Missing, Misaligned, and Incomplete Treatments Missing, misaligned, and incomplete treatments for patients in CROWNWeb can cause problems with your data submission: you may be unable to complete a CMS-2728 for a new patient or the patient may not be counted on your end of the year survey (CMS-2744). Missing means that there is no treatment record associated with an admission to your unit. Misaligned means that the first and/or only treatment record associated with an admission to your unit has a different start date than the admission. Incomplete means there is a treatment record associated with an admission to your unit with the same date, but the treatment record is missing one or more mandatory fields. This guide will walk you through the steps of correcting missing, misaligned, and incomplete treatments in CROWNWeb. 6) Click on the Add Treatment tab in the gray sub-menu 7) Enter the treatment information from when the patient was first admitted to your unit 8) Click Submit NOTE: The first treatment record should have the same date as the admit date to your unit. If the patient s treatment information has changed since the admission, add another treatment by following steps 6-8 again, with a new treatment start date and new treatment information. Misaligned Treatments 1) Click the Patients tab in the main menu 2) Enter your search criteria and search for the patient 3) Click on the blue hyperlinked CROWN UPI for the patient 4) Click on the Admit/Discharge Summary tab in the gray sub-menu 2
5) Click on the blue hyperlinked Treatment Summary for your unit s admission 6) Determine which treatment, if more than one, is misaligned 7) Click on the blue hyperlinked Treatment Start Date for the misaligned treatment 3
8) Click the Edit Treatment tab in the gray sub-menu 9) Change the Treatment Start Date to the date the patient was admitted to your unit and change the treatment information (setting, type of dialysis, sessions, time, and practitioner) to reflect the patient s initial treatment prescription/order at the time of admission 10) Click Submit 11) If the patient s treatment information has permanently changed at any point between the admit date and the present date, you must add a new treatment by clicking the Add Treatment tab in the gray sub-menu once CROWNWeb returns you to the Treatment Summary screen 4
12) Enter the treatment information to reflect the patient s permanent change in setting, type of dialysis, sessions per week (if applicable), time per session (if applicable), or attending practitioner 13) Click Submit NOTE: A patient s Treatment Summary should reflect all treatment information over the patient s length of stay at your unit (i.e. from admit date to present/discharge date). If the patient s treatment information permanently changes at any time during the patient s admission to your unit, you should add a treatment record to the patient s Treatment Summary by following steps 11-13 again, with a treatment start date that reflects the first date of the patient s new treatment prescription/order. Incomplete Treatments 1) Click the Patients tab in the main menu 2) Enter your search criteria and search for the patient 3) Click on the blue hyperlinked CROWN UPI for the patient 4) Click on the Admit/Discharge Summary tab in the gray sub-menu 5) Click on the blue hyperlinked Treatment Summary for your unit s admission 5
6) Determine which treatment record, if more than one, is missing the required information by clicking the blue hyperlinked Treatment Start Date one at a time (Required: Treatment Start Date, Primary Dialysis Setting, Primary Type of Treatment, Sessions Per Week (Hemo), Time Per Session in minutes (Hemo), and Attending Practitioner) 7) Once you ve determined which one needs to be corrected, click the Edit Treatment tab in the gray sub-menu 6
8) Enter the missing information 9) Click Submit NOTE: In all of the above instances of incorrect treatment information, if the patient s modality is incenter hemo, the Expected Self-Care Setting, Type of Dialysis Training, and training date fields should be left blank; entering data in these fields when the modality is in-center hemo may cause the patient to be counted in the wrong modality setting or not counted at all on your CMS-2744. Correcting instances of inaccurate treatment information will allow CROWNWeb to accurately interpret the data and count your patients where they need to be counted. Units that are part of a Batch Submitting Organization (BSO) DaVita, FMC, DCI, NRAA should make sure the treatment information is correct in your internal system and work together with your corporate IT department or EMR Vendor to correct CROWNWeb.
