LADYSMITH NURSING HOME PROFESSIONAL SERVICES INDIVIDUALIZED CARE AND TREATMENT PROGRAM: NURSING - provides a individualized care program to meet each residents physical and psychological need implementing their medication regimen and specialized services per the physicians orders or nursing protocol. ACTIVITIES - provide individual and group activities daily with programs being structured to each resident s physical, mental and psychosocial abilities. PASTORAL SERVICES - clergy of all faiths are involved in meeting the spiritual needs of our residents. SOCIAL SERVICES - work with the resident and their family to ensure psychosocial well-being, making sure that resident rights are maintained at all times. DIETARY - provide meals to meet each resident s need that are therapeutic, texture modified, nutritious, eye appealing and appetizing. SUPPORTIVE SERVICES - our supportive team consisting of laundry, housekeeping, and maintenance stand ready to assist the residents with their needs and requests. We believe the residents should be able to personalize their surroundings to create a homelike atmosphere. 24-HOUR LICENSED AND CERTIFIED STAFF Nurses are on duty 24 hours a day. Additional care needs are provided by Certified Nursing Assistants around the clock. Our ratio of licensed nursing is one nurse per every four residents. Our ratio of CNAs is approximately one per every 2.5 residents. One full-time Social Worker is on staff. Our Activity Department consists of one full-time Activity Director, one parttime Assistant Activity Director and three full-time Activity Aides. (Continued)

(PROFESSIONAL SERVICES CONTINUED) RESTORATIVE PROGRAM Our restorative team including nursing, therapies and dietary work to achieve and maintain the highest practical level of function focusing on what each resident can do, adapting to lost skills by teaching new skills and by maximizing independence in all ADL s. DISCHARGE PLANNING WHICH INCLUDES HOME ASSESSMENT AND EVALUATION OF EQUIPMENT NEEDS 1. Each individual is assessed for desirability and potential for returning home or to an alternate living situation. a. Physical and mental status and potential. b. Financial requirements. c. Assessment for physical environment requirements in the home or alternate living situation. 2. Alternate living situations available. 24-HOUR LAB, X-RAY, AND PHARMACEUTICAL SERVICES Services contracted with area hospitals, clinics and pharmacy providers. SPECIALIZED SERVICES AVAILABLE PHYSICAL THERAPY A licensed therapist assesses each resident with a need working on mobility of both upper and lower extremities. The therapist develops walking and exercise programs as needed. RESPIRATORY THERAPY A licensed therapist assesses the respiratory status of resident in need. He makes sure the oxygen, nebulizers and other respiratory equipment meets the needs of the resident. WOUND THERAPY Our Clinical Care Coordinator will work with each residents physician to make sure appropriate wound care is provided.

(SPECIALIZED SERVICES AVAILABLE CONTINUED) TRACHEOSTOMY, COLOSTOMY, ILEOSTOMY AND GASTROSTOMY TUBE CARE These services will be provided per physicians instructions. DIALYSIS Peritoneal - licensed staff trained in same. Hemodialysis - transportation to and from, monitoring by licensed nurses. SPEECH/LANGUAGE THERAPY A licensed therapist will assess each resident with a need, developing exercises for speech and swallowing and providing adaptations as needed. OCCUPATIONAL THERAPY A licensed therapist assesses each resident with a need, developing programs to improve dexterity and abilities to meet own care needs. I.V. THERAPY Licensed nurses under the direction of the physician will start and maintain I.V. for hydration or medication needs. RAPID RECOVERY CARE PROGRAM Short term care for persons recovering from surgery or serious injury or condition, helping them to return to independent care. RESPITE CARE Short or long term care to allow caregivers time for themselves. TRANSPORTATION 24-hour non-emergency medical transportation is available. OTHER HEALTH CARE SERVICES AS NEEDED Dental, optical, chiropractic, audiologist, dietary consultation, etc. ACCOMMODATIONS AVAILABLE DAILY/MONTHLY RATE $131.50 to $162.50 per day, dependent on level of care. Private room $141.50 to $1172.5 per day, dependent on level of care. COVERED UNDER RATE All routine services and supplies (nursing, cleaning, laundry, etc.) (Continued)

(ACCOMMODATIONS AVAILABLE CONTINUED) NON-COVERED UNDER RATE Services furnished by other providers (doctor, prescription drugs, dentist, chiropractors, dry cleaning, all therapies, transportation, etc.) IF UNABLE TO PAY RATE Assistance may be available through Wisconsin Medical Assistance Program (Medicaid). You should inquire at Rusk County Department of Social Services. Basic asset limits: $2000 cash $1500 life insurance $1500 burial trust Income (Social Security, V.A., pensions, etc.) must be applied to cost of care. Resident is allowed $40 per month for personal needs. ADDITIONAL QUESTIONS What may a resident bring with him/her? Personal clothing, pictures, chair (no rockers), small stand or dresser, radio, TV (cable provided), etc. Furniture must by ok d by administrator. Visiting hours are unrestricted; however, we encourage visiting between 8:00 a.m. and 8:00 p.m. Home visits are encouraged dependent upon the resident s condition.
