1 The Kansas City Zoo awards scholarships to families and individuals based on their financial need and available space. Below are the guidelines: Scholarships are available for children between the ages of 4 and 15. Multiple scholarships may be awarded per person. If applying for multiple individuals, each person must have a separate application. Applications must be submitted or postmarked by Friday, March 16, 2018 to be considered. NO EXCEPTIONS! All scholarships will subsidize 90% of the program fee; awardees will be required to pay the remaining 10%. This percentage is based off of the Non-FOTZ member program pricing. All applicants will be notified via of their status by about April 6, Please do not or phone about your status of your application. Scholarships are awarded based on your level of financial need and available space. Please send: Documentation of eligibility/participation in free and reduced lunch program from your school or administrator If do you not have this item, we still encourage you to apply! Scholarships are available for the following programs on weekdays from May 29 th through August 10 th : Summer Break Half Day Camp (8:30am-11:30am) ages 4 to 5 The Kansas City Zoo will provide a snack and one T-shirt for this program. If your child has special dietary needs, we highly suggest you pack a snack. Summer Break Full Day Camp (8:30am-3:30am) ages 5 to 12 The Kansas City Zoo will provide two snacks, daily lunch and one T-shirt for this program. If your child has special dietary needs, we highly suggest you pack snacks and a lunch. Summer Break Junior Zoologist Day Camp (8:30am-3:30pm) ages 13 to 15 The Kansas City Zoo provides two snacks, daily lunch and one T-shirt for this program. If your child has special dietary needs, we highly suggest you pack snacks and a lunch. 3 PART 2: STATEMENT OF INTEREST IN PROGRAM This section is to be answered by the potential scholarship recipient with assistance from parent or guardian if necessary. Why would you like to participate in summer programming at the Kansas City Zoo? 3

4 PART 3: LETTER OF REQUEST This section is to be filled out by parent or guardian applying for the scholarship. Please explain why you are seeking financial assistance. Include any special circumstances, such as high medical bills or unemployment, and any other pertinent information. This information will be kept confidential. 4

5 NOTE: Demonstration of financial need is a requirement for qualification for the Kansas City Zoo Education Scholarship. We ask that you provide documentation of participation in free and reduced lunch program. If you do not have this documentation, please make sure to list in Part 3 your financial reasons for applying for a scholarship. I certify that my answers are true and complete to the best of my knowledge. I understand that failure to truthfully set forth this information could result in loss of the Kansas City Zoo Education Scholarship. I have attached documentation of participation in a federal free or reduced lunch program as proof of financial information or I have listed in Part 3 of this application my financial reasons for applying for a scholarship. I understand that demonstration of financial need is a requirement for qualification for the Kansas City Zoo Education Scholarship.
