Please number each answer corresponding to the question number. Please use standard letter (8.5x11 inch) paper with black, 12-point type, printed on one side. Please submit 1 unbound or electronic copy of the full application. 1. Organizational Profile Briefly describe your organization (500 words max). Please include: a) Vision and Mandate b) Geographic Area of Service c) Brief Organizational History Board Structure and Governance Describe the role of the Board within the organization including relationship to staff, role in programming and outreach. Please indicate the frequency with which the Board meets. 2. Description of Programs and Services Provide a brief overview of programs and services offered and/or events presented, and a description of those planned for the application year. Include all artists, performances, presentations and/or exhibits as well as events, workshops, etc. Where possible, please provide dates and brief description of resource requirements as well as estimated number of participants/attendees. Describe the public impact of the organization and its programming. Please highlight any changes perceived over the previous year (e.g. trends in audience development, partnership opportunities, etc.). 4. Organizational Status Please assess the current operating climate of the organization, both internally and externally, including discussion of any opportunities or challenges that your organization is currently facing, and steps your organization is taking to respond. 5. Required Financial Information All applicants are required to submit: a) A statement indicating the organization s effective fiscal year (start and end dates) b) Financial statements signed by the appropriate Board Officer (balance sheet or statement of financial position, and income statement) for the previous three (3) years of operation. Statements prepared by an external party must also include all Notes to the Statements. c) Financial information in a comparative table, formatted as follows: Expenses LINE ITEM ACTUALS (if not complete) (if applicable) Revenue LINE ITEM ACTUALS (if not complete) (if applicable) *Notes on Required Financial Information Funds allocated to this program are derived from the 2018-2019 operating budget and the program is based upon the Municipality s fiscal year (April 2018-March 2019). Understanding that different organizations utilize different fiscal calendars, * herein refers to the previous (or current) year s budget. The current program will provide funding for fiscal years beginning in 2018 and ending in 2019. 2018-2019 Forecast is required for those organizations that are in the midst of their (equivalent) 2018-2019 fiscal calendar at the time of submission. Reports should categorize revenues and expenditures by category. Reports should list all, and differentiate between, sources of government funding (Municipal, Provincial, Federal). Reports may list in-kind expenses and must balance with listed in-kind contributions under Revenues. Notes should be included to items listed on the financial table for the purposes of explaining: Any significant change in a particular category of revenue or expense; Any category subtotals that are particularly large in comparison to other categories; Any significant deficits or surpluses listed (expected) for the coming year s budget; and Any particular items that the applicant wishes to highlight and address. Applicants whose budget for 2018-2019 (or equivalent) projects significant debt (in excess of 5% of the total annual budget for that fiscal year) must include a debt-reduction plan and a brief explanation of what steps are being taken to eliminate the shortfall. Submission Information The Deadline for Applications is June 29 th, 2018 4:30PM Applications must be received by the program officer or designate prior to the deadline as listed. If sent via Canada Post applications must be postmarked on or before June 29 th The program officer will not consider late submissions under any circumstances. Extensions to the stated deadline will not be granted.
