5 PART 3 1. If the Applicant is not an individual, provide a certified copy of certificate showing that the Applicant is in good standing. 2. What is the requested term of the Export Dealer Licence (a maximum of 5 years)? 3. What is the annual estimated volume and species of fish to be purchased by the Applicant? 4. What are the regions or lakes from which fish are proposed to be purchased by the Applicant? 5. What is the minimum purchase price, per species, to be paid to fishers, specified by form and location? 6. What products, if any, are to be produced by the Applicant? 7. Provide the legal names and addresses of the owners of any facility which will be used by the Applicant to produce the products, the address and location of such facility, and a general description of such facility. (The Applicant must provide a copy of all information set forth in Section V(c) of the Policy. 9. Financial information that will show Applicant s ability to pay fishers and all other stakeholders in their application. 10. Information regarding how deductions from fishers required under federal and provincial law will be remitted to the appropriate governmental authorities. 6
