You can apply for an innovative program grant to receive up to $40,000 for programs and healthcare solutions that directly impact the patients and communities we serve! Help make a difference in the way we serve our patients and communities throughout Einstein Healthcare Network. Every member of the Einstein family physicians, nurses, clinicians and even those in nonclinical roles like administrative or facilities support is invited to apply for an Albert Einstein Society Innovative Program grant for FY These funds help to implement new, groundbreaking programs that directly impact you, your ability to care for your patients and our community. Last year, the Innovative Program Allocation Committee awarded 13 grants with applications ranging in request amounts from $12,000 to $40,000. Formal oral presentations, along with a question and answer period, will also be scored by the committee. We urge you to adhere closely to the guidelines of this application. Proposals must demonstrate a connection between the activities for which it seeks funding and both Einstein Healthcare Network (mission and pillar goals) and Albert Einstein Society goals and objectives. Your proposal must be innovative (defined as to begin or introduce something new; to effect or make changes; to do something in a new or creative way, characterizing a new device, idea or method ). Although your program is innovative, it should not be research - it should not require IRB approval. It can answer a question such as will this project achieve its objective? The application must include all required signatures. The application should be ed in word (no PDF s) to moskowic@einstein.edu, and the original signature page delivered or faxed if not scanned into the grant request for proposal to the Development Department by Wednesday, January 24, 2018 at 3PM there will be no exceptions. Projects receiving AES grant funding are considered the sole property of Einstein Healthcare Network. Programs identified for commercialization, are then outside the scope of the AES process and should be referred to the Office of Research and Technology Development to identify sources of venture capital. FUNDING OBLIGATIONS Investigators of funded proposals are required to submit an annual status report.

Written Proposal Format It is required you fill out the application electronically (only word documents will be accepted, no PDF s), using stated section headers. The grant must be typed in Times New Roman - 12 point font with 1 margins. Your application must be submitted by in the existing electronic format by 3 PM on January 24, 2018 to moskowic@einstein.edu. The original signature page must be delivered to Caren Moskowitz in the Development Department in person, faxed or a scanned version by within the same time period. B. Grant Approval Announcements: Innovative Program grants will be announced in May, PRESENTATION The second step of the application process requires you make a formal presentation to the Innovative Program Allocation Committee. Your presentation must touch on all aspects of your written proposal. Your presentation will be timed and may not exceed 10 minutes. All presentations can be supported by Powerpoint or other visual effects. A maximum of 20 slides are permitted for the 10 minute presentation. We encourage you to relay your message clearly while stimulating interest in and portraying the human/emotional aspect of your project. Your message can also serve to explain the evidence-base in support of your project idea. Presentations will be followed by a 10-minute question and answer period. Your assigned date and time will be ed to you in February. If you are unable to appear at your assigned time, it is your responsibility to find someone else to present on your behalf. 5 BRIEF DESCRIPTION - ABSTRACT PAGE (1 Page Maximum 12 point font) The abstract is structured with the following information, under the headings indicate: Brief Introduction (paragraph summarizing grant intent no more than 100 words) Rationale (primary need / reason being addressed) / Innovation (what is new) / Plan (how is the need to be addressed) / Sustainability (ensuring project success now and in the future including on-going funding support). Also include how your project supports and upholds the mission of one of the Pillar Goals.

6 Innovative Program Application & Format Your proposal must be submitted with these headers, in this specific order SECTION I PERSONAL STATEMENT and DEPARTMENT BACKGROUND (MAXIMUM 1 PAGE) Part A Personal Statement: Describe the team member s qualifications and what each of their role is in the project. How will your team work together to achieve this grant? Part B Department Background: Describe your department and how your program fits into its goals and objectives. SECTION II PROPOSAL NARRATIVE (MAXIMUM 3 PAGES) Part A Statement of Need and Rationale: Explain the need and basis for the program, while also describing the targeted population and how the project meets the mission of Einstein and the Albert Einstein Society. How will you engage the population you choose for your program? There should be no doubt of the importance of the project to Einstein and its communities. Part B Innovation: Describe how the proposed activities and services in this project are innovative. The innovation may be new anywhere or just new to Einstein (if done elsewhere describe whether this is local, regional or national). It may be a known process applied in an innovative way to a new population (if so, describe what is new about the population). It may be a new process for achieving objectives in an innovative way (if so what makes the process new and worthwhile over the old process). Part C Objective / AIM: There should be at least one objective / AIM of the project clearly stated, and tied direclty to the project need and rationale. Each should be stated in SMART terminology (Specific, Measurable, Attainable, Relevant, Time-bound). Also, describe the outcome that is expected to address the need of the specific target population likely to benefit from the project. Part D Plan: Outline the project plan, the strategy and activities being used to achieve the Objective / Aim. Specific activities and implementation steps must be placed in a timeline. Your plan should also include the staff needed, the training required, the materials used and clearly state how each will be utilized. Part E Measures: Describe the project s evaluation plan. For each objective / AIM name the indicators of success you will use to track your progress and explain how you will track it. The process measure should assess whether or not one or more key parts of the plan are being implemented as intended. The outcome evaluation should determine whether the aim of the project is met. Each measure should be clearly defined, how often, and by whom. How are you ensuring accuracy, and what analysis is to be applied to determine the measure of success. Part F Pillar Goals: Explain how your project fulfills the mission of one of the Pillar Goals. Describe which goal it is and how it will help Einstein reach this goal.

7 Part F Sustainability: Should the grant be approved, the first year s budget is covered by the grant. Describe what will support the project financially in future years should its aim be achieved and future funding is needed. Also explain how personnel issues in the future will be addressed and who will continue the project into the future. Describe any prior grant successes or other accomplishments of the project / team leader that increase the likelihood of success. If this is a brand new kind of venture for this team, where would the team turn for support if challenges to completing the project present themselves? SECTION III BUDGET (MAXIMUM 1 PAGE) Part A Budget Narrative: Please provide a justification for your costs on the budget page below. The budget must be cost-effective and appropriate to the scope and nature of the project. In the Budget Narrative, your justification should include three elements: Amount Reason why each expense is needed How you calculated each expense (remember to include justification for all categories including staff salaries, consultants, materials and supplies, training, marketing, equipment, and any other direct costs (IPAC will not fund travel for conference presentations) SECTION IV For previous AES grant recipients only Please list any previous AES grants that have been awarded to members of your grant team. Name of grant, member involved in the grant, date of award and current status of grant must be included. If the grant is still in existence, how is it being financially supported, and how has the grant evolved from its original intent? If the grant is no longer in existence, why was it discontinued?

9 MISSION STATEMENT The mission of Albert Einstein Society is to provide a Fellowship of community and medical leaders whose shared commitment to Einstein Healthcare Network and the health requirements of humanity would impel them to respond to those needs in a meaningful manner. The Society s Fellows will provide support through heightened understanding of the healthcare concept, and enable Einstein staff to carry through a series of projects responsive to Einstein s charter to care for those in need, without regard to race, creed or color, or ability to pay. The Society supports original research projects that allow for the exploration of new ideas in the laboratory, hospital and community. It also supports innovative programs that directly enhance patient care. It is anticipated that many will result in benefits that will be far reaching.
