2 Overview The New Zealand Government is driving significant change in the health and disability sector through its increased focus on evidence-based decision-making, and prioritisation of resources for frontline services. This applies to Pacific populations through two strategic policy settings: Ala Mo ui and Whānau Ora. Ala Mo ui: Pathways to Pacific Health and Wellbeing ( Ala Mo ui) sets out the ways in which the Government expects that health and disability services for Pacific Peoples will be enhanced and improved. The six priority outcomes of Ala Mo ui placed in a broad context, enable the health system to be more responsive to the needs of Pacific people and are as follows: 1. Pacific workforce supply meets service demand. 2. Systems and services meet the needs of Pacific people. 3. Every dollar is spent in the best way to improve health outcomes. 4. More services delivered locally in the community and in primary care. 5. Pacific people are better supported to be healthy. 6. Pacific people experience improved broader determinants of health. Whānau Ora is an inclusive, culturally- anchored approach to provide services and opportunities to whanau and families across New Zealand. The overall aim of whānau ora as described in He Korowai Oranga , is to empower families to make decisions for themselves. This concept provides a useful context for Ala Mo ui, and both documents ask the health and disability sectors to recognise the interdependence of people, that health and wellbeing are influenced and affected by the collective as well as the individual, and the importance of working with people in their social contexts, not just with their physical symptoms. The Ministry of Health has established the Kete: Pacific Grant Fund (the Fund), a fund of $2m (GST exclusive) to support applied analysis and develop action plans to assist the implementation of priority outcomes of Ala Mo ui. The Fund will enable selected health and disability service providers to undertake initiatives that will lead to service and clinical quality improvements and innovation to address Pacific peoples health needs. Funded initiatives should therefore demonstrate evidence of what works in improving Pacific peoples health outcomes so that successful practice can be shared and applied across the health sector through a dynamic evidence-base. The Ministry of Health has contracted Pacific Perspectives Limited to manage and support the Fund. This report provides an overview of the progress that Pacific Perspectives Limited has made with the management and support of the Fund during the period 1 July 2011 to 31 December Kete Pacific Grant Fund Quarterly Report 28 February 2012
3 Key Achievements Pacific Perspectives is pleased to report the following achievements between July and December 2011: 1. Appointment of the Pacific Grant Fund Steering Group, and Pacific Grant Fund Selection Panel; 2. Agreement of the project plan and methodology for the management and support of the Pacific Grant Fund; 3. An extensive fund promotion programme including a series of fono/workshops in Auckland, Hamilton, Porirua, and Christchurch attended by 115 people, with an additional workshop for whānau ora providers; 4. Publication of a Pacific Grant Fund website with a range of supporting information for potential applicants; 5. Publication of a literature review, with a whānau ora component, providing a summary and critique of health service quality improvements in New Zealand and highlighting successful or potentially successful innovative quality improvement initiatives; 6. Development of a comprehensive, cost-effective and relevant monitoring and evaluation framework, incorporating a logic model and Most Significant Change Technique. 7. The receipt of 24 applications with the selection of 12 initiatives for funding, with provision for the funding of a further three initiatives; and 8. Commencement of contract negotiations with the successful applicants. Each of these achievements is discussed in more detail below. Project Steering Group, and Selection Panel We were pleased to obtain the agreement of the Ministry of Health to appoint the following to the Project Steering Group for the Fund: Dr Api Talemaitoga (Chair); Linda Aumua; Gerardine Clifford-Lidstone; Di Grenell; Stephen Lungley (from January 2012); Cynthia Maling (until January 2012); Dr Jim Primrose; Dr Margaret Southwick; and Dr Teuila Percival. The Chair of the Steering Group appointed a selection panel for the Fund on 25 August The members of the selection panel are Gerardine Clifford-Lidstone (Chair/Ministry of Health), Dr Gary Jackson (Health Partners Consulting Group), Tina McCafferty (Counties Manukau District Health Board), and Jen Shieff (Ministry of Health). Project plan and methodology The Pacific Grant Fund Steering Group considered and agreed the project methodology and plan at its first meeting on 12 August The Ministry of Health subsequently reviewed and agreed the project methodology and plan at a project progress meeting held on 6. 3 Kete Pacific Grant Fund Quarterly Report 28 February 2012
4 Fund promotion We developed in consultation with the Ministry of Health a comprehensive communications plan, and refined that plan regularly during this phase as appropriate. As part of the communications plan, Pacific Perspectives contacted 312 individuals from 192 organisations to advise them of the Fund, and to invite them to register their interest. A series of sector engagement fono and workshops were held between 30 August 2011 and 15. These meetings and fono were a: meeting with Whanau ora providers; fono on 30 August 2011 at the Waipuna Conference Centre in Auckland attended by 55 people; fono on 1 at Pataka in Porirua attended by 23 people; fono on 8 at Kaute Pasifika in Hamilton attended by 21 people; and fono on 15 at Christchurch Polytechnic Institute of Technology in Christchurch attended by 16 people. As part of these fono, attendees were briefed on the background to the introduction of the Fund in the context of the significant achievements of the health system, and the considerable opportunities there are to improve health outcomes for Pacific peoples and communities. We have prepared a report relating to these fono ( Pacific Grant Fund - Themes, and FAQs ) which has been published on the Fund website. 6 Literature review The goal of this stocktake and literature review was to start the process of compiling a dynamic evidence base by showcasing recent initiatives that seek to improve service delivery and/or clinical quality to address Pacific health needs in New Zealand. The Fund is intended to help grow and disseminate this evidence-base by including reports from funded initiatives. In completing this stocktake and literature review it is evident that across New Zealand innovative initiatives are being undertaken that seek to improve service and clinical quality to improve Pacific people s health outcomes. However, many of these are not formally written up and are not being evaluated, or this information is not being made available to the wider health sector. The stocktake and literature review identified 87 service and clinical quality initiatives that sought to address Pacific health needs including four Pacific initiatives seeking to apply the principles of Whānau Ora show some promising alignment with Pacific models of care and 'Ala Mo'ui. A comprehensive report comprising the findings of the stocktake and literature review was published on the Fund website in. A dynamic evidence base Further work was conducted between October and December 2011 to build on the stocktake and literature review. This work involved preparing summaries of the initiatives included in the initial stocktake and literature review, and incorporates relevant international literature on the effective components of health service and clinical quality improvement initiatives for ethnic minorities. Over the next two years, the Pacific Grant Fund will add reports from funded initiatives to this dynamic evidence base. This will enable the dissemination of innovative practice and help to improve service and clinical quality for Pacific people. The report was published on the Fund website in February Monitoring and evaluation framework Pacific Perspectives Limited engaged Litmus, a research-based consultancy, to develop the monitoring and evaluation framework for the Fund. The monitoring and evaluation framework depicts the intended short, medium and long outcomes of the Fund, the indicator associated with each level of outcome, and an outcomes framework for assessing the results achieved by each successful initiative. 7 First selection round The Selection Panel met on 20 October 2011 to discuss the Fund, including the role of the panel, and the selection process with a view to better understanding the selection criteria. The deadline for applications as part of the first selection round was midday on 7 November A total of 24 applications were received on 7 November 2011 and accepted for consideration. All applications were receipted within two working days of the deadline for the submission of applications, and were distributed to panel members on 10 November The selection panel met on 17 November 2011 and considered each application, and agreed recommendations to the Steering Group covering: whether applications should be recommended for funding; what, if any, level of funding would be appropriate; the duration of funding for each initiative; what, if any, additional support may be appropriate for applicants; and what, if any, feedback could be provided to applicants. The report of the selection panel arising from that meeting addressing each of these matters is attached for consideration by the Steering Group (appendix four refers). Contracting providers The project team has now contacted all successful and unsuccessful applicants to advise them of the outcome of their application. All applicants received a telephone call which was followed up by a letter detailing the decision of the Project Steering Group and the feedback from the selection panel. Ninety percent of applicants were advised through both channels within three working days of the agreement of the minutes of the Project Steering Group. The project team also met with nine successful applicants (five in Auckland and four in Wellington) on 2 and 5 December 2011 and 10 January 2012 respectively. The focus of these discussions was identifying ways in which the project team could support the applicants to respond to the feedback from the selection panel. The project team elected not to meet with all successful applicants prioritising those that were more likely to benefit from direct support. Successful applicants were set varying deadlines for providing responses depending on the complexity of the feedback provided by the selection panel. Contract negotiations were well advanced by 31 December 2011 with most successful applicants having provided responses to the feedback provided to them by the Pacific Grant Fund Selection Panel. Work is currently underway to review the responses that have been provided, and complete the contracting process. Fund outcomes and indicators The monitoring and evaluation framework for the Fund describes short, medium and long-term outcomes for the Fund, along with indicators for these outcomes. 7 Kete Pacific Grant Fund Quarterly Report 28 February 2012

10 Current work The main focus of the activities to support the management and support of the Fund is the completion of contract negotiations, and support of the successful initiatives. The current focus of the support for the successful initiatives is the preparation of individualised plans to assist with the refinement of the approach that the applicants have proposed for monitoring and evaluation. Pacific Perspectives Limited has engaged the services of Associate Professor Jackie Cumming and her team at the Health Research Centre of Victoria University of Wellington to review each proposal and develop these plans. Next steps The main focus of the first quarter of 2012 will be to: 1. Complete contract negotiations for the 12 successful initiatives, and support the further refinement of the other three initiatives (noted above); 2. Continue the work to refine the monitoring and evaluation framework for the Fund; 3. Release updated supporting documentation, including revised guidelines and application forms that take account of feedback from the first selection round; 4. Continue engagement with potential applicants to promote the Fund; and 5.
