3 What is an STP? Plan for health service (clinical, financial & operational) sustainability for next 5 years & beyond Looks at health and care recognizing the NHS does not operate alone 44 footprints across England, of which HIOW is one Based on real intelligence about population health needs at a local level Brings together health and care organisations to agree priorities - in HIOW, 16 NHS bodies + 4 top tier councils Will act as a guide for future commissioning & service transformation priorities

4 What it the HIOW STP Vision?

5 Place based systems of integrated care the bedrock of our plan 5 Our local place based services in Southampton, Isle of Wight, Portsmouth and in natural communities in Hampshire are the bedrock of our plan, each one brings together primary, community, social, mental health, and voluntary sector services into a multi-disciplinary team providing extended access and simplified care for the local population. We are delivering this new model through three vanguard programmes and through transformation programmes in Portsmouth & Southampton City, as illustrated below: These programmes will deliver place based integrated care through consolidated single points of access and sustainable primary care in each locality in HIOW, with 5 big ticket interventions consistently implemented: Foundation for independence & self care Fully Integrated Primary Care Integrated Intermediate Care We will deploy an econsult platform for primary care supporting self-care and channelling people to the optimal care settings. We are also introducing care navigators & social prescribing: shifting current primary care activity to a nonclinical workforce Primary care working at scale to deliver urgent care across 7 days. Joined up, enhanced multiprofessional primary care teams with extended skills and extended access care hubs in localities Integrated health and social care including: domiciliary recovery and rehab teams, non-acute beds, urgent community response, Emergency Department liaison. Complex & End of Life Care LTCs: Diabetes & Respiratory Dedicated support from the multi professional team for those patients at greatest risk, including the 0.5% of patients with the most complex needs and those at end of life. More specialist cases managed in primary care setting, specialist roles as a core part of the local primary care team, and consultants working to support shared management of cases with GPs without the need for formal referral.

6 Clinical Service Strategy We continue to improve service quality today, but we need to keep a view as to how SHFT works with the STP to strengthen future care delivery The CSS is a four-month project to: Identify how services can be delivered to best meet the needs of our service users and communities in the future, Consider whether current organisational arrangements need to change to support the delivery of these services We ve engaged Deloitte LLP to support us, working in partnership with experienced clinicians from Northumberland Tyne & Wear NHS Foundation Trust

7 Trust Plan Annual document setting Trust s priorities to improve care outcomes and how services will be delivered within quality, operational & financial standards This year is different: NHS Planning Guidance issued 3 months earlier than usual Deadline to conclude 2-year contracts and operating plans by Christmas Expectation contracts will align with STP visions The Plan affects everyone patients and service users, SHFT staff, commissioners and wider stakeholders. A co-produced plan will be a better plan - we are committed to making it more accessible
