans) regarding that 3

4 client and any other clients for your supervisor s review. Notes are expected to be properly redacted to protect your client s confidentiality. Evaluation Your site supervisor and your LU supervisor will give you feedback on a regular basis regarding your performance and progress. If you are not receiving regular feedback at your site, please inform your LU supervisor and/or the clinical director. You will be responsible for setting a date with your site supervisor to complete the midterm and final evaluation forms. Please allow at least 2 weeks before the evaluations are due to begin discussions with your site supervisor. Late evaluations are strongly discouraged. Your LU supervisor will speak/ with your site supervisor bi weekly during your semester. Attendance You are required to meet with your Lipscomb group for supervision weekly during the semester. If you fail to meet a minimum of 12 meetings, you may not receive credit for the course. You may earn participation points for each week. Please note that you will not be able to earn the participation points for any class missed. Timeliness is expected so that we can make the most of our time together and not have to stay late. One missed session may be rescheduled in a manner agreed upon with your supervisor. It is up to your supervisor s discretion whether additional missed sessions will be made up. Any activities or assignments that are missed by the student because of an absence are the student s responsibility to make up. Adjustments will be made for holidays, school breaks, or inclement weather. Individual time with your supervisor can be requested at any time and may occur for video feedback by request of either the LU supervisor or the student. Please do your best to be on time and ready to start. A tardy may cause a loss of points (e.g., loss of participation points for that week). Excessive tardies (more than 2) may be charged with an absence. Legal & Ethical Behavior Lipscomb University and the Graduate Counseling Program expect our students to maintain appropriate legal, ethical, and professional behavior. As a counseling student, you are expected to perform and act in a manner consistent with the American Counseling Association s Code of Ethics and with Federal and Tennessee State Laws governing client care and professional behavior. Students should learn how to avoid dual relationships, to abide by confidentiality, to properly manage client suicidality, or to report abuse, neglect, and duty to warn situations, should they arise. The supervisee agrees to always act in a fashion that will not jeopardize, harm, be injurious, or potentially damaging to clients, including avoidance of any sexual contact with clients, current or past. Violations will be dealt with strongly by the Clinical Director and Graduate Counseling Director. Social Networking Students who use social networking sites (e.g., Facebook, MySpace, Twitter) and other forms of electronic communication should be mindful of how their communication may be perceived by clients, colleagues, faculty, site supervisors, and other mental health professionals. As such, students should make every effort to minimize visual or printed material that may be deemed inappropriate for a professional counselor. To this end, students should set all security settings to private and should avoid posting information/photos or using any language that could jeopardize their professional image. Students should consider limiting the amount of personal information posted on these sites. Students should never accept a client as a friend or expose a client through pictures, posts, tweets, or otherwise as part of their social network, since doing so constitutes a boundary violation. Additionally, any information that might lead to the identification of a client or represent a violation of client confidentiality is a breach of the ethical standards that govern the practice of a counselor or counselor in training. Engaging in these types of actions could result in the student being dismissed from the program. Client Confidentiality All client information revealed in group supervision is strictly confidential. Students who breach client confidentiality will be disciplined according to procedures set for in the Graduate Student Handbook. Students are expected to properly inform their clients of their student status, including their need for supervision and their time frame onsite. All documentation of client information should be treated with the utmost of confidentiality. Client paperwork brought in for review should contain no specific identifying information; notes are expected to be well redacted. Client paperwork and 4

5 videos are expected to be disposed of properly immediately after their educational value is completed, specifically by the end of the semester. We will discuss these procedures in supervision group. Professionalism As a representative of yourself, LU, and the Graduate Counseling Program, exhibiting professional behavior is a must. Such behaviors may include compliance with the dress code of your facility, compliance with all of the site s policies just as a paid staff member would be required to do, being on time for your duties, and using appropriate language at your site. We expect that you will only miss work for legitimate reasons, giving proper notice given to your supervisor as soon as possible and always before your shift was to begin. To be as successful as possible, we suggest that you demonstrate a positive attitude which shows a willingness to try new things, that you receive feedback appropriately, and that you strive to be a team player. Personal Disclosure Supervision is not intended to provide the supervisee with counseling or therapy, although personal issues may be discussed as they are relevant to client treatment. If personal issues or concerns arise that interfere with or negatively impact client care, the supervisee agrees to seek support to immediately resolve these issues. We expect that everyone will respect each other s privacy in supervision group. Confidentiality will only be broken by your LU supervisor in the following situations: 1) the completion of necessary documentation regarding your performance during the semester, 2) any situation where the treatment of a client violates legal or ethical standards, 3) any situation when problems or disagreements between the student and the LU supervisor do not seem resolvable and outside consultation with the clinical director is required, and 4) situations where disciplinary action or termination of the supervisee is being considered. Safety at Site We want our students to feel safe at their practicum sites, both physically and emotionally. It is important for you to remember to take normal safety precautions with volatile or aggressive clients, by doing things such as always sitting closest to the door. Please follow all of the safety regulations at your site. We will not allow you to be at your site alone, nor see clients when staff are not present. This is a potential liability for you, the site, and LU. If you are required to travel with your site to see clients, we would prefer that you do so with another staff member, especially early in your semester there. Always let your site supervisor (and a friend) know where you are going, which client you are visiting, and when you should return. In addition, if you are experiencing any discomfort emotionally at your site, whether from harassment from a staff member or client, or whether emotionally you feel unable to handle your caseload, please discuss this immediately with your LU supervisor, and if necessary, we will include the Clinical Director and/or Graduate Program Director. Class Schedule Disruption Policy Except in the rarest of instances, Lipscomb University does not cancel classes or close offices. However, should an event (weather-related or otherwise) occur that requires disruption of the entire Lipscomb University class schedule, students will be notified via multiple venues including the Lipscomb homepage ( a text message sent through LU ALERT ( and the Lipscomb Weather Information Line ( ). For information on possible snow-related closures or late starts, students should consult local television stations (2-WKRN, 4-WSMV, 5- WTVF and 6-WZTV, and Channel 9 on campus). Students should look for information regarding Lipscomb University, not David Lipscomb Campus Schools. If the LU supervisor is unable to reach campus for weather related reasons, then an will be sent to the class at least 1 hour prior to the normal start time. Student s Responsibility for Notification of Extended Absence In the case of individual absences, students should contact their supervisors directly. However, students who find themselves in circumstances which would cause them to miss meetings/practicum for an extended period of time should contact your supervisor immediately. When necessary, Steve Prewitt, Associate Provost for Student Academic Support, (steve.prewitt@lipscomb.edu; ) will be notified for any assistance as well. LU Code of Student Conduct 5

6 Because Lipscomb s primary mission is to integrate Christian faith and practice with academic excellence, integrity is important in this course. Your LU supervisor will deal with each student fairly and honestly. As students, you are expected to do your own work on all assignments unless expressly indicated that collaboration is allowed on a specific assignment. Penalties for integrity violations will range from failure on the assignment involved to failure in the course. The instructor also reserves the right to report violations to members of the administration. For clarification, refer to the Graduate Student Handbook, University s Code of Conduct ( and Academic Integrity Policy ( Supervisor Expectations & Practical Issues As your Lipscomb supervisor, I am here to support you in your growth as a professional counselor through my time and energy in group and individual sessions and proper, timely feedback. I am available for any concerns even outside our scheduled time that you may have about your site, site supervisor, clients, or your ability to perform your expected duties. You may contact me by or phone, but if you need me urgently, call my cell phone. I expect that you will be compliant with the above University, departmental, and course expectations. Feedback As your Lipscomb supervisor, I am required to give you appropriate feedback on your performance, assign your grade, and document our interactions. I will provide you feedback in both formal and informal methods throughout the semester. We will discuss communication I have with your site supervisor. Summaries of our group and individual sessions will be recorded as well as any specific directives I give you about handling a case. I will be looking to document your competencies in counseling skills. At the end of the semester, I will complete a summary document which will record your total hours, requirements met, and an overall performance review. Your grade will be based on the responsibilities, goals, and objectives in this supervisory contract as specifically outlined below. Emergencies - I expect that you will contact me within a few hours (if not immediately), if there is an emergency at your site. After the immediate crisis is resolved with the aid of your site supervisor or clinical director or if you need assistance in resolving the crisis, contact me. An emergency is defined as a suicidal client, the necessity of hospitalization or commitment, the necessity of breaking confidentiality to report any abuse, neglect, suicidal, or homicidal threats (if the need for a breach has not been discussed with your site supervisor, call me before you break confidentiality), legal requests or actions, or some other serious conflict with a client or person at your site. When these situations are present, speak immediately with your site supervisor, the site s clinical director, or if they are unavailable, another licensed clinical staff member to first protect the client. It is fine to step outside your room during a session to ask for help (e.g., asking closest staff person to find your supervisor), trying not to leave the client alone (you may also take the client with you to find your supervisor). You will need to follow your site s specific protocol for handling an emergency. Crisis situations require very detailed documentation of the event, such as recording things the client said to indicate the emergency, your decision making, your immediate steps, any supervision/consultation during event, any outside contacts made, and a safety plan. I will want to see all of this documentation for that client until the situation is clearly resolved days or weeks later. Abuse Reporting If you suspect that your client (or another identified person) is suffering abuse or neglect, you are required by state law to report this to the appropriate authority. You should first discuss your concerns with your site supervisor for aid in decision-making and determination if report needs to be made. If there is a conflict with your site supervisor (or site clinical director) about the need to report, please discuss your concerns with me. If necessary, we will discuss the situation further with your site supervisor. My Commitment - I will follow ethical codes and standards as it is related to treating supervisees with respect and dignity. Conflict During the course of supervision, there may be disagreements about the strategies, interventions, procedures, processes, or other issues. The supervisee should address any concerns with me, and we will attempt to resolve the conflict or disagreement. In the event that a satisfactory resolution cannot be achieved, the supervisee has the right to request a meeting with the Clinical Director (or Graduate Director) to attempt to resolve and mediate the dispute. Participation - Together we are a learning community. You must be in class on time with readings and assignments completed to make the most of this experience and contribute to others learning. While in class, you are expected to participate fully and respectfully. When you miss class, you miss the opportunity to learn from your peers, which 6

7 cannot be made up. You also prevent your peers from learning from you. Any absence, excused or unexcused, will result in a loss of participation points. Please notify the professor before missing class or as soon as possible. Professionalism - Professional behavior and dress are expected at all times. You are responsible for knowing and adhering to the ACA ethical code. Use discretion and respect confidentiality. Learning - You are expected to contribute to a safe learning environment. While it is perfectly acceptable (and welcomed) to disagree with the professor and peers, you must express your opinions respectfully. Condemning, hurtful or prejudiced comments will not be tolerated. Electronic Assignments - You may most assignments to me instead of a paper copy. Please name the file with your name first and the title of the assignment. For example: Jane_Doe_Journal 1 Late Assignments - Late assignments will be accepted, but you will only be able to earn 90% of the points available on that assignment. Assignments are considered late if not completed by the start of class on the due date. Revising Assignments - Because I believe that mistakes are full of learning potential, you have the opportunity to revise and resubmit nearly any assignment to earn more points. The assignment will be re-graded and you will earn the higher grade. I also reserve the right to return assignments that are done poorly or do not follow the instructions for you to revise. I expect your best work, but your final work quality is your choice. Cell Phones - Cell phones are a distraction to you, your classmates and your professor. Please put them away during class time. I understand that at times you may need to be available by phone. If you need to take a call, step out into the hall to avoid disrupting your peers. If your phone rings in class, please silence it immediately. Texting is inappropriate except during breaks. If you urgently need to text, please step out in the hall or wait until a break. Distracting behavior may decrease your participation points. During breaks you may make phone calls and text if needed. Laptops - Laptops can be a learning tool, but they can also be distracting for you, your classmates and your professor. If you need to bring your laptop for learning purposes, then feel free to do so. Checking , playing games and social networking sites are inappropriate during class time; however, you may do so during breaks if needed. Distracting behavior may decrease your participation points. Grades - You are responsible for you. My goal is that you will grow tremendously in your counseling knowledge, skills and confidence during this class. I am available to help you, and you are welcome to , call or set up a meeting with me anytime you have questions. However, I do not give grades. The grade you earn is completely your responsibility. Incompletes - As a rule I do not do incompletes. Exceptions must be extreme cases. Therefore, you are expected to complete all course requirements by final exam time. Participation: Together we are a learning community. You must be in class on time with all assignments and readings completed to make the most of this experience and contribute to others learning. While in class, you are expected to participate fully and respectfully. When you miss class, you miss the opportunity to learn from your peers, which cannot be made up. You also prevent your peers from learning from you. Any absence will result in a loss of participation points. Your job in class is to be fully engaged and to demonstrate what you are learning by your participation. Videos Practicum will require a minimum of 2 video recordings of 50-minute individual client sessions per semester. Students will sign up for presentation dates on the first day of supervision. The video segments will be selected by the student to demonstrate a will be approximately minutes. Videos should be cued and ready to show. If you do not have a video on your assigned date, you are responsible for finding a classmate to show a video in your place. Late videos will be accepted and may be shown on the make-up date as time allows, but you may only earn up to 90% of the points. One video will need to be an intake session, where you are required to introduce the session, to cover the limits of confidentiality, to conduct a clinical interview, and to discuss treatment goals. A second video will need to capture an exploration phase where further clinical interviewing occurs to fully understand the client s difficulties and goals. If your site absolutely will not let you videotape your session, audio tapes will be accepted. In the absence of either option, it may be necessary for your LU supervisor to observe a session live, with the cooperation of your site supervisor or make arrangements to carry a client at the University Counseling Center on campus. Case Presentation: Before showing the video, students will give a brief verbal introduction, using the Tape Presentation Form and proper case presentation style, of the client and session to help viewers understand the clip. Students or LU supervisor may request that the LU supervisor watch more of the video outside of class. Case Notes: Case notes for the client are due on the night of your video presentations. Please bring notes and treatment plans for that client as well as any other clients, minimum of 5. Notes should be properly redacted. Written Case Conceptualization For one video (and for one mock client session in class), the student will write a written case conceptualization in narrative form. Guidelines will be given in class. Verbatim Transcript: Please submit a written transcript of selected video segment of each of your video recordings with your reflections. The following format is suggested. Include your thoughts and reactions in parentheses. In addition, use your textbook, chapters 8-11, to identify an therapeutic interventions that you have utilized in your video. EXAMPLE: Counselor 1: Hello, BJ, how are you today? (I am tired today) 8

9 BJ 1: Oh, OK, I guess. (She looks really sad) Counselor 2: What seems to be the trouble? BJ2: Well, it s a long story (Do I have enough time to listen?) Theoretical Case Conceptualization Form For one video, students will complete the theoretical case conceptualization form which assesses the client from different theoretical paradigms. These theoretical insights should be presented as part of the case presentation. Written Mock Dr. Referral Letter: Students will write a doctor referral letter based off the mock client case in class. Format will be provided. CSSR-S Training Students will go online to and complete the training. Print the completion certificate and turn it in to the LU supervisor. On this website, at the bottom right hand corner, it says CSSR- S Training. Click this link to go to the next page. You will need to create a login ( ) and password. On the following screen, you will see My Activities on top left corner. On the next page, choose English (or your preference) for the training. Once that is clicked, a blue line will open under. Click and wait for videos to load. Professional Disclosure Statement Each student will be required to write a professional disclosure statement to aid the student in developing his/her own introductory conversation to inform the client of the therapeutic relationship. The statement must include a statement of confidentiality and limits to it. The student is expected to discuss this statement for approval to the site supervisor. The template for the Professional Disclosure Statement will be provided. Journals Self-reflection is an important part of professional counselor development. To facilitate this process, students will thoughtfully write four journal entries on the topics listed below; the course schedule gives the due dates for each journal topic. Journals are expected to demonstrate higher order critical thinking skills and honest reflection with adequate depth. Students must complete the following journal prompts: 1) My strengths and areas for growth 2) An ethical dilemma I have faced 3) Multicultural Reflections: What have I learned about myself as I consider multicultural issues 4) One thing I wish I had known at the beginning of the semester OR- Something I did not expect during my Practicum experience Logs and Notebook The Notebook is an integral part of your practicum experience. As such, you will be expected to keep it up-to-date, and your LU supervisor will expect you to bring it to each scheduled meeting. Functionally, it will be kept in a large 3-ring binder, and it will contain (1) Agency Information, (2) Documentation Time Logs and Evaluations, (3) Non-Identifying Client Records, and (4) Documentation of. You will keep this notebook throughout your clinical experiences in the graduate program, documenting all of your internship work. The information in this notebook will be essential for you in your licensure process. It is critical for you to keep detailed and current records of your practicum experiences. Statement of Agreement I have read and understand the information contained in this document. I have been provided a copy of this document, and I agree to participate in supervision and my practicum or internship course according to these guidelines while under contract with my practicum/internship site for the dates of, 20 to, 20.
