2 HEALTH SCREENING INFORMATION SHEET What is SingHealth StopDiabetes Health Screening? SingHealth StopDiabetes Health Screening helps you to find out if you have or are at risk of developing chronic diseases (diabetes, high blood pressure and high blood cholesterol). The aim of the screening is to encourage relatives of Type 2 Diabetes patients to undergo screening to assist with disease prevention and detection of the abovementioned diseases to enable early intervention. Why should I undergo the screening? If you have a parent, brother or sister, or child who has Type 2 Diabetes, your odds of having the disease are 2-6 times more likely than someone without such relatives. This could be because you share a similar lifestyle or environment, or genetics. Who is eligible for SingHealth StopDiabetes Health Screening? A parent, sibling or child of an individual with Type 2 Diabetes Mellitus, who: Is a Singaporean or Permanent Resident Is aged 30 years and above Has no known Diabetes Mellitus or Impaired Glucose Tolerance, and Has not undergone health screening for the past one year What will you be screened for? Conditions Screening Tests Obesity High Blood Pressure High Cholesterol Diabetes Lifestyle Height and Weight Body Mass Index (BMI) Calculation Blood Pressure (BP) Measurement Blood Tests Fasting Venous Glucose & Lipid (a) Oral Glucose Tolerance Test (OGTT) if applicable (b) Wellness Survey (c) (a) You will need to fast for at least 8 hours prior to the blood test. There will be a blood taking procedure involved. (b) If your glucose test result is at risk / abnormal, the GP will advise you to take a second FBG / OGTT. The GP will advise you on what to expect for the second FBG or OGTT. (c) This survey will take approximately 10 minutes. The survey result will help the doctor determine your risk based on your lifestyle. 1

3 Where and when will SingHealth StopDiabetes Health Screening be held? There is no scheduled screening time and date. You can have your screening done at any one of the selected General Practitioner ("GP") clinics, located island-wide. Where can I find information on the selected GP? Do I have to make an appointment before I go for the screening? You will need to register for the screening before you are assigned a GP that is located at a location convenient for you. There is no need to make a prior appointment. However, it is advisable to give your allocated clinic a call before dropping by to ensure that the clinic is open and the doctor is on duty. How do I register? You will need to call the SingHealth StopDiabetes officer at or (Mondays to Fridays 9:30am to 5:00pm) or stopdiabetes@singhealth.com.sg. The officer will address any of your concerns or queries regarding this information sheet and consent form. These forms can be found inside the StopDiabetes Registration Pack. After speaking to or ing the SingHealth StopDiabetes officer, you will complete and sign the consent form, registration form and wellness survey before visiting the allocated GP. On the screening day, you must bring these completed forms and the lab test form found inside the envelope to the allocated GP. How much does the screening cost? No payment is required for the screening session, but review of your screening result may incur charges. What do I need to do before the screening day? You need to fast for at least 8 hours prior to the screening day. What can I expect on the screening day? At the GP clinic, the doctor or clinic assistant will verify that you are aware of and accept the SingHealth StopDiabetes Health Screening programme s information and terms. If you have further queries, please speak to the doctor before you sign the relevant forms and proceed with the screening. Once your queries have all been addressed, the doctor or clinic assistant will collect the signed and completed registration form, consent form and lab test form from you. You can retain the brochures for your future reference. While waiting for your turn to undergo the health screening, you will be asked to complete a wellness survey (if you have not completed it prior to visit). Once all the necessary documents have been signed, completed and returned to the clinic assistant, you will be ushered into the doctor s room for screening. The screening consists of: Weight and height measurement Blood pressure measurement Blood tests The entire screening should not take more than 1 hour (not inclusive of waiting time). 2

4 Where and when do I collect the screening results? You can collect your screening results around 2 weeks later at the GP clinic where you underwent the screening. The result will be retained at the GP clinic for a maximum of 3 months from the day of your screening, after which the result will be destroyed. If you wish to collect after the 3 months period, kindly inform the GP to work out a suitable result collection period. In the event that you wish to collect your result after the 3 months period and your result has been destroyed, you may request for a reprint of the result from the GP, which will take 3 working days for it to be ready. You will also be charged for the administrative and processing fees involved in reprinting your result. What happens after I receive my results? If your result is: Normal (for fasting blood glucose and lipids test) At risk / abnormal (for BP, lipids and/or BMI) What will happen You can leave after collecting the result over the counter. However, if you wish to have the GP review and interpret your screening result, you may request for a minutes consultation, which will be chargeable at $28. You will be asked to return to the GP clinic for result collection and a 10 minutes review and interpretation of your result. The GP may recommend follow up actions and/or an intervention programme depending on your screening result. You will also be issued a referral letter for your further follow up, if necessary. You may also request for a more comprehensive consultation, which will be chargeable at the GP clinic s consultation rate. At risk (for glucose test) You will be asked to return to the GP clinic for a review and interpretation of your result as well as an Oral Glucose Tolerance Test (OGTT). The GP will explain the OGTT guidelines and procedures as well as address any of your concerns before proceeding to do an OGTT, if you are agreeable. You will also be issued a referral letter for your further follow up and/or recommendation of intervention programmes, if necessary. The entire review and procedure should not take more than 10 minutes, but if you wish to have further clarifications from the GP, you may request for an extensive consultation chargeable at the GP clinic s consultation rate. Abnormal (for glucose test) You will be asked to return to the GP clinic for a review and interpretation of your result as well as a second fasting blood glucose test (FBG). Depending on your second FBG result, you may or may not need to do a third test, which is the Oral Glucose Tolerance Test (OGTT). The GP will explain the OGTT guidelines and procedures as well as address any of your concerns before proceeding to do an OGTT (when necessary), if you are agreeable. You will also 3

5 be issued a referral letter for your further follow up and/or recommendation of intervention programmes, if necessary. The entire review and procedure should not take more than 10 minutes, but if you wish to have further clarifications from the GP, you may request for an extensive consultation chargeable at the GP clinic s consultation rate. Negative (for OGTT) You can leave after collecting the result over the counter. However, if you wish to have the GP review and interpret your screening result, you may request for a minutes consultation, which will be chargeable at $28. Positive (for OGTT) The GP will call you to inform you of the need to seek medical follow-up and / or intervention programmes. You may be asked to return to the GP clinic for consultation, which will be chargeable at the GP clinic s consultation rate. If you wish to seek follow-up and/or intervention programmes somewhere other than the GP that you had done the screening with, you may collect your OGTT result and bring it to your preferred doctor. Any other screening tests, consultations, and/or intervention programmes recommended by GP for further investigation or follow up will be payable by participants at the GP clinic s rate or the service provider s rate. In addition to that, depending on your screening results, the StopDiabetes officer may contact you and refer you to any of our Partners 1 for appropriate follow-up to manage your medical condition(s). Follow-up may include medical treatment and lifestyle interventions programmes such as invitations to counselling sessions, lifestyle activities and disease management workshops. The decision to take up these recommendations is entirely yours. If you decide to go for the recommended follow-up, we would like to highlight the following: 1. You must ensure that you are fit and able to participate and comply with the Physical Activity Readiness Questionnaire (PAR-Q) requirements for physical activities. 2. You are required to bear additional costs such as medical consultation fees and/or material costs and administrative fees which are essential for the follow-up. 3. Please update the SingHealth StopDiabetes officer at or or stopdiabetes@singhealth.com.sg of any changes in your contact details and health condition. 1 Refers to healthcare providers (GPs, specialists, nurses and allied health professionals) from private and public corporations, nurse educators, Health Promotion Board (HPB), government constituency managers, volunteers and community partners which work with SingHealth Institutions on the post-screening follow-up. 4

6 How can I withdraw from this screening programme? You will need to write to the SingHealth StopDiabetes officer officially via to stopdiabetes@singhealth.com.sg to express your intention to drop out from this screening. You will receive an acknowledgement to confirm that you are withdrawn from the screening programme. Alternatively, you may send an official written request to withdraw from this programme to the below address: Attn: StopDiabetes Officer Clinical Networks, Regional Health System, Singapore Health Services Pte Ltd, National Heart Centre Singapore, 5 Hospital Drive, #10-SHS-04 Singapore You will receive an acknowledgement letter to confirm that you are withdrawn from the screening programme. 5

7 Collection, Use and Disclosure of Personal Data Your personal, screening and follow-up data ("Information") will be collected and used by SingHealth and its related institutions (collectively, SingHealth Institutions ), and the organisations listed below (collectively, Parties ): (i) Ministry of Health (ii) Third party laboratory service provider engaged by SingHealth (iii) People s Association (iv) Partners as described above (v) HPB Collaborators 2 (vi) Any other third parties authorised by you SingHealth Institutions and the Parties will use the Information for the purposes of administering and conducting the SingHealth StopDiabetes Health Screening, as well as managing and implementing follow-up action arising from the SingHealth StopDiabetes Health Screening. Your Information will be made available to all public healthcare institutions and may also be aggregated / de-identified for future research, health services studies, statistical and planning purposes. SingHealth Institutions may request to obtain your Information from the Parties for the purposes of patient care, treatment and/or clinical/programme review. Likewise, SingHealth Institutions may release your Information to the Parties for the purposes of evaluating if you require rescreening, further tests, follow-up and/or programme review. By signing the consent form, you also confirm that you have read, understood and consented to the SingHealth Data Protection Policy, the full version of which is available at Hard copies are also available on request. Screening tests results are for general information purposes Please note that the results of the screening are for general information purposes and are provided on the understanding that no dental, surgical or medical advice or recommendation is being rendered. Medical treatment has to be individualised and can only be rendered after adequate assessment of your condition through appropriate clinical examination. Please do not disregard the professional medical advice of your physician or local healthcare provider or delay in seeking medical advice from them because of any information herein. For the reasons given above, you should not solely rely on the information herein or any test results obtained from the screening conducted, and to the fullest extent permitted by law, we do not accept any responsibility if you do. They can be polyclinics, hospitals, private GP and other clinics, Family Medicine Centres (FMCs), Community Health Centres (CHCs), private laboratories, screening, reading and assessment centres, other healthcare institutions and providers, voluntary welfare organisations, academic institutions, statutory boards, related agencies and community partners. 6

8 GENERAL TERMS & CONDITIONS 1. SingHealth, participating GP clinics ( DOT GPs ) and all Partners may jointly or individually conduct the screening, workshops and programmes of the SingHealth StopDiabetes Health Screening Programme. 2. Registration is not confirmed until: i) The SingHealth StopDiabetes Health Screening Programme Officer receives a telephone call or from interested StopDiabetes Health Screening Programme participant indicating his or her interest to participate in the screening and the participant consents to being registered by the Officer in SingHealth s database; ii) Participant undergoes programme counselling (either over the phone or via ) with the StopDiabetes Health Screening Programme Officer, and agrees to all terms and conditions stated on the consent form; and iii) Participant gets assigned a DOT GP clinic and consents to being contacted by the StopDiabetes Health Screening Programme Officer for details of the assigned DOT GP clinic. 3. Participants will not be allowed to undergo the screening before they submit the duly completed and signed registration form, consent form and wellness survey to the DOT GP. 4. The SingHealth Institutions reserve the right to change, cancel or reschedule any intervention programme workshop / activity at its discretion. In such event, it shall not entitle the participant to any compensation or damages from SingHealth Institutions. Where possible, prior notice may be given to participants of the changes. 5. The SingHealth Institutions shall not entertain requests for change of date or time of a specific intervention programme workshop / activity session. 6. To the fullest extent allowed by law, the SingHealth Institutions shall not be liable for any loss, damage or injury of any nature whatsoever and however caused (including through negligence) arising from or in connection with the SingHealth StopDiabetes Health Screening programme. 7. SingHealth reserves the rights to change the terms and conditions without prior notice, which shall be deemed effective immediately upon such change and binding between the participant and SingHealth. 7
