Second round of 18 grantees began their work in December A total of 30 organizations have been funding to address the social and economic factors that drive health

The RWJF Roadmaps to Health Prize is rooted in the idea that every community is on a unique journey toward better health and that communities stories of success are as diverse as their populations. The prize is an opportunity for RWJF, UWPHI, and the nation to honor and promote these successes

6 6 WHO IS ELIGIBLE? Towns Cities Counties Tribes and Tribal Communities Regions (contiguous towns, cities, counties) PRIZE CRITERION #1 HARNESSING THE COLLECTIVE POWER OF LEADERS, PARTNERS, AND COMMUNITY MEMBERS by listening to diverse voices, inspiring each other and developing strategies for buy in, decision making, and coordinated action among groups (including employers, community advocates, health care and public health professionals, government officials, grantmakers, policymakers, educators, and others)

7 7 PRIZE CRITERION #2 IMPLEMENTING A STRATEGIC APPROACH TO IMPROVING HEALTH THAT FOCUSES ON THE MULTIPLE FACTORS THAT INFLUENCE HEALTH including health behaviors, clinical care, social and economic factors, and the physical environment as illustrated by the County Health Rankings model. PRIZE CRITERION #3 ADDRESSING PROBLEMS THAT DISPROPORTIONATELY AFFECT VULNERABLE POPULATIONS such as ethnic minorities and those with limited English skills, income, and/or education, and creating opportunities for all members of the community to make choices allowing them to live a long, healthy life PRIZE CRITERION #4 DEVELOPING SUSTAINABLE, LONG TERM SOLUTIONS TO SHARED COMMUNITY PRIORITIES including planning and implementing policy, systems, and environmental changes that target populations rather than individuals. PRIZE CRITERION #5 SECURING AND MAKING THE MOST OF AVAILABLE RESOURCES including innovative strategies to make the most of financial resources and individual and organizational experience and expertise dedicated to programs and policies that are improving the health of the community

8 8 PRIZE CRITERION #6 MEASURING AND SHARING RESULTS including setting achievable goals and engaging in thoughtful and regular monitoring and public reporting of strategies to improve health as well as the impact those strategies are having on health measures. HOW DO WE APPLY? APPLY VIA THE MYRWJF.ORG PORTAL THREE PHASE APPLICATION PROCESS 1. Phase I Application (all U.S. communities invited) 2. Phase II (select communities invited) 3. Site Visits (up to 12 finalists invited) If you have not already done so, you will be required to register

9 9 PHASE I PRIZE ESSAY: COMMUNITY INTRODUCTION Key physical, social, and economic characteristics Important historical and cultural considerations Vital assets and pressing needs Genesis of community health improvement activities PHASE I PRIZE ESSAY: DESCRIPTION OF FOUR ACCOMPLISHMENTS DO showcase four accomplishments that best reflect the prize criteria. DO include specific policies, programs, and/or strategies that your community has implemented DO consider new initiatives as well as those that are wellestablished and have had sustained impact over time. DO NOT identify coalition development, health assessments, improvement plans, or similar activities as one of your four main accomplishments (please mention these where appropriate) PHASE I PRIZE ESSAY: DESCRIPTION OF FOUR ACCOMPLISHMENTS What event(s) or circumstance(s) prompted this community action? What is your overall goal? What are the key activities that contributed to this accomplishment? Who benefits from this effort, e.g., the entire community or specific groups? If the effort targets specific groups, please describe who they are. What individual and organizational contributions are noteworthy (in terms of leadership, partnership, funding, etc.)? What impact have you seen as a result of this accomplishment? How are you measuring success? PHASE II APPLICATION: GETTING THE WHOLE PICTURE Invitation extended to select Phase I applicant communities Additional essay questions will provide the judges with a complete picture of how your community is exemplifying the six prize criteria Draft site visit itinerary Brief multimedia presentation 35 36

10 10 SITE VISITS: GIVING LIFE AND VOICE TO PHASES I & II Up to 12 Phase II applicants will be honored as finalists and be invited to participate in site visits Teams of 3 5 site visitors will spend about 24 hours in each community. Activities will include a community tour, a leadership dinner, and a community conversation. REQUIRED INFORMATION Phase I Primary contact (with organizational affiliation) Alternate contact (with organizational affiliation) Phase II Any needed changes to primary/alternate contacts A local governmental or tax exempt public charity to accept the $25,000 prize on community s behalf TIMELINE AND KEY DATES May 23, 2013 Phase I Applications due. June 21, 2013 Select communities invited to Phase II. August 1, 2013 Phase II Applications due. September 30, 2013 Select communities invited to site visits. October 17 December 20, 2013 Site visits with finalist communities. First Quarter 2014 Winners notified. June 2014 Winners Celebration Event. 39 CAN ANY COMMUNITY WIN? Yes every community can participate in the process of becoming a healthier place to live, learn, work, and play. We are looking for your unique story not a replication of last year s winners. Judges will assess individual community applications against the six prize criteria. The quality and breadth of each community s accomplishments and its capacity and dedication to create positive momentum to improve health will be key factors in assessing the strength of the application. 40
