1 Medical Student Rotation Handbook Welcome to Henry Ford Department of Otolaryngology. We are excited to have you spend some time with our department over the next couple of weeks. This rotation offers you a chance to become immersed in the clinical and operative aspects of ENT. You will be exposed to every sub-specialty that ENT has to offer including Allergy, Endocrine Surgery, Facial Plastics, Head and Neck, Laryngology, MOHS Reconstruction, Neurotology, Rhinology, Pediatrics, and Sleep Medicine. Henry Ford Hospital Main Campus was opened in This tertiary care hospital has roughly 900 beds (about 200 ICU beds) and is a Level 1 trauma center. There are 3 other locations where our department has clinics and performs surgery. West Bloomfield location is a 200 bed in-patient hospital, Fairlane in Dearborn is an outpatient surgery center, and Lakeside in Sterling Heights is also an out-patient surgery center. (See Maps on last page) The hospital annually trains more than 500 residents and 125 fellows in 46 accredited programs. More than 400 medical students train at the hospital each academic year. One-third of all physicians in Michigan receive training at Henry Ford, and its post-graduate medical education enterprise is among the largest in the country. Research programs at Henry Ford Hospital have total annual funding exceeding $70 million. In 2009 alone, Henry Ford provided more than $173 million in uncompensated care. Henry Ford also is a major economic driver Michigan: the health system plans to invest $500 million to expand the Henry Ford Hospital in Detroit, and employs more than 23,000. Our program currently trains 2 residents a year. Many of our graduates go on to fellowship programs, though many also choose to enter a general otolaryngology practice. We are very proud of the experience we have to offer. Your Mission, Should You Choose to Accept It Work hard. The more you can help us, the more time it frees up for us to teach you things (e.g., letting you have time to sew, etc.). Learn and read as much as you can. Ask questions. No questions are stupid questions, but asking pertinent and well-informed questions helps you learn more (and makes you look good). Try to work with every attending in the department, whether that is in the clinic or operating room. If you have not achieved this by the last week of your rotation, talk to the chiefs and make them aware. 1

2 Learn and be competent at performing a full head and neck physical exam. Get to know the residents and evaluate whether this is a program that you would like to be a part of. Duties You will be given a schedule of your assignment for each day by the chiefs at the beginning of the week. If you don t know what site your assignment is at, please ask. Overall, we want you to be performing at the level of an intern by the 2 nd week of your rotation. The lecture schedule for the month is available from Margaret Calhoun, the program coordinator. Please obtain a copy the first day on rotation and plan on attending all didactic conferences. ROUNDING: Show up for morning rounds (ask the junior residents what time). This may fluctuate depending on the amount of primary patients and consults. We ask that you follow 1-2 patients per day. This patient should be someone in which you were in the operation or assigned to you by the chiefs. Preround on these patients and have a case presentation ready. Ask the staff or residents if you should also write a note. Follow along when the team is running the patient list and mark down what needs to be done with each patient. On rounds, have supplies ready for each patient (i.e., Kerlex, 4x4 gauzes, tongue blades, cotton tip applicators, etc.). If you know a patient needs a big dressing change, have the supplies ready prior to the rounds. Always carry a pen light. Always carry an otoscope, tongue blades, and penlight Learn how to clean the flexible scopes from MA s/juniors and clean all scopes that were used after rounds. OPERATING ROOM: If you re assigned to be in surgery, you are expected to know what cases will be performed. You are expected to read for each of those cases and know some associated background knowledge like anatomy, pathophysiology, etc. You will be asked to assist for most surgeries. If you don t know if you should scrub in, just ask. If you don t know how to scrub in, let us know and we can teach you. 2

3 CLINIC: If you re assigned to cover a clinic, head over after rounds are completed. Ask the attending what your role in clinic should be. Some attendings will have you shadow, some will have you see patients by yourself. Remember your white coat as well as to dress appropriately. SATELLITES: If you are scheduled to go to West Bloomfield, talk to the junior resident (or senior if there is no junior) the night prior and ask if there are any primaries or consults to see. If so, meet the junior early and help with the work. If at Fairlane or Lakeside, there is no pre-rounding; so just show up for clinic or OR. CALL: One weekday night of call per week, and 2 weekend calls over the course of your month is mandatory this will give you a chance to see what we do at night. Our program takes home call, so our resident will call you to meet them at the hospital. Both of you may stay overnight if things are busy. On the weekends you re on call, please help the junior resident pre-round, round, and finish floor work or see consults. Talk to the chiefs at the beginning of the rotation to pick the weekday and weekend call days. OTHER RESPONSIBILITIES: After finishing each regular day s activities, please contact the junior resident who is holding the pager and help them with any work that is left over and needs to be done. Often this involves seeing consults, rounding on patients, or finishing floor work. Attend the weekly didactic lectures with the rest of the resident team. These are held every Wednesday from 7-10am. These are usually held in the E & R building, room You are required to give a 20 minute Grand Rounds Lecture at some point during (or shortly after) your rotation. This can be of a topic of your choice, but if you have trouble picking one, talk to the chiefs or senior residents. Research There are many opportunities for research in our department. If you have a special area of interest, just contact that particular staff. Also talk to the residents, as many of them have multiple on-going projects. 3
