My academic career has been focused on anesthetic care for pregnant women; I have authored many chapters and co-edited a textbook about anesthesia for obstetrics. I am the primary teacher of obstetrical anesthesia at UCSF; all anesthesia residents spend at least one month on my clinical service and I am responsible for their education in this important clinical subspecialty of anesthesiology. Fetal Treatment Center: I have been a member of the world-renowned UCSF fetal surgery enterprise since its inception, and a charter member of the UCSF Fetal Treatment Center. I pioneered methods to provide anesthesia for both mother and fetus undergoing these bold, complex procedures, demonstrating success in non-human, primate animal models before the UCSF team considered operating on pregnant women. I pioneered and provided anesthesia for the first human fetal surgical procedure to correct a simple, congenital anatomical fetal malformation that, if untreated, would have resulted in fetal or neonatal death. For the past 25 years, I have provided anesthesia for the vast majority of the hundreds of fetal procedures performed at UCSF for a broad diversity of fetal conditions. I have taught anesthesia for fetal procedures to faculty colleagues and residents at UCSF, as well as nationally and internationally through chapters in many textbooks, invited review articles, invited public lectures, clinical consultation and teaching visitors. The techniques I pioneered have become utilized worldwide. 3

4 Moffitt-Long Hospital Operating Room & Post-Anesthesia Care Unit (PACU): I provide clinical care and faculty supervision for anesthesia residents in the operating rooms and numerous other places in which anesthesia care is required for late running and emergency procedures during in-hospital, 24-hour clinical shifts about 1.5 days per month. During these shifts, I also supervise patient care in the PACU and provide emergent care for patients in the intensive care units and those who suffer cardiac or respiratory arrest ( code blue ). Mt. Zion Hospital Operating Room & Intensive Care Unit (ICU): I provide faculty supervision for anesthesia residents in the operating room, clinical care in the ICU and emergent care for patients who suffer cardiac or respiratory arrest ( code blue ') during in-hospital overnight shifts about 1-2 days per month. SUMMARY OF CLINICAL ACTIVITIES My clinical activities are focused on anesthetic care for pregnant women undergoing labor, vaginal or operative delivery, fetal surgery or other surgical procedures during pregnancy. I specialize in anesthetic care for high-risk pregnancies, pregnancies complicated by co-existing diseases and anesthetic care for fetal surgeries. I have been the primary teacher of obstetrical anesthesia at the University for more than a decade. I also provide clinical care in the operating rooms, post anesthesia care units, intensive care units numerous other places in which anesthesia care is required in Moffitt-Long and Mt. Zion hospitals. 4

21 SUMMARY OF SERVICE ACTIVITIES I have been engaged in many University and Public service activities throughout my career at UCSF. Currently I serve as a Medical Student Career Advisor and Chair the Academic Standards Committee for the School of Medicine. Medical Student Career Advisors assist medical students with choices of specialization and the applications process for residency training. The Academic Standards Committee assesses medical students whose academic progress falls short of established standards, reviews the details of their progress and problems, and explores potential confounding personal problems or other problems that can contribute to impairment of academic progress. The Committee meets with the referred student and makes recommendations to the Dean about whether to offer special assistance, special leniency, some interventional therapy, or whether to proceed to dismissal. I also serve on the Dean s Advisory Committee on Postdoctoral Education and have served on the admissions committee, ad hoc promotions committees and special grievance committees for the School of Medicine. At the Departmental level, I serve in a leadership role by directing the residency-training program, participating as a member of the Chairman s Executive Committee and recently having organized a faculty retreat, at which I led a major session to educate the Department s faculty about new teaching mandates for residency training from the Accreditation Council for Graduate Medical Education (ACGME), and now oversee a major re-engineering of our residency training program. As the residency director, I serve on numerous committees related to resident education and training. Serving in the primary role for the Department s resident selection and recruitment process alone occupies the majority of my time about two months per year. I have given freely of my time to teach at local Bay Area and Northern California hospitals, speaking at their Grand Rounds, participating in their quality assurance program reviews and serving as a consultant, both formally and informally. I also participate with the UCSF Perinatal Outreach program to provide education at facilities that refer their complicated patients to the University and I teach for the Great Expectations Program for Parents about four times a year. I have provided substantial professional service to the American Board of Anesthesiology in its examination process for certification since 1983, for both the written and oral examinations. I served as a question writer, member of the editorial board, and council member for the examination as well as an examiner for the oral examinations, which occupies one week per year. 21

This is an introductory course in anesthesia, focused on preoperative evaluation, intraoperative and postoperative management. Students spend the majority of the two weeks in the surgical operating rooms and spend one day in various anesthesia subspecialty venues. I teach the students who elect to spend their day in obstetrical anesthesia at Moffitt-Long Hospital (Children s Hospital of UCSF). Anesthesia Twelve-week Advanced Clerkship in general anesthesia for medical students who are in the last three months of the medical school phase of the integrated training program in Oral and Maxillofacial Surgery. During this three-month period, students become familiar with administration of general anesthesia for basic surgical procedures. Anesthesia Four-week, advanced elective in obstetrical anesthesia for medical students. The objectives for this course include learning physiologic and anatomic changes that take place during pregnancy, placental physiology and transfer of drugs, their effects on the fetus and newborn, newborn assessment and resuscitation, management of labor analgesia, cesarean section and management of high risk and emergency obstetric patients. 22

23 Anesthesia Didactic lectures in sciences basic to the specialty of anesthesia, as well as case reviews, clinical discussions, and seminars on current medical literature. These are conducted in small group seminars on a, nearly daily basis with residents on my clinical service. Anesthesia Direct supervision of residents in the management of clinical anesthesia care of patients in the labor and delivery suites (UCSF Birthing Center), operating rooms, recovery rooms and other locations in which anesthesia is provided (cardiac catheter lab, neuroradiology suite, interventional radiology suite, MRI and CT scanners, in vitro fertilization lab, etc.) Obstetrics and Gynecology Daily teaching rounds with residents in obstetrics, gynecology and reproductive sciences and anesthesia residents on their obstetrical anesthesia rotation. Weekly one-hour participation in perinatal morbidity and mortality conferences attended by faculty, fellows and residents in pediatrics (neonatalogy), obstetrics (perinatology) and anesthesia (obstetrics). ADDITIONAL TEACHING FORMAL TEACHING POSTGRADUATE AND OTHER COURSES I co-organize and direct an annual continuing medical education conference and teach at numerous other continuing education courses offered by the Department of Anesthesia and Perioperative Care, the Department of Obstetrics, Gynecology and Reproductive Sciences, as well as by invitation at many other national conferences. This has been an important avenue for my teaching and updating the practice of anesthesia for fetal surgery, obstetrics, and surgery during pregnancy, newborn resuscitation and many related topics. By teaching in venues that have large audiences, as well as by writing for textbooks, I fulfill an important personal commitment to teaching and widespread dissemination of information. INFORMAL TEACHING CONSULTANT ACTIVITY International, national and local consultation for clinical care -- I am frequently contacted for assistance by clinicians confronted with complex issues pertaining to anesthetic management of pregnant women for labor and delivery, surgery during pregnancy, fetal surgery or other fetal procedures. 26 FACULTY MENTORING A majority of the Anesthesia Department s faculty are former UCSF residents. For many, as their Program Director during residency training, I spent considerable time as a mentor, and it has been a natural evolution that I informally mentor and counsel many of the faculty members of our Department for professional, as well as personal, issues. (20 hours / year) RESIDENT MENTORING I spend a considerable amount of time mentoring residents. As the Residency Director, I have 72 trainees under my supervision, 24 residents per year for a three-year program and I mentor the majority of our residents for a variety of matters, including educational and academic assistance to help improve their overall academic achievement, career mentoring and/or assistance with securing employment after residency training, as well as personal development mentoring to support residents during times of personal and academic stress or pressure, or for guidance in decision making about career and life issues. I approach mentoring as a guide to residents through a productive thinking, learning, and problem-solving process. Our Residency has a well-established program of planned mentoring that occurs through a structured program in which mentors and participants are selected and matched through a formal process in which a formalized faculty mentor is identified for each resident to facilitate closer faculty-resident relationships and individual counsel for each resident. For the most part, my involvement with residents can be characterized as a more natural mentoring that occurs through friendship, collegiality, teaching, coaching, and counseling. My role as the Residency Director positions me as a contact for many private practices across the country, and therefore as an established and knowledgeable career advisor. Additionally, as the Residency Director I have counseled residents in academic difficulty, residents with attitude or behavioral problems, confronted residents with concern for substance abuse and I ve counseled residents with personal issues or problems ranging from pregnancy to dying, death of family member, marital discord, depression, angst about career choice, and for numerous other reasons. (80 hours / year) MEDICAL STUDENT MENTORING Each year, about a dozen UCSF senior medical students are interested in applying for anesthesia residency-training programs. Through the Department s Medical Student Anesthesia Interest Group, I meet with a group of these interested medical students twice per year to discuss a career in anesthesia and guide them about the application process. Further, I meet with most of the students individually to help them assess their qualifications as an applicant, assist them in preparing an application, and guide them in selecting suitable places for them to apply, based on their academic and professional achievements, goals as well as the many other personal factors involved in such a decision, such as location. I spend about an hour with each student. I directly assist most of the students plan and write their personal statements (I edit many of them), and help them select appropriate faculty for letters of reference. I also see many students from other medical schools that come to UCSF for student externships, counseling them about applications in a similar, but more abbreviated fashion. (20 hours / year) 26
