Passage to Excellence Skills Review: River East Personal Care Home The staff at River East Personal Care Home took part in a 3-day Annual Skills Review on June 21-23, The annual review of the 6 skills is mandatory for all staff of this East Kildonan Personal Care Home. Lisa Martens, DOC started this program 5 years ago in an attempt to assure that all staff has carried out their annual review and that there was a record of such maintained on their individual file. Each staff member is issued a passport which outlines the 6 skills required to be completed. There is space on the passport for the attending management staff to validate that the skill has been successfully demonstrated. The 6 Skills Booths were as follows: PHIA Personal Health Information Act staff must review posted information and answer a series of questions to ensure that the Act & how it applies to them is understood. They also re-sign their pledge of confidentiality. Body Mechanics staff must satisfactorily demonstrate good body mechanics while using a slider or lifting an object. Code Green Evacuation Technique The Sheet Slide staff must satisfactorily demonstrate the sheet slide technique for removal of bedridden residents in a disaster scenario. The use of new door identifiers was demonstrated. A plastic flag has been installed on each door so that it can be turned to signify that the room has been checked during either a fire or disaster. Code Red Review staff must satisfactorily demonstrate knowledge of the procedures to be followed in case of a fire using the acronym REACT. Hand Washing staff must satisfactorily demonstrate a 10 second medical hand wash. There was an ultra violet light for staff to hold their hands under to assure that they had been properly cleansed. Restraints staff must identify 3 alternatives to employing the use of a restraint. They then had to demonstrate the proper procedure in applying a restraint if the alternatives were unsuccessful. When all of the skills have been completed the passport is given to the Staff Development coordinator and is recorded on their individual file. From the completed passports a draw for various prizes was held. The staff appeared to be having a lot of fun with this and were eager to complete the challenge. What a great way to assure that all your staff maintains their skills! Congratulations Staff and Management of River East Personal Care Home! 2
CNS Palliative Care Program presented at Provincial Health Conference (From left to right) Deanne O Rourke, CNS PCH Program, Kathleen Klaasen, CNS PCH Program and Suzanne Wowchuk, CNS Palliative Care Sub-program, with their poster presentation titled Enhancing Clinical Capacity and Knowledge in Winnipeg Personal Care Homes, at the 2005 Provincial Health Conference. The CNS Clinical Corner Question: One of the residents in my facility had a Hollister HeelBoot. When the resident died, the family donated the Hollister HeelBoot to our facility. Can we use the HeelBoot for another resident? Answer: Absolutely. However, you need to keep in mind the following: The liner of the Hollister HeelBoot needs to be replaced between residents. Replacement liners are available at Diamond Athletic Medical Supplies (1387 Grant Avenue), Medimart Pharmacy (756 Pembina Highway), PharmaPlus (233 Kennedy Street), Tache Pharmacy (400 Tache Avenue). The shell of the Hollister Heel Boot needs to be cleaned with Hydrox or a similar disinfectant (the same solution that you use to clean down bedside tables, railings, etc.). Ask your Occupational Therapist to ensure the Hollister HeelBoot is the appropriate size for the next resident. Regular Heel Boots are sized for residents with less than 16 inch mid-calf circumference. Large HeelBoot are sized for residents with 16 inch or greater mid-calf circumference. For other types of heel offloading devices, please check with your Occupational Therapist and/or the vendor to ensure the devices are appropriate for multiperson use. If you have any other questions you wish to see answered in this column, please them to either Deanne O Rourke at dorourke@wrha.mb.ca or Kathleen Klaasen, CNS, at kklaasen@wrha.mb.ca 3
IS IT A COLD or IS IT INFLUENZA? GET THE SHOT - NOT THE FLU! Did you know that getting immunized is an important resident safety issue? Did you know healthcare workers can spread influenza to residents, possibly leading to influenza-related illness and death; and did you know healthcare workers could also spread influenza to their co-workers? Did you know when healthcare workers get sick with the flu; the result is absenteeism and disruption of care? Did you know that last year reported immunization rates for the WRHA Personal Care Home staff ranged from 20.7% to 89% with an overall average of 42.2%? Did you know as a healthcare worker getting the flu shot protects the resident, families, your co-workers and you. Did you know as a healthcare worker getting the flu shot helps prevent illness and death. And did you know as a healthcare worker getting the flu shot decreases sick leave. HELP PROTECT THE RESIDENTS AND OTHERS. GET YOUR FLU VACCINE, AND ENCOURAGE YOUR CO-WORKERS TO DO THE SAME. Reference: 4
WRHA Infection Prevention & Control Routine Practice Project Update The purpose of this project is to provide education on Infection Prevention and Control Routine Practices and precautions to all health care providers employed in the region. This education is essential to satisfy accreditation requirements and is in accordance with WRHA Routine Practice Guidelines and Policies. Although Personal Care Homes have implemented an Infection Prevention and Control Routine Practices policy/procedure and have incorporated it into their practice, this project will give sites an opportunity to ensure direct and indirect health care providers are current in Routine Practices. Training in our sites will resemble a refresher and return demonstration to verify skill. We plan to start an Educational Blitz in November to provide Routine Practices training to the majority of staff. Initial sessions will be made available in all sites. In addition, Train-the-Trainer sessions will be offered to designated PCH staff. We will be starting to contact your designated Site Leads to make arrangements for this education. For this education to be Personal Care Home specific, we have developed an Infection Prevention and Control Routine Practices Self Learning Package and video for the Personal Care Home Program. We would like to extend a special thank you to Bethania Personal Care Home and their staff for participating in the development of the Routine Practices video. To support the success of the Routine Practices each PCH will have a designated Site Lead. This individual will be the main contact during the course of the project, coordinate the local RP educational sessions, forward staff names for Train-the-Trainer sessions, and provide evaluation information to the RP project team as requested. All sites should have provided the name of your Site lead. If you have not already done so, please provide the name of your designated Site Lead to Betty Taylor at or to btaylor@wrha.mb.ca If you have any questions about this project, please contact either Betty Taylor at or Marianne Woods at
Technological Advancements in the Personal Care Home Program continues with the use of the RAI/MDS 2.0. We are the children of a technological age. We have found streamlined ways of doing much of our routine work. Printing is no longer the only way of reproducing books. Reading them, however, has not changed... Author: Lawrence Clark Powell These are wise words, and not so unlike the words that should be chosen to describe the Regional efforts to bring computerized resident information into everyday life. As care providers, we try very hard to be sure of our judgments and decisions when developing care plans and interventions to meet the individual residents needs in our Personal Care Homes. Ongoing education and training is necessary to keep up to the ever-challenging demands of today s health needs. Adopting technological approaches for the assessment of Resident needs has been a Regional, Provincial and National focus in Long Term Care. The Resident Assessment Instrument (RAI), which includes the MDS assessment, has been a great leap into technology for the Personal Care Home Program. This assessment tool has been introduced to act as a support for evidenced based decision-making. However, we remain very aware that our mandate to care for the residents has not changed. The resident remains our priority. The Region s Non-proprietary Personal Care Homes had sent sixty seven of their staff for training in the RAI process between the months of April and November in These staff went through training that involved computer education, assessment training and software program training. Those sixty-seven persons, in turn, have trained well over 800 interdisciplinary team members throughout the PCH program. This effort will benefit future projects that have demands for technological skills. Computer competencies have sharply increased and the foundation is set for any other systems that may require personal care home staff to have this knowledge. At this time the data that has been entered into the RAI/MDS 2.0 is being used by the PCH sites to make decisions about resident care for over 5300 persons in the region. Care Plans are being designed to meet specific, individual care needs based on consistent and team-based information collected by the MDS. Sites now have measurable data that allows for evidenced based quality care and interventions. Anyone wishing to know more about the RAI/MDS 2.0 is welcome to contact Caroline Boyd, RAI Coordinator for the PCH Program, or cwboyd@wrha.mb.ca We want your stories! Do you have news stories, resident profiles or successes in the services you deliver? There are many great things happening in Personal Care Homes in Winnipeg, and we want to share stories from PCH s with others through the PCH View. If you have questions about submitting articles, please contact Leanne Drad at or ldrad@wrha.mb.ca. 6
