Participant s Waiver of Liability Release and Assumption of Risk Dear Parent or Guardian, Your child has indicated an interest in participating in the Arkansas National Archery in the Schools Program sponsored by the Arkansas Game and Fish Commission. We know that is your wish as well as ours that every precaution is taken to protect our participants from injury. We do our utmost to promote this by proper training, by the use of protective equipment and by encouraging good safety habits. Accidents do happen in athletic events as elsewhere. Although the Arkansas Game and Fish Commission provides health and accident insurance for the Arkansas National Archery in the Schools Program, we recommend that each participant be insured with your own personal family insurance. Playing, practicing, or participating in archery activities, like other sports, involves a risk of injury. There is a further risk of injury from the use of archery equipment. The undersigned acknowledges, understands, and accepts the risks of injuries to the participant s person. The dangers and risk include, but are not limited to: death, serious bodily injury, injury to muscles, bones, joints, ligaments, tendons, and other aspects of the muscular system and other areas of the body. The undersigned further acknowledges that there may be risks and angers not known to us or unforeseeable at this time and assumes those risks. The undersigned gives his or her full consent to the participant indicated below to participate in the Arkansas National Archery in the Schools Program and agrees to waive, release, discharge, and hold harmless the Arkansas Game and Fish Commission and Valley Springs School District for any claims, damages, costs, fees, or causes of action resulting from accidents or injuries sustained during participation in any Arkansas National Archery in the Schools Program activity. This waiver is executed on behalf of the participant noted below, the parents or guardians, their heirs, successors, executors, administrators, and assigns. The undersigned hereby certifies that the participant noted below is fully capable of participating in the Arkansas National Archery in the Schools Program and that he or she has no physical or mental disabilities or infirmities that would restrict full participation in Arkansas National Archery in the Schools Program activities except as made known to coaches and officials. The laws of the State of Arkansas shall apply to this waiver and consent, as well as any legal questions relating to the Arkansas National Archery in the Schools Program. If the law of the State of Arkansas renders any part of this agreement unenforceable, the remainder of this agreement shall nevertheless remain enforceable to the fullest extent. Program Information: First and foremost, thank you for allowing your child to participate in the Valley Springs Archery program. I am amazed at the number of students that have shown an interest in the archery program. All new archers (grades 4-12) will have two days of practice October 17-18, 2017 from 3:30 pm 5:00 pm. Practice for all archers will start October 31, 2017. Time is to be determined. Paperwork must be completed and on file by Wednesday, October 24, 2017 along with the $15 registration fee to participate for the 2017-18 archery season. It can be turned in to Coach Jennings or any of the school offices. Divisions: Elementary (grades 4-5) Middle School (grades 6-8) High School (9-12 grades) Equipment: (Furnished purchasing own optional) All use Mathews Original Genesis Bows and Easton 1820 arrows which are both furnished by the school. You can choose to purchase your own bow. Bows are sold thru local dealers, Cabela s, Walmart, Bass Pro, and Academy Sports to name a few. Expense: $15 registration fee (payable to Valley Springs Archery) Tournament fees typically around $7-$8 per tournament (payable before each tournament) Criteria for selection/participation: Student must participate in at least 10 scoring practices and/or tournaments (only two tournaments scores will be used toward the 10 scores). Each practice will consist of a tournament round 1 practice at 10 meters and 3 scoring ends at 10 meters and 1 practice at 15 meters and 3 scoring ends at 15 meters. Student must participate in at least 1 invitational tournament (Valley or other listed on schedule). Student must not have discipline issues (classroom or archery practice) Student will be dismissed from the program on his/her third behavioral detention or second in school or first out of school suspension (beginning October 30 th ). If any archer misses three consecutive practices without notifying one of the coaches he/she will no longer be eligible to participate in archery. Top 24 qualifiers in each division (with appropriate girl/boy makeup) will compete at regionals. Tournament style scoring will be used. Tournament Team Scoring: Tournament team scores is calculated by using the top 4 male and 4 female scores, and then the next top 4 scores (either gender). Practice Time: Practice will be on Tuesdays & Thursdays -Final schedule will be posted by Monday, October 30th once we know the number of participants. We will shoot one group each 45 minutes. Please take into consideration any other activities you participate in when selecting time slots., ie, Jr. High basketball might want to select later times so they can practice after their games and high school basketball, cheer & band might want to pick an earlier time.
