Our emergency room experience provides efficient care for the ill and injured, but goes one step further. We have forged partnerships with community agencies to provide mental health and other social service interventions that are needed by many individuals who find themselves coming back to our emergency room repeatedly. NORTH OTTAWA HEALTH SYSTEM 5 GET BETTER 6 EXPANDING PRIMARY CARE FOR YOU Whether you are looking for a specialist in adult medicine or a doctor to care for your entire family, our primary care medical offices are your best resource. We are ready to serve as your patient-centered medical home your first point of contact for any health or wellness need. In the 2012 Community Health Needs Assessment in Ottawa County*, we learned that access to primary care and managing chronic disease were two areas that need improvement. In response, we grew the North Ottawa Medical Group (NOMG), which is now approaching 30 providers (physicians and mid-levels) in the areas of Family and Internal Medicine, as well as Women s Health. When you need specialized care, we connect you to the experts you need - often our colleagues who are on our campus. This includes our hospitalists group, established in 2014 to oversee the care of patients in the hospital s inpatient and intensive care unit. The underpinning to your medical home experience is your care coordinator, guiding you through your healthcare journey. The care coordinator connects you with the many touch points in our health system. Our primary care offices are equipped with electronic medical records to help streamline your care when you are referred to a specialist. Also, our hospital and medical group each have patient portals that serve as your personal access point and communication hub. Our patient portals allow you to request appointments, review test results, refill prescriptions and interact with our healthcare professionals. *2015 Ottawa County Community Health Needs Assessment released in May of 2015. NOMG was recognized as a patient-centered medical home, acknowledging the patient-focused approach of its providers. AS A PATIENT- CENTERED MEDICAL HOME, WE: Ask what your goal is or what you want to do to improve your health. Ask you to help us plan your care, and to let us know if you think you can follow the plan. Create written copies of care plans for more complex illnesses. Have the care team members doing more and/or different parts of care like exploring methods to care for you better; including ways to help you care for yourself. Remind you when tests are due so that you can receive the best quality care. Ask you to have blood tests done before your visit so that the doctor has the results at your visit. AND WE WILL CONTINUE TO: Provide you with a care team led by your physician who will know you and your family. We will strive to meet your needs. Respect you as an individualwe will not make judgments based on race, religion, sex, age, disability, etc. Respect your privacy-your medical information will not be shared with anyone unless you give us permission or it is required by law. Give care that is based on quality and safety. Have a doctor on call 24 hours-a-day and 7 days-aweek. Tell you about your health and illnesses in a way you can understand so that together we can take care of short or long-term illness and give advice to help you stay healthy. From there, community-based services, which help people stay well and age in place are coordinated, along with hospital services in times of sickness or trauma. Serving as your medical home means we help coordinate your care throughout our health system. You will take many steps in your health journey and our care coordinators serve as a consistent guide through each point of care. They also help ensure that you receive the education you need to manage your health condition or help keep you on track to prevent a medical issue. We realize there may be time to receive care at another facility for more serious conditions. We can coordinate your transfer as well as your return back to our care, enabling us to be the home base for your lifetime of health. NOCHS uses leading-edge technology to make your care coordination efficient and convenient. Electronic medical records make it possible for us to share information with other providers through secure technology platforms. We are part of the Great Lakes Health Connect, a cloud developed specifically for providers throughout Michigan to share health information when patients need care in other parts of the state. We continue to expand our service offerings in two ways: to provide the care you need when you are sick or injured as well as to help you stay healthy and prevent disease. In addition to establishing and expanding our primary care practice, our community-based services are expanding to help individuals heal at home and age in place with home nursing care, medical equipment and hospice care. We also continue to have more patients seeking our specialty and diagnostic care. We have often been the first to bring cutting-edge technology to the lakeshore including Breast MRI, da Vinci HD Si technology and the SPY Elite Imaging System allowing for better outcomes in bariatric, urology, general, orthopedic and reconstructive (breast) plastic surgery programs. NORTH OTTAWA HEALTH SYSTEM 11 GET BETTER 12 A HEALTHIER GETS BETTER Getting healthy and staying healthy is not just something that happens in hospitals. It happens every day in the places where we live, work and play. We have stepped up to address health needs in the community by creating new programs in collaboration with other organizations. When we get involved, we build programs we know can make a difference for your health. Here are some examples of our community outreach efforts: EAT BETTER NOCHS partnered with the local Chamber of Commerce to launch the Farmers Market Education Station at the Grand Haven Farmers Market. Our physicians, community educators and guest chef, Ellen Vonesh of Consider It Done, provided interactive displays and healthy food samples for shoppers to taste and better understand the benefits of leading a more proactive, healthy lifestyle. MOVE BETTER This past year, NOCHS underwrote, trained and coached the participants from our Bariatric Clinic staff and patients in the Coast Guard 5K Run. For some it was the culmination of a life-long journey for others it was the beginning of new lease on life for us, it was the thrill of a lifetime to see so many lives changed for the good, and forever. LIVE BETTER Many of our families and constituents struggle with applying for and securing health insurance, serving as a roadblock on their path to leading a healthy life. NOCHS is fully committed to seeing our entire community Live Better. We are the only facility in Ottawa County that has opted to employ certified application counselors to assist with signing up for health care coverage online. FEEL BETTER When we asked our employees to volunteer their time to help one of our area partners, Tri-Cities Ministries Counseling, the response was so quick we filled our sign-up sheet in five minutes. Our volunteers painted, steam cleaned, dusted, repaired, and had so much fun doing it - we will be back in the spring for more. Additional employee support comes in the form of Jeans Day, when we encouraged our employees to wear their jeans to work for a $5 fee with all proceeds going to our local partners (including TCMC). BENEFIT $9.1M Total Benefit Our annual, multi-million dollar community benefit investment comes in the form of charity care, bad debt, as well as the widening gap of Medicare and Medicaid reimbursement shortfalls that s resulting from health reform. These dollars also reflect an investment in our community s wellness. Free health programs, screenings and support of the Love, INC Free Health Clinic, where we provide free basic lab and x-rays for clinic patients and clinical volunteers. This is part of our long-term commitment to reshape community health into a place that s better known for keeping people healthy vs. treating them only for illness or injury.

NORTH OTTAWA HEALTH SYSTEM 13 GET BETTER 14 A BETTER FUTURE FOR EMERGENCY CARE BUILDING FOR THE FUTURE GOES HAND-IN-HAND WITH ADDRESSING AND ANTICIPATING THE HEALTH CARE NEEDS OF OUR MEMBERS As a community hospital, emergency care is a vital service. A new state-of-the-art emergency room is planned to address the growing needs of our community. The new 15,000-square-foot emergency room will enable our team to provide the optimal experience for ill and injured patients, and it goes one step further. We have designed a care model that also helps individuals who are seeking primary care, mental health resources or other social services. They may not know how to access these services so they come to the emergency room - creating approximately 4,800 visits to our emergency room per year that do not involve an emergency. We want to help these people receive the care they need in the appropriate setting and guide them on a path to wellness. We have forged partnerships with community agencies such as the United Way and various mental health agencies to provide these specialized services in our emergency room so individuals will have their needs addressed onsite as well as virtually. This will decrease the need for future emergency room visits. Over time, if we reduce misuse and overuse of the emergency room, our local population s health will improve. This will help us make way for an increasing number of visits per year due to population growth and statewide insurance expansion. Our goal for the general population and of patients remains focused on thorough, decisive care delivered quickly. A capital campaign is under way to raise $2.5 million for this exciting new project, with an expected groundbreaking in 2015. Please consider an investment in the emergency room project.
