1 PROCESS SERVER APPLICANT CHECK LIST (updated 01/25/2018) 1. Must be at least 18 years of age. 2. Have no mental or legal disability. 3. Must be permanent resident of the State of Florida. 4. Signed Affidavit of Experience (all applicants must complete orientation, regardless of experience. 5. Copy of current valid Driver License. 6. Attach proof of Surety bond in the minimum amount of $5, The bond must reflect effective date, expiration date, and name "Seminole County Sheriff's Office" as obligee. 7. Name, date of birth, and social security number. 8. Residence and business addresses and phone numbers. 9. Names, addresses and telephone numbers of the applicant's immediate supervisor and employer (if applicable). 10. Applicant will submit to electronic fingerprinting at the Seminole County Sheriff s Office Human Resource Section on Thursdays, between 8 a.m. and 4 p.m. An administrative fee of $5.35 will be required at the time of fingerprinting and a fingerprint card will be provided to the applicant. An additional fee of $1.75 will be applied if paying this fee with a credit/debit card. (Exact Change) 11. Upon submission of fingerprints applicants will have 30 days to pay the $24.00 background fee to the Florida Department of Law Enforcement using the following link: NOTE: If background fee is not completed within 30 days of fingerprint submission, applicant will have to submit to electronic fingerprinting again.

Applicants will present a completed application (notarized), Certified Process Server training certificate from one of the approved vendors, proof of bond and fingerprint card to the Seminole County Sheriff s Office Civil Section. Upon electronic receipt of background investigation from the Florida Department of Law Enforcement, an order will be submitted to the Chief Judge of the Eighteenth Judicial Circuit on behalf of the applicant for approval or denial. Once the approval order has been received by the Seminole County Sheriff s Office Civil Section, the applicant will receive notification to obtain an identification card from the Seminole County Sheriff s Office who creates and issues the cards on behalf of the Eighteenth Judicial Circuit. If your identification card is lost or stolen, an additional fee of $7.00 will be required for each additional card. TRAINING PROGRAMS/LOCATIONS: 1. 5 CAPS Civil Applicant Payment System Overview These instructions reference the use of a web application that enables individuals to pay, directly to the Florida Department of Law Enforcement (FDLE), for fingerprintbased criminal history requests when these requests are appropriately transmitted to FDLE via registered livescan devices. This web application is accessible from any device with Internet browser software configured to link to the world-wide-web. All counties, in the State of Florida, offer Internet services at their public libraries. All requests for criminal history, as well as the use of this system, must be in compliance with applicable federal, state and local laws and must adhere to FDLE rules, regulations, policies and guidelines. Please Note: In order to use this payment website, the livescan operator must make the proper designation when submitting the livescan transaction. The Transaction Control Reference (TCR) field of the transaction must contain the words CREDIT CARD. The transaction will only be available for online CAPS payment if this requirement is satisfied. **PAYMENT FOR THE TRANSACTION MUST BE MADE WITHIN 30 DAYS** ACCESS THE CIVIL APPLICANT PAYMENT SYSTEM (CAPS) WEBSITE AT: or scan the QR code below To access the CAPS payment page, press Begin Payment Process. FYI Criminal history record check fees are set by Florida Statutes and federal law and will vary based on the type and purpose of the request. 6 CAPS Civil Applicant Payment System Transaction and Billing Information The TCN and applicant/licensee name must be entered exactly as submitted in the transaction. Individuals must complete the billing information from their credit card. The appropriate cost of the criminal history record check is displayed here. After completing the payment and billing information, individuals must first mark the checkbox and then click Proceed. After clicking Proceed. individuals will be redirected to a secure credit card processing site for actual payment. Upon payment, they will be directed back to the FDLE website for their receipt. If the FDLE CWCS application has not received the corresponding livescan transaction, or if there is a problem with any of the data fields or information entered, an appropriate error message will be displayed providing further details. 7 CAPS Civil Applicant Payment System Credit Card Payment information The information provided in the previous screen populates the same fields for payment. Individuals should complete all required fields with their credit card information. Once all the required fields are completed, click on Pay. After payment is completed, individuals will be returned to the FDLE website for their receipt. Any entry errors, not due to credit card processing, will be presented at the corresponding section of the page. Updated MAY 2017
