Through partnership and advocacy, we are working to transform communities through local and systemwide change so everyone has an opportunity for better health. HCF in the Community Each year we invest over $20 million in grants, advocacy, and leadership to work together with our community partners to improve the health of our communities. Whether it s improving access to healthy foods, safe places for physical activity or ensuring everyone has access to mental health, oral health, and health care services, we are proud of the work being done in the region to create a culture of health. 3 OVERVIEW Healthy Communities In our first 10 years of grantmaking, HCF has dedicated nearly $35 million toward creating healthy communities throughout our region. The goal of our healthy communities funding is to increase access and use of healthy foods, create safe environments for physical activity, and support tobacco use prevention through resident-focused policy change. Mental Health Over the past decade, HCF has provided more than $62 million in grants to improve access to mental health services. The goal of our mental health funding is to provide support, treatment, prevention, and advocacy for mental health and family violence services for children and adults. Safety Net Care To ensure vulnerable populations have access to a strong safety net system, HCF has dedicated nearly $62 million in the past decade to health and oral health services in our region. The goal of our safety net funding supports greater access to a safety net of services that provide safe, timely, efficient, equitable, integrated, affordable, and quality health care and oral health care. HEALTHY COMMUNITIES Support environments and policies for healthy eating, active living, and tobacco use prevention MENTAL HEALTH Provide support, treatment, prevention, and advocacy for mental health and family violence SAFETY NET CARE Support access to quality health care and oral health care 2
4 GOALS & STRATEGIES Encourage health equity and wellness through increased access to behavioral prevention, assessment, treatment, and advocacy for children, youth, and adults. A total of $4.45 million is available for 2018 Mental Health funding. Grants will be awarded for a period of up to two years. Because approval of a two-year funding request can affect the number of grants HCF can award, these applicants are required to provide a compelling explanation of why the project requires multi-year funding. OUR STRATEGIES CAPACITY Support the advancement, efficiency, and sustainability of organizations and systems providing care Requests should employ at least one of the three strategies (pictured to the right) to improve mental/behavioral health outcomes. 5 Implementation of Emerging, Promising, and Best Practices Evidence-based practices (EBP), also referred to as best practices, are those programs and practices that have undergone formal scientific evaluation and expert peer review to document their effectiveness. Substantiated EBPs can be found in literature, databases, and registries of research-based practices. The evidence-based practice continuum includes emerging and promising practices, which also rely on evaluation of outcomes and application of proven guidelines. HCF recognizes that EBPs have not been established for all conditions, populations, and/or service settings some approaches are based on tradition, contextual need, and practice wisdom. Approaches are often a combination of practice categories on the continuum. If an applicant is proposing a project that doesn t fall within the EBP continuum, the applicant is encouraged to provide information describing the practice and a rationale for why the chosen practice is appropriate for the outcomes to be achieved with the target population. Introducing Indicators For each funding round, HCF has developed a list of short-term outcome indicators. Our indicators allow us to collect data and progress toward our short-term outcomes across grantees. They also allow us and our partners to measure and describe program impacts systematically. In this year s proposal, applicants should identify with indicators that correspond with an applicant s selected theory of change strategy and outcomes (see page 5). A full list of indicators is on our website at hcfgkc.org/mental-health/ WHAT WE MEAN Behavioral Health problems include substance use disorders; alcohol and drug addiction; and serious psychological distress, suicide, and mental disorders. SAMHSA Care Coordination/Integration brings together providers and information systems to help better achieve treatment and care goals. Continuum of Care is a comprehensive approach to behavioral health that recognizes multiple opportunities for addressing behavioral health problems and disorders including: creating environments and conditions that support health (promotion), interventions delivered to prevent or reduce problems (prevention), service to those diagnosed with a disorder (treatment), and services to support individuals ability to live productively (recovery). Mental Health is a state of well-being in which the individual realizes his or her own abilities, can cope with the normal stresses of life, can work productively, and is able to make a contribution to their community. World Health Organization Place-based Initiatives are programs designed and delivered within a geographic space, such as a neighborhood, school, region, or an ecosystem. Policy guides the activities of government or organizations and provides authority for the use of resources including institutional practices, and public policy. The Equity Approach is the means to ensure that everyone has the opportunity to participate and prosper when building strategies to improve community health. EQUALITY EQUITY 4
6 MENTAL HEALTH THEORY OF CHANGE PARTNERS All uninsured, underserved, and vulnerable populations in our service area Community mental health centers, communitybased organizations, and public systems BARRIERS Individuals lack adequate access to and coverage for services Services are difficult to navigate and often fragmented Consumers can have complex clinical and practical needs Current financial environment does not match funding needs of systems and providers Perpetual workforce gaps impede service stability Stigma associated with mental illness GUIDING PRINCIPLES Understanding the conditions and adverse experiences a person has gone through can help guide treatments and care Services need to be culturally responsive and equitable The continuum of services includes being responsive to prevention, treatment, and recovery over the lifespan Effective systems of care integrate mental/ behavioral health and medical care Stable, healthy organizations provide better care and services * Mental and behavioral health services are referred to as services throughout this document. 5
7 PURPOSE To improve the availability and quality of mental and behavioral health services.* STRATEGIES SHORT TERM LONG TERM Improve Capacity Advance strategies that deepen workforce and leadership capacity Fund core operating support that leads to organizational improvement, builds infrastructure, or promotes sustainability Develop strategic and accountable partnerships that lead to enhanced continuum of care Improve Access Promote policies, practices, and technology that increases coverage, affordability and availability of services Support place-based services and strategies Encourage coordinated care and linkages among multiple agencies and partners Support outreach strategies to inform and engage target populations Improve Capacity Increased retention of quaity staff and leadership Improved organizational structures that promote sustainable, highquality service Enhanced partnerships that improve efficiency and sustainability Improve Access More affordable, available, and convenient preventive and treatment services Consumers successfully navigate through service delivery systems Improved individual and community engagement in mental/behavioral health wellness BETTER HEALTH Improve Quality Practice Encourage the use of emerging or best practices that are evidence-based Support services that are family focused, person-centered, culturally responsive, and trauma-informed Support the integration of services into primary care settings Improve Quality Practice Improved mental health/behavioral health outcomes for individuals Improved use of evidence-based, emerging, and best practices in service delivery Consumer satisfaction with service delivery Greater integration of services BETTER CARE ULTIMATE IMPACT Mental and behavioral health supports will enable people to improve their state of well-being and live healthy and resilient lives in the community. 6
8 APPLICATION PROCESS All organizations interested in applying for a 2018 Mental Health grant will be expected to complete the online grant application available at hcfgkc.org. Please note there will be slight changes to the application forms this year. Cities, hospitals, and universities are allowed to submit two proposals per each foundation defined grant round. The application process requires applicants to complete two steps: letter of intent and full proposal. Both steps include a number of required attachments, which can be found at hcfgkc.org/online-applications. Please ensure you are using the current 2018 templates from HCF s website. Do not use any previous templates you may have downloaded. The 2018 Mental Health Funding Application Webinar can be found at hcfgkc.org/videos. Please review our Before you Apply section of the website for organizational requirements and to ensure you adhere to our non-discrimination policy. Step 1: (deadline March 21) Letter of Intent and Attachments Applicants are required to submit a letter of intent. It should include a completed online application form and the following attachments: 1. Letter of Intent Narrative (c )(3) IRS Letter of Determination Applicants will receive an confirming receipt of the application. Step 2: (deadline April 18) Full Proposal and Attachments The full proposal should include an updated online application and the information indicated below. 1. 9 SELECTION PROCESS After an initial internal review process, a team of reviewers from outside HCF s service area will make recommendations for grant funding. External reviewers may also recommend further negotiation, modification, or technical assistance. HCF s program officers will make the final decision on the slate of recommendations in response to external reviewers guidance and HCF board direction. These recommendations will be presented to the Programs/Grants Committee for consideration on July 10, 2018, before they are approved or declined by vote of the HCF Board of Directors on August 22, REVIEW PROCESS Internal Review Process External Reviews Internal Due Diligence* HCF reserves the right to reject proposals submitted; to request additional information or clarification from any or all applicants; and/or negotiate changes with applicants at any time before, during, or after the award process. Grant awards are made at the sole discretion of HCF. No entitlement to funding for any organization at any level is expressed or implied. Successful applicants enter into a contract that gives HCF rights to review and evaluate grant-funded activities. Board Approval Awards Announced * As recommended by external reviewers 8

If interested, please contact the main office line at Grantwriting Technical Assistance HCF offers technical assistance to nonprofit organizations with annual budgets under $500,000 for both applicant defined grant and foundation defined grant requests. Fiscal Agents HCF connects nonprofit grantees that do not have an annual audit with fiscal agents. This service is available for both applicant defined and foundation defined grantees. Grant Review HCF invites prospective applicants to visit our office to review past successful proposals. Feedback Sessions HCF invites applicants who were not awarded funding to receive feedback from the program officer(s) who reviewed their request. Feedback sessions may be conducted in person or over the phone within six months of receiving the declination.
