Welcome to the 2017 Philanthropy Day Awards Nomination Form! What is Philanthropy Day? Philanthropy Day is a program of the Idaho Nonprofit Center in partnership with the Association of Fundraising Professionals, Idaho Community Foundation and the Idaho Women s Charitable Foundation to recognize and celebrate the philanthropic contributions of individuals and organizations across Idaho. Philanthropy Day encompasses the spirit of community involvement, celebrates generosity, and promotes the welfare of others. Philanthropy Day Award Categories Philanthropic Company/Foundation: An outstanding Company/Foundation that demonstrates commitment to the profession, knowledge of the industry, and leadership capabilities. This Company/Foundation brings philanthropic work to the core of their mission. Collaboration: Organizations with a proven record of exceptional collaboration that work together to grow their resources, solve a problem and improve their community. Innovation: An organization that is a leader in their field, is vocal and visible in their community and sets examples or starts trends for others to follow *Outstanding Adult Philanthropist: An individual adult (21 years and older) who has demonstrated exceptional leadership in philanthropic efforts and/or projects benefiting charitable organizations. *Outstanding Youth Philanthropist: An individual youth (under 21 years) who has demonstrated exceptional leadership in philanthropic efforts and/or projects benefiting charitable organizations. *These awards exclude the president, CEO and/or executive director of a nonprofit organization. Philanthropy Day Nomination Guidelines Selection Process: Any individual or organization may submit nominations for any one or more award(s).to submit more than one nomination, you will have to submit this form more than once. The application must be completed in its entirety to be considered. Presentation of Awards: 1

Philanthropy Day awards will be presented at the 2017 Philanthropy Day events: *Tuesday, November 14th in Idaho Falls *Wednesday, November 15th in Boise *Thursday, November 16th in Coeur d Alene Submitting Award Nominations: 1) Please discuss your intention to submit an award nomination with your nominee BEFORE proceeding to ensure they want to be publicly recognized. If they are unable to attend, you may still submit a nomination. 2) Application must be completed in its entirety to be considered. This nomination process should take no longer than 10 minutes. * 1.
