(CEDA) is one of the largest private, nonprofit Community Action Agencies in the country and serves more than 500,000 Cook County residents annually. It is CEDA s mission to work in partnership with communities to empower individuals and families to achieve self-sufficiency and improve their quality of life. In order to achieve its mission, CEDA has established numerous community partnerships and collaborations as part of its service delivery model. CEDA offers over thirty programs and services in the areas of children and family services, community and economic development, education, emergency assistance, employment and training, energy conservation and services, health and nutrition, housing and senior services. CEDA is the largest of 40 Community Action Agencies (CAA) in Illinois. 2 Community Services Block Grant (CSBG) s 695 clients served - CSBG works to help stabilize and support families and individuals on a path to self-sufficiency. $769, funds provided in Direct Client Assistance FY scholarships awarded $138, awarded in post-secondary and vocational scholarships *Figures as of s extended through March 31, 2016 Offering case management services and direct client assistance in the areas of auto repair, dental care, employment training, financial literacy, nutrition, postsecondary and vocational scholarships, and vision assistance. Suburban Funding Source: Illinois Department of Commerce Eligibility: To be eligible for CSBG, you must be a resident of suburban Cook County, be able to provide documentation of need, and meet income guidelines (125% of the federal poverty level). CSBG participants must actively participate in case management sessions. 3 Women, Infants, and Children (WIC) Over 50,000 clients served in 2015 WIC is a nutrition education, intervention and prevention program for pregnant, breastfeeding and post-partum women, infants, and children under age five. WIC provides individual assessment and vouchers for nutritious foods. CEDA WIC operates 19 neighborhood based clinics and supports over 50,000 moms and babies in Chicago and suburban Cook County. WIC is partnered with the Chicago Botanic Gardens to increase exposure and promote the benefits of fresh produce to WIC clients. Funding Source: United States Department of Agriculture Eligibility: To be eligible for WIC, you must either be pregnant; be breastfeeding; have had a baby in the past six months; or have a child or infant younger than five years old. You must also have a health or nutrition risk and meet income guidelines (185% of the federal poverty level). Parents, legal guardians or caretakers who have medical and financial responsibility for a child may apply for WIC benefits. 4 1,626 clients served - $14,225, funds saved in mortgage assistance 102 Extremely low and severely low income families placed in permanent housing 53 Homeless Male Veterans and Families were placed in transitional housing Housing Services CEDA s Housing Counseling delivers comprehensive counseling services to homebuyers, homeowners, very-low to moderate income renters and the homeless in. The CEDA Housing Department is a counseling program designed to educate, inform, assess, listen, and find creative ways to empower families and individuals in acquiring and maintaining affordable housing. Shelter services include homeless male veterans and families transitioning from homelessness to transitional housing located in Chicago Heights, IL and western suburban Cook County. Funding Sources: Department of Housing and Urban Development (HUD) Illinois Housing Development Authority For more information visit: Counseling services in the areas of: pre- and post- purchase counseling foreclosure prevention and mortgage default counseling rental and homeless prevention counseling Permanent Housing for extremely low and severely low-income families Transitional Housing for Homeless Individuals and Families Town of Cicero Transitional Housing for Homeless, Honorably Discharged Veterans Eligibility: The Housing Department serves the extremely low to moderate income families and individuals. For pre- and post- purchase counseling, foreclosure prevention and mortgage default counseling, and rental and homeless prevention counseling there are no income limits. Anyone needing housing assistance can receive counseling services. Special restrictions apply to the transitional housing and permanent housing programs.

5 Energy Services 181,930 households served $86,086, in LIHEAP Direct Client Assistance 33,000 households enrolled in PIPP $32,670, total PIPP grants applied PY 2014 $4,946, total ComEd Residential Special Hardship grants applied $832, total Peoples Gas Share the Warmth grants applied CEDA s Energy Services is the largest in Illinois serving over 150,000 households per year. We assist eligible Cook County residents with their home heating and cooling energy costs as well as heating systems repair or replacement. The following programs are available: Low-Income Home Energy Assistance (LIHEAP), ComEd Residential Special Hardship, Peoples Gas Share the Warmth, and Furnace Assistance. Funding Sources: Department of Health and Human Services (HHS) Illinois Department of Commerce For an updated list of available Energy Services programs please contact the CEDA Hotline at (800) 571-CEDA (2332) Eligibility: To be eligible for LIHEAP you must need financial assistance for home energy costs and have a household income at or below 150% of federal poverty guidelines. Households with zero-incomes and/or household members who participate in certain other benefit programs, such as the Supplemental Nutrition Assistance (SNAP), Supplemental Security Income (SSI), Temporary Assistance for Needy Families (TANF) or certain needs-tested Veterans benefits may also be eligible for energy assistance. ComEd Residential Special Hardship is available to residential ComEd customers with household incomes up to 250% of federal poverty guidelines who demonstrate a hardship. Peoples Gas customers who are having trouble paying their bills and meet household income guidelines (at or below 200% of federal poverty guidelines) may be eligible for a matching grant through the Share the Warmth. 6 Weatherization CEDA s Weatherization provides solutions to reduce energy costs and make homes more energy efficient for eligible residents in. 2,180 units weatherized in We install energy saving measures to increase home performance, reduce energy and save money for our clients. We do this through inspections, insulation, and air sealing. Our services are performed at no cost to eligible applicants depending on the program. The Multi-family program requires a 50% contribution of mechanical costs. Funding Sources: Department of Commerce Eligibility: If a household s combined income for the 30 days prior to application is at or below 150% or 200% of federal poverty guidelines, they may be eligible to receive weatherization assistance. Weatherization is a one-time program by address. Homeowners that move into a home that has not been weatherized since 9/30/1994 are eligible for weatherization if they meet the programmatic guidelines. Clients are eligible for weatherization at a new address if it has not been previously weatherized.
