No. Activity Description Status Completion Date 1: Rotarians from Rotary Club of Bukoba (RCB) In collaboration with Misenye District Office (a governmental entity) we organized the sensitization meetings with the beneficially community to address the project objectives and their roles during project implementation and sustainability requirements. The project was officially handled over to the village government during the ceremony - Page 1 of 8 -

2 Publicity attended by the Kagera Regional Commissioner (equivalent of a state governor in the U.S.) on June 27, During this ceremony, the villagers testified how the water tanks have helped them to reduce the distance traveled to the old water pit and how their health has improved from using cleaner water. The Reginal Commissioner thanked the participating Rotary Clubs and Rotary International for such a wonderful project. He asked the villagers to take a very good care of their three water tanks. Complete 07/05/2016 2: Equipment/supply purchase Construction equipment and materials for the water tanks. The construction materials were procured in two different phases of the project implementation. 3 Monitoring/evaluation construction to another and to recommend on the next phase of fund disbursement to the contrator. This also included monitoring of training activities for community members on health related issues. Complete 30/06/2016 4: Training Training of community members on health related issues, especially sanitation and hygiene. Three (3) training events have been conducted involving 236 people. The training also involved preparing a select number of community members on how to maintain the water tanks, especially cleaning of the tanks before the rain season starts. Ten (10) people have been trained on how to maintain and clean the water tanks. Complete 30/06/2016 5: Construction of three water tanks at three different locations. The tanks were constructed in two phases. The first tank tank was constructed at a local Catholic - Page 3 of 8 -

4 Operations church which donated the land and provided permission for harvesting rain water for the church building's roof. This tank with the capacity of 90 cubic liters (23, gallons) serves 850 families. Two last tanks were constructed at two different local schools each with the capacity of 70 cubic liters (18,492 gallons): Kilimilile Secondary School serving 750 user (students and villagers) and Mabele Secondary School (serving 1,650 students and villagers). No. Organization Club/District Describe how the host and international Rotarians committed their time and expertise during the implementation of the project. The international partner was responsible to receive the fund from TRF and to disburse the same to the host club for the project implementation. The International partner disbursed the fund in two phases, whereby the second disbursement was after completion and reporting for phase one implementation of the project by the host club. In phase 1 the tank of 90 cubic meter was constructed at Kilimilile Catholic Church while in phase two tanks each with 45 cubic meter were constructed at two secondary schools. Host club project committee members participated in selecting the contractor and in sourcing all the material requirement for the project. The technical project adviser Rtn. Lugaisa led the project committee in all technical issues from planning stage, during implementation through regular visits to the projects sites. Host Rotarians mobilised the community members to support the project, and the district's technical team were involved and provided advisory support. - Page 4 of 8 -

5 The host partner were updated regularly in the project progress, through reports and project photos. The international partners' comments advise were beneficial in making the project is successfully implemented. How has the cooperating organization participated (if applicable)? Under the leadership of the council chairman and the district director, technical team from health, water and sanitation departments supported the project trough advisory and training of the community members. They also participated during handling the project to the beneficially community, the activity that was headed by the Regional commissioner...(name). Share any financial management challenges. Each partner to this project had financial management role. The international partners was supposed to receive the money from TRF and disburse the funds to the host implementing partner in phases. Since the project implementation was divided in two phases, also the fund were disbursed in phases the next following the successful implementation of the previous phase. The payment to the contractor were also done in phases based on the contract agreement between Rotary Club of Bukoba and the Contractor. The contractor was responsible to the purchase of the building materials after presetting the price quotations that were to be approved by the project committees after verification from the potential suppliers. Inflation and the instability of the US dollar against the TZS and the delays of the project implementation from when the contractor's estimates were submitted to the implementation time affected the project budget. However, the project was implemented successfully and met the expected outcome of the stakeholders. It should be noted that Tshs. 69,537,671 was budgeted and Tshs. 88,682,385 actually spent (showing a deficit of Tshs. 19,144,714). There was a gain on dollar exchange because the Tanzania currency dropped against the U.S. dollar which created exchange rate gains. That also affected the internal price of the industrial materials purchased for the project. Authorizations Both host and international sponsors are required to authorize the report. If the primary sponsor is a club, then the current president authorizes the report on behalf of the club. If the primary sponsor is a district, then the current district Rotary Foundation committee chair authorizes the report on behalf of the district. After the primary contact affirms that the report is complete, the report will be locked for editing and the officers authorizing this report will receive an notification requesting their authorization. - Page 7 of 8 -
