What is the number of full-time equivalent positions currently employed? 2 FT & 3 PT staff in the Rural Ingham Service Center 17. How many volunteers are currently involved? We have two four volunteers on a regular basis. In addition, volunteers vary from one project to another, two or three to entire groups of 35 or more. Several Civic organizations support the activities in the center and over the year volunteers are often involved.

2 18. a. How does your proposal involve coordination or collaboration with County services, other local governments, or other agencies? CACS works collaboratively with other service providers, faith based organizations, local businesses and other non-profit organizations to address unmet needs in the community. Close collaboration with other service providers maximizes resources and avoids unnecessary duplication of services. What other programs similar to your program are operating in the County? DHS provides assistance with utility shuts-offs as well as, the Salvation Army and St. Vincent DePaul. Direct payment assistance provided through Capital Area Community Services requires participation in an Energy Education workshop prior to payment to help individuals reduce their energy consumption which in turn leads to less dependency on organizations. Funding for home heating assistance has been substantially reduced (25%- 40% over the past two years). Requests for assistance with home heating costs has risen in light of the economic situation in our area. 19. Is this your first request for funds from Ingham County? NO 20. a) Have you sought funds for this proposal or concept from any other entity? Please identify who and the response. Yes No Dart Foundation has awarded home heating assistance funds to assist with this unmet need, and we anticipate that continued support through the local Walk for Warmth fund raising efforts in Rural Ingham County. Last year this Rural Ingham fundraising effort raised $17,146. Ml Public Service Commission Funds through the Ml Community Action Agency Association has been the one constant source of metered home heating assistance funds in the CACS service area. The current contract is 26% lower than last year and, at this time funding for 2013 is unknown. b) Does the proposal anticipate utilizing funds other sources? Yes NO In addition to the funding described above, we just received the renewal of the Low income heating assistance contract LCA (deliverable fuel) for However those funds cannot be used for metered fuel. c) To what extent is the other funding assured? At this time the other funding source contracts mentioned in 20 (a) are expected, however the level of funding is uncertain at this time.

3 DETAILED SCOPE OF WORK The Scope of Work should be one page or less and contain: 1. A statement regarding the general nature of services and population with which your agency concerns itself. CACS provides services to the low income population in the four county service area which includes Rural Ingham County. The local office is located at 218 E. Maple Street in Mason and, both outreach and direct services are provided to the outlying areas as well, Stockbridge, Dansville, Leslie etc. CACS services include FREE tax preparation assistance, Emergency Food Bank and surplus commodity food distribution, Senior Services, MMAP, Weatherization, Homeless Prevention and Homeless Case Management, Family Stability to prevent homelessness through direct payment assistance to vendors on the clients behalf, home heating assistance, Client Education including Budgeting and Money Management, Energy Conservation and Home Buyer Education, as well as Information and referral and linkage to other services in the community. 2. A statement explaining how the proposal will address the County's long-term objective of Meeting Basic Needs. (See the attached Resolution # Resolution Approving Criteria for Ranking 2013 Applications for Community Agency Funding). This proposal addresses the Ingham County Board of Commissioners objective of providing direct home heating assistance to meet Basic Needs and avoid duplication of resources. Direct home heating assistance prevents homelessness by maintaining basic services and, result in greater stability of the family unit. 3. A statement detailing the specific services which will be provided by Ingham County' appropriation. A statement of the specific target group for these areas. Direct Home Heating Assistance for low income individuals/families in Rural Ingham County, with specific emphasis on Seniors 60+ years of age. Direct payment assistance provided through CACS requires participation in an Energy Education workshop prior to payment to the heat related vendor. 4. A statement describing the eligibility criteria for these services. (See the attached Resolution # Resolution Approving Criteria for Ranking 2013 Applications for Community Agency Funding and must serve residents of Ingham County are eligible). Eligible households must have an annual income level at or below 150% of the federal poverty level income guidelines, which for a family of two (2) in 2012 is $22,695 and must be a resident of Rural Ingham County as outlined in this proposal guidelines and any other restrictions so set by the Ingham County Board of Commissioners. 5. A statement describing the time for the performance of these services. Services will be proved between January 1, 2012 and September 30, 2013, or as otherwise determined by the Ingham County Board of Commissioners Ingham Co Community App 2013
