2 What is the CCG s Purpose Lewisham CCG is responsible for commissioning planning, monitoring and buying most of the health services you need in Lewisham. It has an annual budget of about 390million and covers: Hospital Care Rehabilitation Care Urgent and Emergency Care Most Community Health Services Mental Health and Learning Disabilities Services Lewisham Council provides social care we work together to integrate the commissioning of local health and social care services provided at a neighbourhood level. NHS England is responsible for Primary Care Commissioning GPs, pharmacists, dentists and opticians - specialised services; the CCG jointly commissioning GP services (Level 2) 4

4 Our Challenges in Lewisham Too many people die early from deaths that could have been prevented by healthier lifestyle - smoking, alcohol, diet and exercise: 21% of Lewisham population smoke, which is more than the national average About a third of adults in the borough are overweight or obese Over 25% of reception age and 37% of Year 6 children are overweight or obese Alcohol related harm is significant and increasing in Lewisham There are significant health inequalities in Lewisham: People living in the most deprived wards, in Lewisham, have poorer health outcomes and lower life expectancy compared to England s average. Health inequalities can be considered by ethnic group. For Example the Department of Health has highlighted ethnicity as the major inequality in Severe Mental Illness. Black residents are disproportionately over-represented in mental health admissions. 8
5 Our Challenges in Lewisham Too many people live with preventable ill health: The likelihood of having a long term condition, including dementia increases with age; over 50% of those aged over 75 are likely to have two or more long term conditions. 70% of the health service budget is spent on supporting and caring for people with long term conditions Demand for care is increasing, both in numbers and complexity: Older people are higher users of health and social care services Our residents aged over 65 have high rates of emergency admissions to hospital for pneumonia; urinary infections and COPD High quality care is not consistently available all the time - too often, the quality of care that patients receive and the outcome of their treatment depends on when and where they access health and care 8
6 Our Challenges in Lewisham The money we have to deliver services is nor keeping pace with demand and we have big financial savings to make. If NHS Lewisham CCG continues to commission in the same way as today it will result in the CCG facing a funding gap between projected spending requirements and resources available of around 10 million in 2016/17. This means we have to use the resources we have in the very best way possible. We must prioritise what we do to improve the performance of health and care services and meet the above challenges. 9

8 Partnership Commissioning Intentions - priorities Prevention and early intervention : To encourage people to live well, stay healthy and independent longer To connect people to services and communities across the borough to promote physical and mental wellbeing GP practices and primary care: To provide strong GP practices and primary care focused on delivering continuity of care which is proactive, co-ordinated and accessible to deliver improved outcomes 11
9 Partnership Commissioning Intentions - priorities Neighbourhood community care for adults provide co-ordinated multidisciplinary support and care for people with long term physical and/or mental health conditions and vulnerable people, with their carers, families and communities to effectively manage their own care, where possible, and maintain their independence Enhanced care and support for adults - develop a coherent and co-ordinated set of services which avoid unnecessary admissions into a hospital or care home and facilitate early discharge into the community /home.. Children and young people s care provide integrated pathways that provide high quality support with choice and control for children, young people and their families which meet their needs and deliver care at the right time and in the right place. 12
10 Partnership Commissioning Intentions commissioning differently Partnership approach with the Public we are committed to: engaging Lewisham people - to understand why change is necessary with effective public communication and engagement involving people earlier to identify options and solutions; working together to redesign the way services are delivered working more closely with Lewisham residents - to develop stronger relationships with all Lewisham people, community groups and voluntary organisations, to connect with them in a more meaningful way individual participation to enable individuals to have the knowledge, skills and confidence to mange their own health; to be involved in all decisions about their care no decision about me, without me 12
11 Partnership Commissioning Intentions commissioning differently Partnership approach with Providers - we are committed to: working in partnership with a broad range of statutory, voluntary and independent sector providers to tackle the way historically heath and care has been provided in a fragmented and disjointed way. changing the way we commission and contract for services for 2016/17 and establish a different way of working with providers in the future, shifting to outcomes and population based models of commissioning. 12
